The molecular pathogenesis of malignant appendageal tumors is poorly understood. Immunohistochemical staining, polymerase chain reaction (PCR)-based loss of heterozygosity (LOH) and sequencing analyses were performed in a mixed group of 21 sweat gland carcinomas. LOH was mostly confined to the chromosome arm 17p. None of the remaining 17 tumors showed LOH at any loci. Malignant hidradenoma is a very rare tumor that originates from sweat glands. We present a 61-year-old man with an ulcerated tumor in his right flank, 4 cm in diameter, that was excised with a wide free margin. A 54 years male presented with painless, indolent ulcers in the left axilla in an area approximately of 5 cm diameter. They were fixed to the underlying structures. Some lymph nodes eg, pectoral, apical and central groups were also enlarged. Incision biopsy for the margin of the ulcer was done. It showed adenocarcinoma of the sweat glands. Wide local incision with 2 cm margin of surrounding skin was performed. The patient was discharged after a course of adjuvant radiotherapy and is doing well.

Sweat gland carcinomas are rare malignant tumors of the skin. The well-defined entities porocarcinoma, microcystic adnexal carcinoma, aggressive digital papillary adenocarcinoma, mucinous eccrine carcinoma, adenoid cystic carcinoma, spiradenocarcinoma, cylindrocarcinoma, hidradenocarcinoma are described. The article summarizes essential clinical, prognostic and histopathological findings of these tumors and takes in focus special recommendations for dermatologists and surgeons to plan biopsies and operations.
