The Application of Pulmonary Ultrasound in Neonatal Ventilator-Associated Pneumonia.

This study explored the application value of lung ultrasound (LUS) in neonatal ventilator-associated pneumonia (NVAP). In this study, 122 newborns suspected of NVAP were treated in the NICU of Liaocheng People's Hospital between July 1, 2020, and July 1, 2021. Of these, 115 were clinically diagnosed with NVAP. The diagnostic value of LUS for NVAP was determined by comparing the different signs of LUS and chest X-ray (CXR). The confirmed cases were divided into the failure and success groups according to the first ventilator weaning test results. The consistency between the results of LUS and C

Invasive Pulmonary Aspergillosis in Hospital and Ventilator-Associated Pneumonias.

Pneumonia is the commonest nosocomial infection complicating hospital stay, with both non-ventilated hospital-acquired pneumonia (HAP) and ventilator-associated pneumonia (VAP) occurring frequently amongst patients in intensive care. Aspergillus is an increasingly recognized pathogen amongst patients with HAP and VAP, and is associated with significantly increased mortality if left untreated.Invasive pulmonary aspergillosis (IPA) was originally identified in patients who had been profoundly immunosuppressed, however, this disease can also occur in patients with relative immunosuppression such

Pulmonary Infections in Ventilated Patients: Diagnostic and Therapeutic Options.

The diagnosis of pulmonary infections in the ventilated patient has threatened the foundations of medicine. Although the lifesaving techniques of endotracheal intubation (developed for the treatment of diphtheria) and artificial ventilation (developed for the management of poliomyelitis) contribute greatly to medical care, they have resulted in the production of the "progress"-related infection of ventilator-associated pneumonia (VAP). Modern ventilator therapy is a substantial technologic advance from earlier days and, as technology inherently does, has removed some of the human element, the

Testing pulmonary physiology in ventilated non-human primates.

BACKGROUND: Animal models of respiratory viral infections are essential for investigating disease pathogenesis and the efficacy of antivirals and vaccine candidates. A major limitation in the research of respiratory diseases in animal models is correlating clinically relevant changes in pulmonary physiology with cellular and molecular mechanistic studies. Few animal models have captured and correlated physiologic changes in lung function and immune response within same experiment, which is critical given the heterogeneous nature of lung disease due to viral infections. In ventilated human pati

A Prospective, Comparative Study of Ventilation-Perfusion Planar Imaging and Ventilation-Perfusion SPECT for Chronic Thromboembolic Pulmonary Hypertension.

This study compared the diagnostic performance of ventilation-perfusion (V/Q) planar imaging and V/Q SPECT and determined whether combining perfusion SPECT with low-dose CT (Q-LDCT) may be equally effective in a prospective study of patients with chronic thromboembolic pulmonary hypertension (CTEPH). V/Q scanning is recommended for excluding CTEPH during the diagnosis of pulmonary hypertension (PH). However, V/Q planar imaging and V/Q SPECT have yet to be compared in patients with CTEPH. Methods: Patients with suspected PH were eligible for the study. PH attributable to left heart disease or l

[Expert consensus on the operational protocol for radionuclide pulmonary ventilation/perfusion scintigraphy of pulmonary thromboembolism].

Radionuclide pulmonary ventilation/perfusion (V/Q) imaging is an important non-invasive tool for diagnosing pulmonary thromboembolism (PTE) and is used for screening, follow-up, and assessment of treatment response of PTE and chronic thromboembolic pulmonary hypertension (CTEPH). Currently, hospitals in China differ in their use of planar and tomographic acquisition methods for V/Q imaging, and interpretation requires a high level of expertise among nuclear medicine physicians. After surveying the nuclear medicine departments of multiple hospitals and holding expert discussions, the working gr…

Independent ventilation and ECMO for severe unilateral pulmonary edema after SLT for primary pulmonary hypertension.

Single lung transplantation (SLT) is now accepted therapy for selected cases of severe pulmonary hypertension. A recognized complication is the postoperative development of reperfusion edema in the graft, a potentially fatal cause of respiratory failure. Because reperfusion edema may be a reversible process, temporizing support measures can be life-saving. We report the case of a 48-year-old woman who developed severe reperfusion edema following right SLT for primary (unexplained) pulmonary hypertension. Extracorporeal membrane oxygenation (ECMO) was instituted. Independent lung ventilation wa

High-frequency oscillatory ventilation versus conventional ventilation for infants with severe pulmonary dysfunction born at or near term.

