The assessment of pylorus in the diagnostic procedure of causes of vomiting in infants is very important. The differentiation between organic and functional changes is principal. The survey covers 65 infants aged from 2 weeks to 12 months. In this group there are 14 vomiting neonates, in 6 of them the hypertrophic pyloric stenosis by ultrasound examination was diagnosed and by operation confirmed. A patient with a recurrent achalasia of the cardia was found, after a Heller cardiomyotomy, also to have pyloric insufficiency necessitating gastric resection. The pylorus appeared to contain virtually no ganglia at all. On the basis of an experiment with rabbits, in which vagotomy was shown to have no appreciable effect on the ganglion cells in the pylorus, it was concluded that the abnormality demonstrated in this patient could not be attributed to a possible injury of the vagus nerves but was probably due instead to achalasia of the pylorus.

In vitro force-length curves of rings from the gastroduodenal junction were obtained in standard tyrode solution, in tyrode with 140 mM KCl, and in calcium-free tyrode with 5 mM sodium EDTA, to determine basal, total, and passive forces, respectively. A 20-day old female with symptoms of intermittent gastric outlet obstruction since the third day after birth was subjected to surgical exploration. Laparotomy revealed duplication of the pylorus communicating with a normal gastric cavity. The patient was successfully treated by excising the wall separating the duplication from the pylorus. Five cases of pyloric duplication reported so far in the English literature are reviewed.

In cases when the pylorus area is significantly modified by the presence of the ulcerative lesions, frequently associated with hypertrophy of the pyloric sphincter, which does not allow to carry out in satisfactory conditions pyloroplasty of the Heinecke-Mikulicz type, the author introduces an improvement of the above mentioned technique by adding anterior sphincterectomy, obtained by a "cross like" incision of the pylorus. The final suture of the pyloroplasty is exclusively of the transversal type, in a single or in a double layer, when the lumina of the pylorus allows for such a suture.

Gastric hamartomata, alternatively called adenomyomata, are rare, benign lesions of the gastric wall. They can present with a wide spectrum of symptomatology. Pure pyloric adenomyomata are exceedingly rare. We are reporting a 13-day-old infant with a gastric hamartoma at the pylorus presenting with gastric outlet obstruction mimicking infantile hypertrophic pyloric stenosis.
