Microvascular decompression is a well established technique in the treatment of medically refractory trigeminal neuralgia when a significant vascular contact is identified during posterior fossa exploration. Microvascular decompression is preferred among open procedures for the treatment of trigeminal neuralgia. The possibility of obtaining a selective effect on different trigeminal branches by glycerol rhizotomy was studied in the treatment of trigeminal neuralgia. Transcutaneous electrical stimulation was used to quantify sensory impairment. A percutaneous technique of selective partial trigeminal root coagulation was evaluated in the treatment of 38 patients suffering from trigeminal neuralgia, 1 patient with pain secondary to oral carcinoma and 1 patient with atypical facial pain. The pain of trigeminal neuralgia was relieved in 94.7 percent of patients. Pain was relieved in the patient with oral carcinoma, but not in the patient with atypical facial pain. We describe a new method for radio-frequency mandibular nerve rhizotomy under CT fluoroscopy. A patient with cancer had severe intractable and drug-resistant pain in his left mandibular region. Because he had an anatomic deformity due to cancer invasion and radiation therapy, we planned a mandibular nerve rhizotomy under CT fluoroscopic imaging. The needle was advanced to the mandibular nerve just caudal to the foramen ovale under real-time CT fluoroscopy, avoiding the cancer region. Pain scores of the patient were reduced after the nerve rhizotomy, without any complications.

The relation between postoperative sensory deterioration and surgical outcome in 54 patients treated by retrogasserian glycerol rhizotomy for trigeminal neuralgia was studied. The facial sensibility was assessed one day and three months postoperatively. Using the Cochet-Bonnet esthesiometer, we evaluated the corneal sensation in 13 patients with trigeminal neuralgia. We compared the preoperative and postoperative results after the trigeminal rhizotomy in a quantitative study. Of 13 patients who underwent operation, a decrease in corneal sensation developed compared with the eye that was not operated on in five patients. This decrease in sensitivity was not suggested by clinical symptoms or signs, and up to 23 months of follow-up failed to reveal any clinical symptoms.
