Measurement of the spinal canal by diagnostic ultrasound.

A method is described of measuring the lumbar spinal canal by pulsed echo ultrasound. It is simple, safe and has a high degree of accuracy. The lumbar canal has been measured in over 800 subjects including 100 mining recruits and fifty nurses between the ages of fifteen and eighteen years. Ultrasound can demonstrate the degree and extent of bony stenosis. It may have value in preventive medicine, identifying the subject at risk.

Expanding lesions of the cervical spinal canal.

Attention has recently been directed to decrease in size of the spinal canal, notably in relation to production of posterior osteophytes in spondylosis. Tumors of the cord, roots, or meninges, syringolmyelia and hydromyelia may enlarge sufficiently in size or be critically located so that expansile change of the cervical spinal canal may result.

Computed tomography of the spinal canal and cord.

Selective cases have been presented to demonstrate the usefulness of computed tomography in disorders of the spine and spinal cord. The osseous structures are more readily demonstrated than the soft tissues. The cord is difficult and inconsistently identified. However, cystic lesions (syringomyelia) may, at times, be demonstrated. The transverse projection of the CT scan is an added benefit in demonstrating bony spinal lesions. The use of metrizamide with faster scanners should improve the usefulness of this technique in evaluating intraspinal lesions.

[Lumbar spinal canal stenosis : A historical perspective].

To know the history of a disease and its treatment is always instructive and helps us to understand the contexts better. It also shows what unbelievable preliminary work was necessary for us be able to treat patients today in the way we are used to. This article attempts to shed light on spinal canal stenosis from a historical perspective and to identify the pioneers who contributed to the understanding of the epidemiology, anatomy, pathogenesis, classification and diagnostic work-up of spinal canal stenosis. In addition, the efforts of scientists and clinicians who have participated in develo…

Lumbar spinal canal osteosarcoma: A case report.

RATIONALE: Osteosarcoma is a rare neoplasm in the lumbar spine. Although osteosarcoma can arise in any portion of the skeleton, it very rarely arises in the spinal canal, which accounts for <0.1% of all cases of adult sarcomas. Here, we describe a case of osteosarcoma arising in the L4-5 spinal canal. PATIENT CONCERNS: The present report describes the case of a 55-year-old female patient with osteosarcoma of the L4-5 spinal canal. DIAGNOSES: The patient was initially diagnosed with lumbar spinal stenosis and underwent lumbar fusion at a local hospital. At the 4-month follow-up, the patient r…

Cervical spinal canal narrowing in idiopathic syringomyelia.

INTRODUCTION: The cervical spine in Chiari I patient with syringomyelia has significantly different anteroposterior diameters than it does in Chiari I patients without syringomyelia. We tested the hypothesis that patients with idiopathic syringomyelia (IS) also have abnormal cervical spinal canal diameters. The finding in both groups may relate to the pathogenesis of syringomyelia. METHODS: Local institutional review boards approved this retrospective study. Patients with IS were compared to age-matched controls with normal sagittal spine MR. All subjects had T1-weighted spin-echo (500/20) an…

Posterior mediastinal angiomyxolipoma with spinal canal extension.

BACKGROUND: Angiomyxolipoma is a benign tumor considered as a variant of lipoma and that occurs mainly in the subcutis. The mediastinal location hasn't been previously reported. AIM: To describe the radiological features of this tumor in its posterior mediastinal location and to confront them to the pathological features. CASE REPORT: We report the case of a 49-year-old woman who was admitted for chest wall pain and neurologic disturbance of her two lower limbs. The chest X-ray showed a posterior mediastinal opacity. On CT examination, this mass contained some small areas of fat and enhanced

[CT analysis of developmental spinal canal stenosis].

The cervical and the lumbar vertebrae were analyzed by computed tomography in two groups. One group consisted of the patients with developmental stenosis of the cervical spine (the smallest AP diameter of the cervical spinal canal in the lateral view of X-ray was less than 12 mm) and the other was the control group (the smallest diameter was more than 14 mm). The canal to body ratio of the cross-sectional area (CBR), the modified Jones' "canal to body ratio" (JNS) and the flat index of the vertebral canal (FI) were measured at C4,C5, C6, L4 and L5. The shape of the vertebral canal of L4 and L5…

Lumbar spinal canal size of sciatica patients.

Seven measures at the three lowest lumbar interspaces were recorded from conventional radiographs of the lumbar spines of 160 consecutive pa … r length at L3 and limited straight leg raising. Where the disc material had been extruded into the spinal canal, the interpedicular distance was significantly wider. Only anterior disc height at L3 revealed differences between good and poor outcome one year after surgery, as did the interarticular distance at S1 in patients with normal myelogram after conserv …

Narrow lumbar spinal canal with "vascular" syndromes.

Symptoms suggestive of vascular origin, both venous and arterial, may be the presenting complaints in patients with lumbar spondylosis. Fourteen patients suspected of having vascular intermittent claudication were found to be free of vascular disease, but had cauda equina compromise from herniated disk, osteoarthritis, and hypertrophic ligaments. Complete follow-up data were available for seven patients. In three, claudication seemed typical; in four, atypical. At operation, herniated intervertebral disks, osteophytic bone, or hypertrophied ligamenta flava, or a combination, were found. All be

Spinal canal morphology and clinical outcomes of microsurgical bilateral decompression via a unilateral approach for lumbar spinal canal stenosis.

PURPOSE: Microsurgical bilateral decompression via a unilateral approach for lumbar spinal stenosis is a less invasive technique compared to conventional laminectomy. Although many technical reports have demonstrated acceptable overall surgical outcomes for this approach, no studies have attempted to clarify the clinical outcomes thereof in regard to anatomical variance of the spinal canal. This study was conducted to analyze the clinical outcomes of microsurgical bilateral decompression via a unilateral approach according to spinal canal morphology in degenerative lumbar spinal stenosis. MET…

Tophaceous Gout in the Lumbar Spinal Canal Mimicking Epidural Spinal Tumor.

Gout is an inflammatory arthritis characterized by deposition of monosodium urate crystals in joints. Though gout frequently involves the big toe or other extremities, it rarely occurs in the spinal canal. A 35-year-old man presented with left L5 radiculopathy. He had leg pain for 8 months and received several epidural steroid injections. Magnetic resonance imaging revealed a 1.7×1.1-cm ovoid contrast-enhancing mass, causing pressure erosion of the left L5 pedicle. Microscopic laminotomy was performed at the left L5 lamina. White chalky materials, identified at the left lateral recess of the s…

Shape of the Spinal Canal Is Not Associated with Success Rates of Microsurgical Unilateral Laminotomy and Bilateral Decompression for Lumbar Spinal Canal Stenosis.

BACKGROUND: Anatomic shape of the spinal canal (oval, round, trefoil) has been reported to predict outcome of bilateral decompression performed in an undercutting technique via unilateral laminotomy in monosegmental lumbar spinal stenosis, with poorest results observed in a trefoil spinal canal, leading to the proposal of using bilateral instead of unilateral laminotomy. The aim of this study was to assess whether this anatomic classification into oval, round, and trefoil shapes is relevant to surgical treatment of lumbar spinal stenosis. METHODS: Retrospective chart review of patients underg…
