[Rheumatoid arthritis and combined pulmonary fibrosis and emphysema].

The combination of pulmonary fibrosis and emphysema is a syndrome described in the last years, which has its own characteristics and it is not only the casual association between the two entities. The idiopathic pulmonary fibrosis is the most common type of pulmonary fibrosis. However other interstitial lung diseases could be part of this syndrome. Among them is the connective tissue disease-associated interstitial lung disease. We report a case of this syndrome associated with rheumatoid arthritis. It has the peculiarity that the connective disease became overt several years after the present

Pulmonary interstitial emphysema in an adult with metastatic choriocarcinoma.

Although typically seen in neonates, barotrauma is a well-established complication of mechanical ventilation in other populations as well, particularly associated with the use of high peak pressures and continuous positive airway pressure. Complications range from subpleural bleb development to life-threatening pneumothorax. We report a case of barotrauma in an adult that initially presented as pulmonary interstitial emphysema; it was seen surrounding choriocarcinoma metastases during computed tomography evaluation for worsening respiratory distress.

Combined pulmonary fibrosis and emphysema syndrome: a review.

There is increasing clinical, radiologic, and pathologic recognition of the coexistence of emphysema and pulmonary fibrosis in the same patient, resulting in a clinical syndrome known as combined pulmonary fibrosis and emphysema (CPFE) that is characterized by dyspnea, upper-lobe emphysema, lower-lobe fibrosis, and abnormalities of gas exchange. This syndrome frequently is complicated by pulmonary hypertension, acute lung injury, and lung cancer. The CPFE syndrome typically occurs in male smokers, and the mortality associated with this condition, especially if pulmonary hypertension is present

CT assessment of subtypes of pulmonary emphysema in females.

We performed a retrospective study examining the prevalence and subtypes of pulmonary emphysema (PE) identified by computed tomography (CT) in females. We reviewed the records of 1,687 female subjects who had undergone CT. They were divided into the following 2 age groups:group A (<50 years) and group B (≥50 years). PE was diagnosed by the presence of low-attenuation areas using visual assessment (grades 0-3) on CT images. Two subtypes of PE were observed:centrilobular emphysema (CLE) and paraseptal emphysema (PSE). PE was divided into the following 3 categories:I (CLE or CLE-predominant);II (…

Persistent pulmonary interstitial emphysema in a preterm infant.

Persistent pulmonary interstitial emphysema is a chronic disease reported in mechanically ventilated premature newborns. We describe a case of localized persistent pulmonary interstitial emphysema in a preterm infant without mechanical ventilation but on continuous positive airway pressure using nasal prongs. The condition resolved without surgery.

A case of localized persistent interstitial pulmonary emphysema.

Interstitial pulmonary emphysema is a well-documented complication of assisted mechanical ventilation in premature infants with respiratory distress syndrome. Localized persistent interstitial pulmonary emphysema (LPIPE) confined to a single lobe was incidentally presented in a 4-day-old female infant. This patient was a normal full-term baby with no respiratory distress symptom and no experience of assisted mechanical ventilation. Chest radiograph showed radiolucent area in right lower lobe zone, which needed differential diagnosis from other congenital lesions such as congenital cystic adeno…

Dexamethasone for pulmonary interstitial emphysema in preterm infants.

The efficacy of a 3-day course of dexamethasone (0.5 mg/kg/ day) in 10 preterm infants (< or = 30 weeks gestation) with pulmonary interstitial emphysema (PIE) was studied in a retrospective case review. PIE was diagnosed at a median age of 7.5 days and treatment with dexamethasone began at 8.5 days. Seven of the 10 subjects had at least 2 days of conservative treatment (lowered mean airway pressure) preceding dexamethasone during which the mean airway pressure (MAP), oxygenation index (OI) and mechanical ventilation index (MVI) were not significantly different although within 3 days of dexamet

Effect of pulmonary emphysema on diaphragm capillary geometry.