RATIONALE: Pulmonary disease is a common cause of morbidity and mortality in term and near-term infants. For several decades, conventional ventilation (CV) has been widely used to manage pulmonary dysfunction. High-frequency oscillatory ventilation (HFOV) may prove to be more effective than CV in these infants. However, with recent innovations in CV, it remains unclear whether HFOV is more effective than CV in term or near-term infants with severe pulmonary dysfunction. This is an update of a review previously conducted in 2009. OBJECTIVES: To determine the effect of HFOV compared with CV on

Comparison of pulmonary ventilation/perfusion tomography and conventional diagnostic techniques in the diagnosis of pulmonary thromboembolism.

BACKGROUND: Pulmonary thromboembolism (PTE) is a common and potentially fatal cardiopulmonary vascular disease. Early and accurate diagnosis is crucial for improving patient prognosis. Computed tomography pulmonary angiography (CTPA) is widely used as a first-line imaging modality, while the clinical value of pulmonary ventilation/perfusion single-photon emission computed tomography (V/Q SPECT) in specific patient populations is increasingly recognized. However, systematic evidence comparing the diagnostic performance and corresponding patient clinical phenotypes across different imaging techn…

Pulmonary issues in acute and critical care: pulmonary embolism and ventilator-induced lung injury.

Many patients admitted to acute care hospitals are at risk for VTE. Nurses play a pivotal role in prevention of VTE events by assessing risk and implementing prophylactic interventions, promptly recognizing and reacting to signs and symptoms of DVT and PE, and collaborating with other team members to ensure rapid treatment ensues. When patients require mechanical ventilation, nurses need to remain alert for complications indicative of VILI, effectively communicate assessment findings to other team members and confidently implement nursing and ordered medical interventions to promote the best p

Closure of the ductus arteriosus with indomethacin in ventilated neonates with respiratory distress syndrome. Effects of pulmonary compliance and ventilation.

The reported effects of indomethacin on pulmonary compliance are variable depending upon the patient population and on the degree to which indomethacin resulted in successful ductal closure. Eleven fluid-restricted, furosemide-treated premature infants being mechanically ventilated for respiratory distress syndrome (RDS) who also had a significant patent ductus arteriosus (PDA) had pulmonary function testing performed before and after successful closure of the PDA. The diagnosis of a significant PDA was made by clinical and echocardiographic criteria. Indomethacin was administered at a dosage

Changes in the distribution of ventilation and perfusion associated with separation from mechanical ventilation in patients with obstructive pulmonary disease.

A trial of separation from mechanical ventilation may induce an abnormal respiratory pattern and a maldistribution of ventilation-to-perfusion ratios (VA/Q), especially in patients with chronic obstructive pulmonary disease. This study was designed to assess the effects of three different modes of ventilation on the distribution of global and also regional VA/Q in eight patients with chronic obstructive pulmonary disease recovering from acute respiratory failure who remained dependent on mechanical ventilation after more than 5 days of attempted separation from the ventilator. VA/Q distributio

Elective high-frequency oscillatory ventilation versus conventional ventilation in preterm infants with pulmonary dysfunction: systematic review and meta-analyses.

OBJECTIVES: To systematically review the evidence to determine whether the routine use of high-frequency oscillatory ventilation (HFOV) as compared with conventional ventilation (CV) is beneficial or harmful in preterm infants requiring mechanical ventilation for pulmonary failure principally due to respiratory distress syndrome. METHODS: All randomized controlled trials of elective HFOV versus CV in preterm infants <36 weeks' gestation with respiratory failure mainly attributable to respiratory distress syndrome were identified from the literature through a search of MEDLINE, EMBASE, Oxford

Multicenter controlled trial comparing high-frequency jet ventilation and conventional mechanical ventilation in newborn infants with pulmonary interstitial emphysema.

One hundred forty-four newborn infants with pulmonary interstitial emphysema were stratified by weight and severity of illness, and randomly assigned to receive treatment with high-frequency jet ventilation (HFJV) or rapid-rate conventional mechanical ventilation (CV) with short inspiratory time. If criteria for treatment failure were met, crossover to the alternate ventilatory mode was permitted. Overall, 45 (61%) of 74 infants met treatment success criteria with HFJV compared with 26 (37%) of 70 treated with CV (p less than 0.01). Eighty-four percent of patients who crossed over from CV to H
