Piperacillin/tazobactam-induced reversible pancytopenia in a pregnant patient: A case report.

Hematologic responses (< 1%) to piperacillin/tazobactam were uncommon, and mostly reported in non-pregnant adults. The use of piperacillin/tazobactam during pregnancy is considered to be moderately safe for the human embryo-fetus (pregnancy grade B). However, changes in pharmacokinetics during pregnancy (such as an increase in plasma volume and an increase in renal clearance rate) may alter the toxicity threshold of the drug. This is a case of a pregnant woman (26 weeks of gestation) treated with piperacillin/tazobactam for psoas abscess (Proteus mirabilis). After 22 days, the patient develope…

Kidney injury, dialysis, and mortality with vancomycin plus piperacillin-tazobactam or cefepime.

BACKGROUND: Retrospective studies have consistently reported higher rates of acute kidney injury (AKI) when vancomycin is combined with piperacillin-tazobactam compared with other β-lactams, raising concerns about nephrotoxicity. However, recent prospective studies and randomized trials have not demonstrated increased risk of clinically meaningful kidney injury, suggesting that creatinine-based endpoints may overestimate harm. We applied a multistate modelling approach to evaluate whether vancomycin-piperacillin/tazobactam is associated with progression to severe renal outcomes beyond creatini…

Piperacillin Sodium and Tazobactam Sodium-Induced Neutropenia and Thrombocytopenia: A Case Report.

Piperacillin sodium/tazobactam sodium (PIP-TAZ) is a commonly used antibiotic for various infections. While generally well-tolerated, it has been associated with rare cases of neutropenia and thrombocytopenia. This case report highlights the potential for PIP-TAZ to cause these adverse effects, emphasizing the importance of vigilant monitoring and timely intervention. A 79-year-old female with chronic renal failure and end-stage renal disease developed neutropenia and thrombocytopenia during treatment with PIP-TAZ for a suspected lung infection. Her neutrophil count plummeted from 11.17 × 109/…

DRESS syndrome with cerebral vasculitis provoked by piperacillin/tazobactam: a case report.

BACKGROUND: Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) syndrome is a rare, potentially life-threatening hypersensitivity reaction to medications. While commonly associated with hepatic, renal, and pulmonary involvement, central nervous system complications such as cerebral vasculitis are exceedingly rare and not well characterized. CASE PRESENTATION: We present the case of a 69-year-old woman with a history of hypertension who developed DRESS syndrome following a 2.5-week course of piperacillin/tazobactam for an infected malleolar wound. She presented with fever, generalize…

Piperacillin-Tazobactam-Induced Urticaria and Angioedema: A Case Report With Literature Review.

Drug-induced urticaria and angioedema cases are typically reversible upon discontinuation and can be triggered by antibiotics, angiotensin-converting enzyme inhibitors, or nonsteroidal anti-inflammatory drugs. Piperacillin-tazobactam, a common broad-spectrum antimicrobial, has been linked to severe adverse reactions, such as thrombocytopenia, hemolytic anemia, and Steven Johnson syndrome in some cases. A 35-year-old male presented to the emergency department with fever, cough, and acute breathlessness, complicating his ongoing treatment for pulmonary tuberculosis with bedaquiline and delamanid…

A puerperal patient with agranulocytosis during tazobactam / piperacillin administration : A case report.

Tazobactam / piperacillin (TAZ / PIPC) is an injectable combination drug consisting of a broad-spectrum penicillin and a β-lactamase inhibitor. This antimicrobial has a wide spectrum of efficacy against both Gram-positive bacteria and anaerobes. Adverse events usually present as diarrhea or liver dysfunction ; agranulocytosis has not been reported in Japanese patients with puerperal disorders. However, we report a 32-year-old Japanese woman who received TAZ / PIPC to treat an intraperitoneal infection that developed after complications related to transvaginal delivery. Within 14 days of beginn…

Stability of Meropenem and Piperacillin/Tazobactam with Heparin in Various Peritoneal Dialysis Solutions.

BACKGROUND: Infections caused by ceftazidime-resistant Pseudomonas and extended-spectrum beta-lactamase (ESBL)-producing gram-negative bacteria are increasing worldwide. Meropenem and piperacillin/tazobactam (PIP/TZB) are recommended for the treatment of peritoneal dialysis-associated peritonitis (PDAP) caused by ceftazidime-resistant Pseudomonas and other resistant gram-negative bacteria. Patients may also receive intraperitoneal heparin to prevent occlusion of their catheters. However, the stability of meropenem or PIP/TZB, in combination with heparin, in different types of peritoneal dialys…

Pharmacodynamics of Ceftolozane plus Tazobactam Studied in an In Vitro Pharmacokinetic Model of Infection.

Ceftolozane plus tazobactam is an antipseudomonal cephalosporin combined with tazobactam, an established beta-lactamase inhibitor, and has in vitro potency against a range of clinically important β-lactamase-producing bacteria, including most extended-spectrum-β-lactamase (ESBL)-positive Enterobacteriaceae. The pharmacodynamics of β-lactam-β-lactamase inhibitor combinations presents a number of theoretical and practical challenges, including modeling different half-lives of the compounds. In this study, we studied the pharmacodynamics of ceftolozane plus tazobactam against Escherichia coli and…

Shared Clavulanate and Tazobactam Antigenic Determinants Activate T-Cells from Hypersensitive Patients.

β-Lactamase inhibitors such as clavulanic acid and tazobactam were developed to overcome β-lactam antibiotic resistance. Hypersensitivity reactions to these drugs have not been studied in detail, and the antigenic determinants that activate T-cells have not been defined. The objectives of this study were to (i) characterize clavulanate- and tazobactam-responsive T-cells from hypersensitive patients, (ii) explore clavulanate and tazobactam T-cell crossreactivity, and (iii) define the antigenic determinants that contribute to T-cell reactivity. Antigen specificity, pathways of T-cell activation,…

Optimal Piperacillin-Tazobactam Dosing Strategies against Extended-Spectrum-β-Lactamase-Producing Enterobacteriaceae.

Piperacillin-tazobactam has been proposed as an alternative to carbapenems for the treatment of infections caused by extended-spectrum-β-lactamase (ESBL)-producing Enterobacteriaceae However, limited understanding of optimal dosing strategies for this combination may curtail its utility. In this study, we correlated various exposures of piperacillin-tazobactam to efficacy, using a modified pharmacokinetic/pharmacodynamic index. Using a clinical Klebsiella pneumoniae isolate expressing CTX-M-15, piperacillin MIC values were determined with increasing tazobactam concentrations and fitted to a si…
