Ranitidine-induced bradycardia in a neonate--a first report.

Bradycardia, an extremely rare side-effect of ranitidine therapy is described in a 4-day-old full-term male neonate, who was admitted because of massive gastro-intestinal bleeding. Two hours after the intravenous injection of 1 mg/kg body weight per day, ECG showed sinus bradycardia of 60 beats/min with normal axis and QRS complex. The bradycardia gradually resolved in the next 24 h.

[Lethargy in the course of treatment with ranitidine in a newborn infant].

The neuropsychiatric complications of histamine H2 receptor antagonists are poorly described in children. In this case report, lethargy occurred in a neonate treated with ranitidine. We urge caution when a histamine H2 receptor antagonist needs to be prescribed in such cases.

High doses of ranitidine in patients with reflux oesophagitis.

As in duodenal and gastric ulcer patients, a highly significant correlation between suppression of 24-h intragastric acidity and healing rates in reflux oesophagitis patients was demonstrated by meta-analysis of data obtained from the literature (r = 0.90; p less than 0.001). Furthermore, we have demonstrated in eight patients with reflux oesophagitis that 2-week treatment courses with 300 mg ranitidine twice daily and 300 mg four times daily progressively decreased 24-h intraoesophageal acidity but only moderately elevated basal and meal-stimulated serum gastrin concentrations, significantly …

Comparison of omeprazole with ranitidine in the treatment of reflux oesophagitis.

The amount and concentration of acid in the refluxed gastric contents are considered to be the major factors in the development and progression of reflux oesophagitis. Omeprazole, with its pronounced inhibition of gastric acid secretion, should therefore be a most effective drug for the treatment of patients with this disease. In an international clinical programme, the effects of treatment with placebo, ranitidine, 150 mg b.d. and omeprazole in doses ranging from 20 mg to 60 mg once daily, have been compared in patients with erosive and/or ulcerative oesophagitis. One placebo-controlled study…

Pharmacokinetics of ranitidine in acute upper gastrointestinal haemorrhage.

A pharmacokinetic study of ranitidine was performed in 14 patients with haematemesis divided into two groups according to the severity of blood loss. Pharmacokinetic values were calculated from plasma concentrations after the first of three daily injections (100 mg) and compared with those obtained in five healthy volunteers (50 mg i.v.). There were no significant differences between patients in the two haemorrhage groups and controls. The low, or even questionable, effectiveness of histamine H2-receptor antagonists in the treatment of upper gastrointestinal haemorrhage does not seem to be due…

Hemodynamic studies on cimetidine and ranitidine in anesthetized dogs.

The cardiovascular effects of the H2-receptor-antagonists cimetidine and ranitidine have been compared in a randomized double-blind crossover study. The experiments were performed in seven anesthetized mongrel dogs. Cimetidine (200 mg) and ranitidine (50 mg) were infused i.v. for 2 min. The time interval between cimetidine and ranitidine application was always 3 h. The direct comparison of the drug effects on blood pressure shows a significant difference between cimetidine and ranitidine (P less than 0.001). Apart from the sequence of application, cimetidine causes a significant decrease (P le…

Ranitidine as an antacid before elective Caesarean section.

In a preliminary study, 20 women in labour received ranitidine 50 mg intravenously. No significant changes were seen in the height, frequency or amplitude of uterine contractions or in fetal heart rate or pattern. No neonatal problems attributable to ranitidine were found. Ranitidine crossed the placenta, the mean fetal-maternal ratio being 0.9. Levels in the infants 12 hours following delivery were all very low. Ranitidine 150 mg orally was given to 80 healthy women undergoing elective Caesarean section at varying times from 75 to 510 minutes pre-operatively. From 2 to 6 hours following inges…

Latex anaphylaxis causing heart block: role of ranitidine.

PURPOSE: Treatment with H2 receptor antagonists may cause the heart to be more susceptible to atrioventricular conduction delay when exposed to an overwhelming insult by histamine released during an anaphylactic reaction. We present the case of a woman, pretreated with ranitidine, who developed 3:1 heart block secondary to latex anaphylaxis. We propose that H2 antagonist premedication alone in patients susceptible to anaphylaxis increases their risk of heart block. CLINICAL FEATURES: A 38 yr old obese woman with cervical cancer presented for a radical hysterectomy. Systems review yielded a hi…

Acute treatment of reflux oesophagitis: a multicentre study to compare 150 mg ranitidine twice daily with 300 mg ranitidine at bedtime.

A randomized, double-blind, clinical trial was undertaken to compare 150 mg ranitidine b.d. with 300 mg ranitidine nocte in the treatment of reflux oesophagitis. Endoscopy data were evaluable for 336 patients after 8 weeks of treatment. At this time 75% of patients who received 150 mg ranitidine b.d., and 73% of those who received 300 mg nocte, had healed or showed endoscopic improvement to grade I oesophagitis. At 12 weeks these rates had increased to 89 and 88%, respectively. Oesophageal biopsies from 258 patients at 8 weeks showed histological improvement in 44 and 47% of those treated with…

A double-blind controlled trial of ranitidine 300 mg nocte and ranitidine 150 mg b.i.d. in the short-term treatment of gastric ulcer.

A multi-center study was carried out to compare the healing rates of ranitidine 300 mg nocte and 150 mg b.i.d. in the short-term treatment of gastric ulcers and to assess the side-effect liability of ranitidine 300 mg given in a single daily dose. Forty-five outpatients suffering from endoscopically and bioptically proven uncomplicated benign gastric ulcer were selected for the study. The patients were treated, for 4 weeks, on the basis of a double-blind randomized design. An endoscopic examination was repeated within 4 days after the end of the treatments. The patients who did not demonstrate…

Long-term maintenance treatment of gastric ulcers with ranitidine.

One hundred and twenty patients with gastric ulcer disease, who had been receiving maintenance treatment with ranitidine (150 or 300 mg/day) for periods up to 7 years, were studied retrospectively. The proportion of patients remaining free from symptomatic recurrence of ulcer during maintenance treatment was 97% after 1 year; 90% after 3 years; and 79% after 5 years. No patient developed haemorrhage or perforation during maintenance treatment. None of the demographic features was shown to be associated with a significantly increased risk of ulcer recurrence during maintenance treatment. Compar…

Allergic contact dermatitis from occupational exposure to ranitidine hydrochloride.

Contact sensitivity to ranitidine hydrochloride has been demonstrated in 6 of 8 symptomatic exposed pharmaceutical manufacturing employees; a 7th has been found to be sensitized to ranitidine base. Although closed manufacturing processes diminish the risk of such sensitization, operatives with potential exposure should be made aware of the importance of avoiding skin contact.

[Regression of primary gastric lymphoma after treatment with ranitidine].

In a 60 years old patient affected by primary gastric MALT lymphoma stage IE, we observed a complete regression after aspecific treatment with ranitidine 150 mg x 2/day; after regression we treated the patient with anti-Helicobacter pylori therapy for 30 days. Nowadays, after 3 years of clinical and endoscopical follow-up we observe no relapse of disease. The elimination of antigenic stimulation of Helicobacter pylori, stopping T-lymphocytes' effect on B-lymphocytes, stopped neoplastic proliferation, causing the complete regression of neoplastic lesion. This clinical case, according to biologi…
