Hydrodynamics of a Confined Surfactant-Laden Compound Droplet under Steady and Pulsatile Flows.

Compound droplets, consisting of a liquid core encapsulated within another liquid shell, find extensive applications in the food, pharmaceutical, and cosmetic industries to encapsulate nutrients, flavors, and active ingredients, as well as in drug delivery and the fabrication of porous particles and functional materials. These compound droplets are generally stabilized by surfactants, which increase interfacial deformability, thereby preventing coalescence. Motivated by these applications, this study numerically investigates the dynamics of a deformable compound droplet laden with an insoluble

An effective case of pulsatile flow for cerebral malperfusion of stanford type A aortic dissection.

The surgical management of preoperative malperfusion poses considerable challenges, particularly in cases of acute type A aortic dissection … ue oxygenation index (TOI) and the oxyhemoglobin change rate (ΔHbO2). To mitigate the malperfusion, pulsatile flow (PF) was utilized during CPB. This report elucidates the meticulous application of PF during CPB in the management of this complex malperfusion scenario, culminating in a favorable postoperative outcome.

Hemodynamic Effect of Pulsatile on Blood Flow Distribution with VA ECMO: A Numerical Study.

The pulsatile properties of arterial flow and pressure have been thought to be important. Nevertheless, a gap still exists in the hemodynamic effect of pulsatile flow in improving blood flow distribution of veno-arterial extracorporeal membrane oxygenation (VA ECMO) supported by the circulatory system. The finite-element models, consisting of the aorta, VA ECMO, and intra-aortic balloon pump (IABP) are proposed for fluid-structure interaction calculation of the mechanical response. Group A is cardiogenic shock with 1.5 L/min of cardiac output. Group B is cardiogenic shock with VA ECMO. Group C…

Blood Pressure, Carotid Flow Pulsatility, and the Risk of Stroke: A Community-Based Study.

BACKGROUND AND PURPOSE: High blood pressure is a major cause of cardiovascular events, and carotid flow pulsatility may be associated with cardiovascular events. However, the combined effect of blood pressure and flow pulsatility on the development of stroke remains unclear. Therefore, we investigated the combined influence of central blood pressure and pulsatility index (PI) on the incidence of stroke. METHODS: Baseline data from 2033 adults (≥30 years) without stroke history in the Cardiovascular Disease Risk Factor Two-Township Study were linked to incident stroke. Common carotid flow PI w

Application of large-eddy simulation to the study of pulsatile flow in a modeled arterial stenosis.

The technique of large-eddy simulation (LES) has been applied to the study of pulsatile flow through a modeled arterial stenosis. A simple stenosis model has been used that consists of a one-sided 50 percent semicircular constriction in a planar channel. The inlet volume flux is varied sinusoidally in time in a manner similar to the laminar flow simulations of Tutty (1992). LES is used to compute flow at a peak Reynolds number of 2000 and a Strouhal number of 0.024. At this Reynolds number, the flow downstream of the stenosis transitions to turbulence and exhibits all the classic features of p…

Sleep Quality after Coronary Artery Bypass Graft Surgery: Comparing Pulsatile and Nonpulsatile Pump Flow.

Purpose  Turner syndrome is a sex chromosomal aberration where majority of the patients have 45,X karyotype, while several patients are mosaic involving 45,X/46,XX; 46,X,i(Xq); and other variants. Cytogenetic analysis, karyotyping, is considered to be the "gold standard" to detect numerical and structural chromosomal abnormalities. In the recent years, alternative approaches, such as array comparative genomic hybridization (aCGH), have been widely used in genetic analysis to detect numerical abnormalities as well as unbalanced structural rearrangements. In this study, we report the use of kary

Pulsatile venous Doppler flow in lower limbs: highly indicative of elevated right atrium pressure.

OBJECTIVE: The purpose of this study was to determine if pulsatile flow in lower limbs as seen on venous Doppler waveforms correlates with increased right atrium pressure. MATERIALS AND METHODS: Of 429 patients who, over a 14-month period, underwent venous Doppler imaging of the lower limb to exclude deep venous thrombosis, 343 records were available for review at the time of the study. Of these, 74 had right atrium pressure measurements available for correlation. Seventeen patients were excluded because of thrombosis in the common femoral vein. Six other patients were also excluded because t…

[Detection of cerebral microembolisms during extracorporeal circulation with pulsatile flow using transcranial Doppler monitoring].

The incidence of focal neurological deficits and diffuse neuropsychiatric disorders in patients undergoing open-heart surgery still remains unacceptably high. The consensus is increasing that diffuse microembolism is the most important cause. Changes in blood flow velocity in the middle cerebral artery were investigated by transcranial Doppler sonography in 26 patients regarding the effect of continuous and pulsatile perfusion during extracorporeal circulation. The quotient of systolic and diastolic flow velocity was significantly increased in patients with pulsatile perfusion. Abnormal high-f

Comparison of continuous-flow and pulsatile-flow blood pumps on reducing pulmonary artery pressure in patients with fixed pulmonary hypertension.

