Synchronous hybrid procedure combining interventional radiology and endoscopy for esophagogastric varices with large gastro-renal shunt.

Successful treatment of esophagogastric varices (EGV) with giant portal-systemic shunt is challenging. To explore the feasibility and safety of a novel hybrid procedure involving interventional radiology and endoscopy in the same sitting.Three cases clinically diagnosed to have decompensated cirrhosis and EGV with giant gastrorenal shunt (GRS) on contrast-enhanced computed tomography (CT) were included. The hybrid procedures included: indirect portography, hepatic vein pressure gradient (HVPG) measurement, HVPG-based partial splenic embolization (PSE), retrospective GRS balloon occlusion, endo

CIRSE Standards of Practice for the Interventional Radiology Management of Acute and Chronic Arterial Mesenteric Ischaemia.

PURPOSE: Mesenteric ischaemia due to arterial occlusion, usually secondary to atherosclerosis or arterial thromboembolism, may be acute or chronic. Endovascular intervention is now the primary treatment based on its minimally invasive nature and lower in-hospital mortality. Open surgery is indicated in the presence of bowel necrosis, the diagnosis of which remains challenging. This document makes best-practice recommendations about the interventional radiology management of acute and chronic mesenteric ischaemia. METHODS: The CIRSE Standards of Practice Committee established a writing group o

Key Osteoarthritis Biomarkers: What Interventional Radiologists Performing Genicular Artery Embolization Need to Know.

Knee osteoarthritis (OA) is a major cause of pain and disability. Genicular artery embolization (GAE) is a promising alternative for patients unresponsive to conservative treatment or unsuitable for total knee replacement. GAE may alleviate knee pain by reducing synovitis and by causing ischemia to pathologic knee nerves. However, outcomes vary based on baseline MRI and radiographic findings, making patient selection challenging. Recently, reduction in synovitis on contrast-enhanced MRI has been associated with pain relief post-GAE. Novel OA phenotypes (inflammatory, cartilage, bone, and atrop

Percutaneous image-guided mesenteric biopsy: how we do it in a high-volume training center.

Lesions in the mesentery are unique from other potential biopsy targets in the abdomen or pelvis for several reasons. Mesenteric lesions are among the deepest in the abdomen and are often surrounded by or adjacent to small bowel or colon. Mesenteric vasculature is often crowded, and traversing the mesentery often involves crossing multiple vascular planes. Mesenteric lesions and the structures surrounding them within the peritoneal cavity are often highly mobile. All these features can be daunting to any radiologist asked to perform a mesenteric biopsy. We provide a comprehensive overview and

Renal transplant complications: diagnostic and therapeutic role of radiology.

Kidney was the first and is the most frequently transplanted organ. Despite improved surgical techniques and transplantation technology, complications do occur and, if left untreated, may lead to catastrophic consequences. Renal transplantation complications may be vascular (eg, renal artery and vein stenosis and thrombosis, arteriovenous fistula, and pseudoaneurysms); urologic (eg, urinary obstruction and leak, and peritransplantation fluid collections, including hematoma, seroma, lymphocele, and abscess formation); and nephrogenic, including acute tubular necrosis, graft rejection, chronic a

Venous interventions in children.

Advanced medical treatment options have improved pediatric survival but often require invasive vascular procedures or venous access. These procedures increase the risk for thromboembolism in children, and there has been a corresponding increase in the reported incidence of deep venous thrombosis and postthrombotic syndrome in the pediatric population. Percutaneous venous interventions using catheter-directed therapy (CDT), like mechanical thrombectomy and infusion thrombolysis, have been used much less frequently in children, even though they have shown good results in adults. A multidisciplin

Successful Interventional Treatment of Pyopneumothorax Caused by Streptococcus constellatus Associated with Hashimoto's Thyroiditis: A Case Report and Literature Review.

BACKGROUND: Streptococcus constellatus rarely causes pyopneumothorax, which is a serious state and requires a surgery. However, not every patient can tolerate surgery and individualized solutions are needed. Furthermore, many known situations are risk factors of S. constellatus infection, but S. constellatus pyopneumothorax associated with Hashimoto's thyroiditis has not been reported. CASE PRESENTATION: We present the case of a 74-year-old male with multiple encapsulated pyopneumothorax caused by S. constellatus. Given his respiratory failure, we provided two-stage percutaneous right empyema

The history of imaging in obstetrics.

