[Strabismus and reading: Effect of strabismus on reading tests in children from 8 to 11 years].

PURPOSE: We try to show a relationship between strabismus and changes in reading skills. MATERIAL AND METHODS: We have carried out a prospective study including 135 children from 8 to 11 years (French level CE2 to CM2). They were given an ophthalmologic and orthoptic examination and then divided into 4 groups: strabismus with vertical deviation without binocular vision, accommodative strabismus with binocular vision, accommodative strabismus without binocular vision and control group (children without strabismus). Each child took 4 validated reading tests: reading fluency, uncommon words read…

Imaging in strabismus re-operations.

Purpose: Accurate identification of extraocular muscle insertions in previously operated muscles, along with defining their positions, caliber, and surrounding structures via pre-operative imaging, aids surgical planning for re-operations. Methods: This narrative review summarizes imaging tools and techniques for strabismus re-operation planning, including anterior segment OCT (AS-OCT), ultrasound biomicroscopy (UBM), CT, and MRI. Results: Imaging enables a more predictable surgical course and better informs patients about procedure outcomes. Discussion: Key steps include selecting the optimal…

[Case of acquired paralytic strabismus].

This report described one case of acupuncture treatment for acquired paralytic strabismus. Acupoints included bilateral Fengchi (GB20), Sibai (ST2), Taiyang (EX-HN5), Tongziliao (GB1), Jiexi (ST41), and Taichong (LR3). Fengchi (GB20) was treated with the deep-needling nayang technique, Jiexi (ST41) with the qinglong baiwei method, and the remaining acupoints with conventional acupuncture techniques. Needles were retained for 40 min, once daily, six times per week. After two months of treatment, the patient's diplopia resolved, the right eye exhibited flexible abduction, and the ocular position…

Retrobulbar hemorrhage during strabismus surgery.

PURPOSE: This case report presents an event of retrobulbar hemorrhage (RH) occurring during the initial stage of strabismus surgery after incision of the conjunctiva and Tenon's capsule. OBSERVATION: Significant bleeding with subsequent proptosis was observed intraoperatively after the incision of conjunctiva and Tenon's capsule during routine strabismus surgery on the medial rectus muscle in a 5-year-old boy. Intravenous mannitol was administered intraoperatively and surgery was completed as planned. The RH receded within 24 hours without the necessity of orbital decompression. Tenon's capsu…

Orbital Causes of Incomitant Strabismus.

Strabismus may result from abnormal innervation, structure, or function of the extraocular muscles. Abnormalities of the orbital bones or masses within the orbit may also cause strabismus due to indirect effects on the extraocular muscles. This paper reviews some disorders of the orbit that are associated with strabismus, including craniofacial malformations, orbital masses, trauma, and anomalous orbital structures.

Strabismus in childhood eyelid ptosis.

PURPOSE: To report the prevalence and causes of strabismus in children with eyelid ptosis diagnosed in a well-defined population over a 40-year period. DESIGN: Retrospective, population-based cohort study. METHODS: We retrospectively reviewed the charts of 107 patients (<19 years) for the prevalence and causes of strabismus in children who were diagnosed with childhood eyelid ptosis as residents of Olmsted County, Minnesota, from January 1, 1965, through December 31, 2004. RESULTS: Strabismus was diagnosed in 20 (18.7%) of the 107 patients with childhood ptosis. Of the 81 patients, 8 (9.9%)…

[Clinical classification of incomitant strabismus].

Based on study of etiology, pathogenesis, clinical course, results of treatment of 489 patients aged from 1 till 73 years old with incomitant strabismus and integration of literature data clinical classification of incomitant strabismus was developed. Classification considers type and degree of extraocular muscle impairment (paralysis, palsy, pseudoparalysis, atypical forms), localization of damage (III, IV, VI cranial nerves, concurrent nerve damage), level of damage (cranial, orbital, combined), etiology and pathogenesis (local or systemic disease), age of manifestation (congenital, early an…

Anomalous extraocular muscles with strabismus.

