Distinguishing Radiculopathies from Mononeuropathies.

Identifying "where is the lesion" is particularly important in the approach to the patient with focal dysfunction where a peripheral localiz … cation of the described method to the common scenario of distinguishing radial neuropathy versus C7 radiculopathy in the setting of a wrist drop and provides additional examples for self-evaluation and reference.

Evaluating the patient with suspected radiculopathy.

Persons with back, neck, and limb symptoms are commonly seen by health care providers. They constitute a major referral population to specialists in electrodiagnostic medicine. The evaluation of these patients involves consideration of both the common and less common disorders. The electrodiagnostic examination with needle electromyography is the most important means of testing for radiculopathy. This test has modest sensitivity but high specificity. It complements imaging of the spine. Electromyography in combination with nerve conduction testing is valuable in excluding entrapment neuropathi…

Bilateral sacral radiculopathy in a cyclist.

UNLABELLED: OBJECTIVE-PURPOSE: The purpose of this case report is to describe a gait disorder presenting as a bilateral sacral radiculopathy after vigorous cycling. Also, we discuss the pathogenic mechanisms and we revise the bibliography. PATIENT AND METHODS: The patient complained of a rapid painless weakness in legs, after intense and prolonged cycling 4 months ago. The physical and electromyographical examinations revealed important weakness in foot and knee flexors, and signs of acute denervation with mixed reinnervation (active and chronic) in myotomal S1 muscles, respectively. The lumb…

The Spurling test and cervical radiculopathy.

STUDY DESIGN: A cross-sectional study design was used. OBJECTIVE: To determine the sensitivity and specificity of the Spurling test for cervical radiculopathy. SUMMARY OF BACKGROUND DATA: The Spurling test is an accepted physical examination test, but there is little data on its sensitivity or specificity. METHODS: From 1988 to 1993, 255 consecutive patients were referred for electrodiagnosis of upper extremity nerve disorders. A Spurling test administered before other testing was performed. The Spurling test was scored as positive if it caused pain or tingling that started in the shoulder …

Electrodiagnostic examination of lumbosacral radiculopathies.

To determine the diagnostic efficacy of late responses and of magnetic stimulation in the electrodiagnostic evaluation of lumbosacral radiculopathies, 42 patients with acute monoradiculopathies of L5 or S1 were examined. We performed conventional nerve conduction studies, F-wave studies, needle electrode examination (NEE) and magnetic stimulation. The results were compared with a control group of 36 persons. In the patients with weakness, we found a diagnostic sensitivity for NEE of 90% in L5 and of 80% in S1. F-waves had the same sensitivity as NEE in the patients with weakness and were more

Radiculopathy as a complication of ankylosing spondylitis.

A cauda equina syndrome in association with ankylosing spondylitis is well recorded. We report the occurrence of an upper limb radiculopathy which progressed to a cauda equina syndrome in association with ankylosing spondylitis.

Diagnosis and Treatment of C4 Radiculopathy

STUDY DESIGN: Clinical case series. OBJECTIVE: This study sought to clarify symptoms, diagnostic criteria, and treatment of C4 radiculopathy, and the role of diagnostic C4 root block in this entity. SUMMARY OF BACKGROUND DATA: Although well understood cervical dermatomal/myotomal syndromes have been described for symptoms originating from impingement on the C2, C3, C5, C6, C7, and C8 roots, less has been written about the syndrome(s) associated with the C4 root. METHODS: The senior author reviewed surgical records and describes his personal experience with the diagnosis and treatment of C4 …

Evaluation and treatment of cervical radiculopathy.

The evaluation and treatment options for cervical radiculopathy continue to evolve as it becomes better defined and gaps in the literature are identified. This article provides a current synopsis of the definition, history, physical examination, imaging, and treatment options available.

Approach to the patient with suspected radiculopathy.

Radiculopathy is a common neurologic disorder. Electrodiagnosis can provide a physiologic assessment of the localization, degree of axon loss, severity, and chronicity of the intraspinal canal lesion, and distinguish it from other neuromuscular disorders. This article reviews electrodiagnostic aspects related to evaluating patients with suspected radiculopathies.

Mechanical diagnosis and therapy for radiculopathy.

Despite its classic clinical presentation, and even when confirmed by compatible imaging findings of a herniated disc, the radiculopathy diagnosis provides only limited assistance for making decisions about treatment. The 2003 Medicare data revealed an eightfold variation in the rates of lumbar laminectomy and discectomy across geographic regions. In an effort to address this uncertainty in care, this article describes the management paradigm known as Mechanical Diagnosis and Therapy and its usefulness in decision-making for patients with lumbar radiculopathies, and reviews the relevant litera…

Surgery for cervical radiculopathy or myelopathy.

BACKGROUND: Cervical spondylosis causes pain and disability by compressing the spinal cord or roots. Surgery to relieve the compression may reduce the pain and disability, but is associated with a small but definite risk. . OBJECTIVES: To determine whether: 1) surgical treatment of cervical radiculopathy or myelopathy is associated with improved outcome, compared with conservative management and 2) timing of surgery (immediate or delayed pending persistence/progression of relevant symptoms and signs) has an impact on outcome. SEARCH STRATEGY: We searched CENTRAL, MEDLINE, and EMBASE to 1998 …

The electrodiagnosis of cervical and lumbosacral radiculopathy.

This article reviews the usefulness of the electrodiagnostic examination in patients who have suspected cervical and lumbosacral radiculopathy. This study can verify the presence and severity of radiculopathy, determine which levels are involved, and provide an electrodiagnostic correlate to imaging abnormalities. A practical approach for conducting the nerve conduction portion and needle electrode examination in these patients is discussed.