Pulmonary hypertension (PH) is considered as a risk factor for morbidity and mortality in patients undergoing heart transplantation. Recently, left ventricular assist device (LVAD) implantation has been increasingly used in reducing pulmonary artery pressure (PAP) in patients with PH unresponsive to medical therapy. Herein, we aimed to compare the efficacy of continuous-flow and pulsatile-flow blood pumps on the improvement of PH in mechanical circulatory support patients. Twenty-seven patients with end-stage heart failure who underwent LVAD implantation surgery were enrolled. Fifteen of them

Flow patterns in the venous sinus of pulsatile tinnitus patients with transverse sinus stenosis and underlying vortical flow as a causative factor.

BACKGROUND: Transverse sinus stenosis (TSS) is commonly found in Pulsatile Tinnitus (PT) patients. Vortex flow is prominent in venous sinus with stenosis, and so it is important to determine the distribution and strength of the vortical flow to understand its influence on the occurrence of PT. METHODS: In this study, by using computational fluid dynamics for hemodynamic analysis in patient-specific geometries based on Magnetic Resonance Imaging (MRI), we have investigated the blood flow within the venous sinus of 16 subjects with PT. We have employed both laminar and turbulent flow models for

In Silico Analysis of Pulsatile Flow Veno-Arterial Extracorporeal Membrane Oxygenation on Human Aorta Model.

Electrocardiogram (ECG)-synchronized pulsatile veno-arterial extracorporeal membrane oxygenation (V-A ECMO) is a recent development in extracorporeal therapy for patients with severe cardiogenic shock. Although preclinical studies have shown benefits of pulsatile flow relative to continuous ECMO flow, none have explored the effects of the timing of ECMO pulses with respect to the cardiac cycle and its possible implications on ECMO complications. This study aimed to develop a computational fluid dynamics (CFD) model of V-A ECMO in a patient-specific human aorta and evaluate the effect of ECMO t…

Pulsatile flow in descending aorta: Can coarctation of aorta be ruled out by transesophageal echocardiography?

A 5-year-old child with L posed great arteries, large subpulmonic ventricular septal defect (VSD), atrial septal defect (ASD), and a large patent ductus arteriosus (PDA) with mild isthmic narrowing was scheduled for surgical correction. Intraoperatively, it was a case of anatomically corrected malposition of great arteries. Due to abnormal positioning of great vessels, the isthmus was ligated instead of the large PDA. The postoperative transesophageal echocardiography showed pulsatile flow in descending aorta as it was being filled by large PDA, and thus iatrogenic coarctation (CoA) was missed…

High-speed visualization of disturbed pathlines in axial flow ventricular assist device under pulsatile conditions.

OBJECTIVE: To investigate potentially prothrombotic flow patterns within an axial flow ventricular assist device under clinically relevant pulsatile hemodynamic conditions. METHODS: A transparent replica of the HeartMate-II left ventricular assist device (Thoratec, Pleasanton, Calif) was visualized using a high speed camera at both low and high frame rates (125 and 3000 fps). Three steady-state conditions were studied: nominal (4.5 lpm), low flow (3.0 lpm), and high flow (6.0 lpm). Time-varying conditions were introduced with an external pulsatile pump that modulated the flow rate by approxim

Differential impact on post-transplant outcomes between pulsatile- and continuous-flow left ventricular assist devices.

BACKGROUND: The HeartMate II (HMII) left ventricular assist device (LVAD) has proven reliable and durable and has become the preferred choice for bridge to transplant therapy (BTT) when compared with the pulsatile HeartMate XVE (XVE). In this study, we compared the post-transplant (PTx) outcomes between XVE and HMII using a large national data registry. METHODS: The Organ Procurement and Transplantation Network (OPTN)/United Network for Organ Sharing (UNOS) Thoracic Registry database was queried for all patients implanted with either an XVE or an HMII as BTT during 2004-2009. Numerical analysis of pulsatile blood flow and vessel wall mechanics in different degrees of stenoses.

Hemodynamics factors and biomechanical forces play key roles in atherogenesis, plaque development and final rupture. In this paper, we investigated the flow field and stress field for different degrees of stenoses under physiological conditions. Disease is modelled as axisymmetric cosine shape stenoses with varying diameter reductions of 30%, 50% and 70%, respectively. A simulation model which incorporates fluid-structure interaction, a turbulence model and realistic boundary conditions has been developed. The results show that wall motion is constrained at the throat by 60% for the 30% stenos

Physiological basis of flow dependence of Gorlin formula valve area in aortic stenosis: analysis using an hydraulic model of pulsatile flow.

BACKGROUND AND AIM OF THE STUDY: The study aim was to clarify the basis of the cardiac output dependence of aortic valve area calculated with the Gorlin formula which has been reported in patients with aortic stenosis. Clinical and experimental studies which have attempted to differentiate between a change in physical orifice area, versus a defect in the Gorlin formula as the cause of cardiac output related variations in Gorlin valve area in aortic stenosis have yielded conflicting results. METHODS: We employed a numerical model of pulsatile flow in which the total instantaneous transvalvular…

Experimental and computational study of pulsatile flow characteristics in Romanesque and gothic aortic arch models.

Romanesque and Gothic are two types of deformed aortic arch geometries after surgical repair of coarctation of the aorta. The abnormal arch geometry and hemodynamics are associated with late systemic hypertension, aortic aneurysms, and other cardiovascular complications. To understand the fluid dynamic signatures of such flow, a combined experimental and computational fluid dynamic (CFD) study has been conducted to quantitatively compare the main (axial) and secondary flow characteristics. In the experiments, a pulsatile flow simulator was used to generate the pulsatile flow conditions. Phase-…