An 8-month-old boy with Gorlin syndrome presented with a large right-face turn and constant exotropia of the left eye. Eight-millimeter recession of the left lateral rectus muscle was performed at 23 months of age without complete postoperative improvement. Orbital imaging revealed bilateral anomalous extraocular muscles inferolateral to the optic nerves. Surgical resection of the tissue confirmed the accessory musculature with postoperative correction of the strabismus. To our knowledge, this appears to be the first reported case in the radiologic literature.

Strabismus in senile cataract patients.

PURPOSE: We sought to determine the prevalence of strabismus among patients with senile cataract. METHODS: Forty-seven consecutive patients who were scheduled for cataract surgery underwent complete eye examinations, including ocular alignment evaluation. Twenty-four normal adults of the same age range underwent also complete eye examinations and were used as a control group. All patients of the cataract group underwent unilateral cataract surgery. The angle of strabismus was measured by the prism and cover test or prism and corneal reflex test. RESULTS: Mean preoperative deviation of the st…

Persistent strabismus after cataract extraction.

BACKGROUND: Transient ocular misalignment as a complication of parabulbar and peribulbar anesthesia has already been reported in the literature. The aim of our study was to present a case of irreversible iatrogenic vertical strabismus after cataract surgery, which had to be operated on. METHODS: Clinical and orthoptic evaluation of a female patient with vertical diplopia after phacoemulsification cataract surgery. RESULTS: One week after the uneventful surgery, a 68-year-old patient complained of a sudden vertical deviation in the operated eye. The patient had not had a history of previous m…

Presbyopia complicating pre-existing strabismus.

BACKGROUND: Presbyopia may affect pre-existing sensory adaptations or aggravate previously asymptomatic heterophoria. We describe the presentation, underlying problem and management of 11 patients with pre-existing strabismus or heterophoria who presented with new symptoms of double vision attributable to presbyopic change, an association not previously reported. METHODS: Patients with new strabismic symptoms attributable to presbyopia were recruited prospectively over a 1-year period. RESULTS: The 11 patients had had a recent decrease of accommodative amplitude that resulted in blurred visi…

Outcome criteria in strabismus surgery.

Criteria for success in strabismus surgery should include more than just the alignment result in primary position. It is important to consider as well the extent of the field of BSV gained: restoring a useful range of BSV will maximize the patient's ability to carry on daily activities. Psychometric tests can also be used to provide objective measures of improvement following strabismus surgery. Irrespective of the patient's age, the realigning of an eye is reconstructive, not cosmetic, surgery. Surgery in children and adults can restore fusion and normalize the field of BSV in most cases. The…

Cyclic strabismus without binocular function.

Various forms of cyclic strabismus have been described. A cyclic variation in binocular function modifying a tendency to strabismus is said to be the underlying mechanism. Cyclic esotropia developed in two adult patients, one having no demonstrable binocular function and the other having suffered complete unilateral visual loss in the affected eye. In each case, on the "good days" there was still a manifest esotropia, in contrast to those patients with cyclic esotropia and good binocular function in which the eyes are straight on the "good days." The presence of binocular function is not requi…

[Binocular vision in concomitant strabismus].

It was discovered that in a small angle concomitant squint there occurs a kind of binocular vision maintained by abnormal retinal correspondence. This phenomenon may be discovered by a campimetric examination, by VEP and by other psychophysical tests. There appear accomodational movements which tend to maintain a stable angle of deviation which enables a binocular vision. Analysis of these movements is presented.

Complicated strabismus and adjustable sutures.

Clinical experience in performing over 500 adjustable strabismus operations is mentioned in this paper particularly as it relates to complicated strabismus. A maximum hang-loose recession of a rectus muscle has a limited effect, which is tabulated and the implications discussed. Aids in finding the lost medial rectus muscle are mentioned. Adjustable sutures are then exploited in managing the found "lost" medial rectus. Adjustable recession of both vertical recti in the affected eye were used in some cases of blow-out fracture to manage limitations of upward and downward gaze. Adjustable recess…