During the past century, imaging of the pregnant patient has been performed with radiography, scintigraphy, computed tomography, magnetic resonance imaging, and ultrasonography (US). US imaging has emerged as the primary imaging modality, because it provides real-time images at relatively low cost without the use of ionizing radiation. This review begins with a discussion of the history and current status of imaging modalities other than US for the pregnant patient. The discussion then turns to an in-depth description of how US technology advanced to become such a valuable diagnostic tool in t

Endotracheal placement of balloon-expanded stents: an experimental study in rabbits.

The authors implanted balloon-expanded stents in the trachea of 10 rabbits for 1 day to 6 months. Follow-up included chest radiography, clinical examination, light microscopy, and scanning electron microscopy. Pneumothorax occurred in two cases after placement, marked stridor due to mucus plugging in all cases, signs of infection in three cases (two of which developed into pneumonitis), compression of the upper stent end in two cases, and moderate to severe mucus plugging in all cases. Microscopically, none of the stents was completely covered with epithelium even at 6 months. The high rate of

Echocardiography, not chest radiography, for evaluation of cannula placement during pediatric extracorporeal membrane oxygenation.

OBJECTIVE: Optimal cannula position is essential during extracorporeal membrane oxygenation (ECMO). We hypothesize that echocardiography is superior to chest radiography in diagnosing abnormal cannula position during ECMO. DESIGN: Retrospective. SETTING: Pediatric hospital. PATIENTS: 100 pediatric patients requiring ECMO. MEASUREMENTS AND MAIN RESULTS: We reviewed the medical records of all ECMO patients (n = 100), including reports of all echocardiograms (n = 326), during the years 2002-2004. Of the 91 patients who had echocardiograms while on ECMO, 33 had at least 1 echocardiogram for ca

Pain Management in Pediatrics: What the IR has to Offer.

Pediatric pain management presents unique challenges due to the intrinsic characteristics of children such as their developmental stages, communication barriers, and varying pain perceptions. Life-limiting conditions affecting children are a growing medical concern, requiring a comprehensive, multidisciplinary approach to improve quality of life or ensure a dignified end of life. Interventional radiology (IR) plays a critical role in this strategy, similar to its role in adult care. Not only life-limiting conditions pose a challenge in pediatric chronic pain management, but also other benign c

The Role of Interventional Radiology in the Management of Biliary Disease.

Interventional radiologists are an important part of a multidisciplinary team for the management of patients with biliary disease. Minimally invasive image-guided procedures can diagnose, temporize, and treat patients presenting with a broad spectrum of disease, including acute, chronic, benign, and malignant etiologies. In certain cases, interventional radiologists may provide definitive therapy for this often-difficult patient population. Effective treatment of patients with biliary disease is predicated on cooperation and collaboration of a multidisciplinary team working together providing

Evolving indications for pediatric neurointerventional radiology: A single institutional 25-year experience in infants less than one year of age and a brief historical review.

Background and PurposePediatric neurointerventional radiology is an evolving subspecialty with growing indications and technological advancement such as miniaturization of devices and decreased radiation dose. The ability to perform these procedures is continuously balanced with necessity given the inherently higher risks of radiation and cerebrovascular injury in infants. The purpose of this study is to review our institution's neurointerventional experience in infants less than one year of age to elucidate trends in this patient population.MethodsWe retrospectively identified 132 patients fr

Radiation Doses to Patients in Interventional Coronary Procedures-Estimation of Updated National Reference Levels by Dose Audit.

The objective of this study was to estimate the French national updated reference levels (RLs) for coronary angiography (CA) and percutaneous coronary intervention (PCI) by a dose audit from a large data set of unselected procedures and in standard-sized patients. Kerma-area product (PKA), air kerma at interventional point (Ka,r), fluoroscopy time (FT), and the number of registered frames (NFs) and runs (NRs) were collected from 51 229 CAs and 42 222 PCIs performed over a 12-month period at 61 French hospitals. RLs estimated by the 75th percentile in CAs and PCIs performed in unselected patien

Technique and indications of percutaneous cholecystostomy in the management of cholecystitis in 2014.

The gold standard in treatment of acute cholecystitis is cholecystectomy associated with antibiotics. In certain circumstances, percutaneous cholecystostomy is an interventional alternative. Percutaneous cholecystostomy is usually performed under local anesthesia by the radiologist using ultrasonographic or CT guidance. A drain can be inserted either through a trans-hepatic or a trans-peritoneal approach. Complications occur in nearly 10% of cases including hemorrhage, hemobilia, pneumothorax or bile leaks, depending on whether the approach was trans-hepatic or trans-peritoneal. The main indic
