Subcutaneous granuloma annulare occurs either as a single or multiple lesion. When the ocular adnexa are involved, the lateral aspect of the eyelid and the lateral canthus are sites of predilection. These lesions occur chiefly in children and only rarely in adults. They have the same clinical (and histopathological) appearance as rheumatoid nodules and are, therefore, also known as "pseudorheumatoid nodules." We report a clinicopathologic study of an unusual case of pseudorheumatoid nodule involving the right orbit of a child. Pathologically, the orbital lesion was similar to subcutanous nodules of rheumatoid arthritis and rheumatic fever. The physical findings and laboratory studies in our case did not reveal any evidence of systemic disease. Based on a review of reported cases involving other locations, this lesion appears clinically benign in children.

A woman first seen in 1978 had a history of seropositive rheumatoid arthritis (RA) of 12 years' duration with attacks of palindromic rheumatism for 3 years. She was treated with D-penicillamine, pyridoxine and hydroxychloroquine and serial measurements of her grip strength and proximal interphalangeal joint circumference were taken. A woman in her early 50s presented with haemoptysis and cavitary pulmonary nodules. Initial testing showed a low-titre positive cryptococcal antigen (CrAg); however, repeat testing on two occasions was negative. Her cavitary nodules were ultimately attributed to an overlap systemic autoimmune rheumatic disease. She achieved complete clinical and radiological resolution with corticosteroid therapy. This case highlights the importance of recognising false-positive serum CrAg results.

The term "rheumatoid nodulosis" was coined by Ginsberg in 1975 to designate a rare and distinctive form of rheumatoid disease. Anecdotal case reports suggest a benign nondestructive course, although prolonged follow-up data are usually unavailable. We present two patients with subcutaneous nodular lesions of several years' duration. These lesions were well defined and painless and were located next to bony surfaces or in the pulps of fingers and elbows. Sixty-six Nigerian children with rheumatic fever were studied between 1975 and 1979. Equal numbers of patients had acute and chronic disease. Carditis was the most common major manifestation, being present in 98% of the cases, whereas subcutaneous nodules, erythema marginatum, and chorea were relatively rare. Fifteen asymptomatic children with benign rheumatoid nodules, followed up to 12 years, are described. Nodules are characterized by subcutaneous location with predilection for pretibial regions and scalp, occasional large size, spontaneous regression, and frequent recurrence.
