The highly developed diagnostic technologies of our time applied to acute scrotum is not readily available in many of our hospitals. Abnormalities of the bovine scrotum and testes are an important cause of infertility. Proper evaluation of the male reproductive system is a critical first step in screening for such abnormalities. Excessive periscrotal fat, cutaneous scrotal defects, and unilateral scrotal swelling are common deformities that warrant further investigation. Early diagnosis and surgical intervention are often needed to restore reproductive soundness. This article reviews these conditions and provides therapeutic modalities.

BACKGROUND: Median raphe cyst is usually benign and asymptomatic male genitalia lesions. Although uncommon, infection may be a complication.
CASE PRESENTATION: We report the case of a 4-year-old child presented to the emergency department for a serpiginous and redness lesion extended from the basis of the penis until the perineum. An infected median raphe cyst was suspected, and the patient underwent surgical treatment and antibiotic therapy with complete resolution of symptoms. Liquid culture resulted positive for Serratia Marcescens.

Castleman disease (CD) is a rare lymphoproliferative disorder that can affect any lymph node in the body, but CD occurring in the scrotum has not been reported to date. We report the case of a 79-year-old man with a painless hard mass in the right scrotum that has been gradually increasing in size for more than 1 year. Abdominopelvic CT scan showed a heterogeneously enhancing mass of 10 cm long in the right scrotum. The patient underwent resection of the right scrotal mass and the pathological diagnosis was Castleman's disease, plasma cell (PC) type.

A clinical case of scrotal atavism in the form of bone plate, which simulated neoplasm of scrotal organs and has not been previously described, is presented in the article. In our opinion, the tumor is a vestige of Bakulyum, an organ, which disappeared in Homo sapiens. The patient was successfully operated in the outpatient surgical department with good functional and long-term results. After 1-year follow-up, there were no complaints or recurrence. The patient had normal sexual function.