In emphysema, the diaphragm shortens by losing sarcomeres. We hypothesized that unless capillaries undergo a similar shortening, capillary geometry must be altered. Without quantifying this geometry, capillary length and surface area per fiber volume, which are critical measurements of the structural potential for blood-tissue exchange, cannot be resolved. Five months after intratracheal elastase (E) or saline (control; C) instillation, diaphragms from male Syrian golden hamsters were glutaraldehyde perfusion fixed in situ at reference lung positions (residual volume, functional residual capac

Pulmonary hyperinflation and emphysema in infants with the Marfan syndrome.

The Marfan syndrome is a dominantly inherited disorder of type I collagen with well recognized skeletal, cardiac and ophthalmological manifestations. Less recognized are the pulmonary manifestations of the disease. Although chronic pulmonary emphysema, interstitial lung disease with honey combing and spontaneous pneumothorax have been sporadically reported, there is no documentation of these abnormalities in the radiological literature. We describe the clinical and radiographic findings in the lungs of four infants with Marfan's syndrome and provide pathologic correlation from autopsy specimen…

Combined Pulmonary Fibrosis and Emphysema Preceding Lupus Pleuritis.

An 83-year-old man, who was a former smoker, with anti-ribonucleoprotein (RNP) antibody-positive combined pulmonary fibrosis and emphysema presented with a cough and dyspnea. A chest radiograph showed bilateral pleural effusions. His laboratory data showed proteinuria and elevated levels of anti-nuclear antibodies, anti-double strand DNA antibodies, and CA125, with decreased serum complement levels. Thoracentesis showed an exudative pleural effusion with an increased lymphocyte count and elevated CA125 levels. A thoracoscopic biopsy specimen showed proliferation of CA125-positive mesothelial c

[Minimally invasive surgery of large bullous pulmonary emphysema].

An enormous pulmonary cyst (phi approx. 20 cm) in a 72-year-old male patient with large bullous pulmonary emphysema caused compression of the right lung and the mediastinum with consecutive, O2-dependent dyspnoea at rest. The symptoms did not improve under conservative therapy of an accompanying COPD. Thus, in spite of two heart attacks in the previous history, an operation with bullectomy was indicated. A thoracotomy had to be avoided because of the very high cardiac risk. In the present case, a minimally invasive procedure enabled the complete cyst resection with a smooth postoperative cours…

High frequency jet ventilation in experimental pulmonary emphysema.

The effects of high frequency jet ventilation (HFJV, f = 2 Hz and 8 Hz, I:E = 0.43, FiO2 = 0.4) were studied and compared with intermittent positive pressure ventilation (IPPV, f = 10-14 breaths/min, VT = 15 ml/kg, I:E = 0.5, FiO2 = 0.4) in 8 dogs before and after induction of panlobular emphysema (PLE). PLE increased alveolar-arterial PO2 difference (PA-aO2) during all modes of ventilation, whereas PaCO2 did not change significantly. In both periods of the study, HFJV8 Hz was less effective in terms of CO2-elimination and oxygenation. In the control-period, functional residual capacity (FRC)

Constant-flow ventilation in canine experimental pulmonary emphysema.

The efficacy of constant-flow ventilation (CFV) was investigated in eight mongrel dogs before (control-phase) and after development of papain-induced panlobular emphysema (PLE-phase). For CFV, heated, humidified and oxygen-enriched air was continuously delivered via two catheters positioned within each mainstem bronchus at flow rates (V) of 0.33, 0.5 and 0.66 l/s. Data obtained during intermittent positive pressure ventilation (IPPV) served as reference. In the control-phase, Pao2 was lower (P less than or equal to 0.05) and alveolo-arterial O2 difference (P(A-a)O2) was higher (P less than or

Localized pulmonary interstitial emphysema: treatment by bronchial occlusion.

Selective bronchial occlusion for the treatment of localized cystic pulmonary interstitial emphysema (PIE) offers a means to facilitate weaning from ventilatory support and to preserve potentially functional lung parenchyma. We employed a commercially manufactured balloon catheter, placed under direct visualization to occlude a left mainstem bronchus in two cases of localized PIE. This is the safest method for selective bronchial occlusion so far reported. Therapeutic success was achieved with either continuous or intermittent bronchial occlusion. Evaluation of pulmonary function with nuclear
