Cardiology patient simulator and computer-assisted instruction technologies in bedside teaching.

The design and uses of an innovative technology-based approach which addresses critical problems with bedside teaching during ward rounds in the current health care situation are described. A cardiology patient simulator (HARVEY) and an accompanying computer-based interactive laserdisk system provide medical students, house officers and other health professionals with the opportunity to encounter a wide variety of clinical problems for learning and assessment. A group of cardiologists and medical educators from a consortium of US medical schools has guided the development and formal assessment

Defining Ultrasound-Guided Nerve Block Competency for Emergency Medicine: A Delphi-Method Consensus Statement.

BACKGROUND: Ultrasound-guided nerve blocks (UGNBs) are increasingly incorporated into multi-modal analgesia in the Emergency Department (ED). Despite their growing adoption, there is no consensus defining when an Emergency Medicine (EM) clinician is competent to perform UGNBs. Training methods, assessment approaches, and credentialing standards remain highly variable across institutions. The objective of this study was to define competency in UGNBs for EM physicians through a modified Delphi method that included national experts in EM and Anesthesia. METHODS: A comprehensive librarian-assiste

Designing and Implementing Emergency Department Pain Management Curriculum: A Delphi Approach.

OBJECTIVES: Our primary objective was to use the Delphi method (DM) to choose learning objectives for an Internet-based pain management teaching module for emergency care providers. The DM is a structured communication technique that uses systematic, interactive data gathering to reach a consensus among a panel of experts. METHODS: We extracted preliminary educational objectives from nationwide pain fellowship training programs. After redundant objectives and those relating to procedures outside the scope of emergency medicine (EM) were removed, 23 preliminary objectives remained. We enlisted

Virtual Surgical Grand Rounds Allow Greater Multidisciplinary and Interprofessional Education and Collaboration.

BACKGROUND: Over the past few years, Surgical Grand Rounds across the country have been overwhelmingly transitioned from in-person to a virtual or hybrid format. This has been shown to have improved attendance, allowed for greater geographical diversity of speakers and learners, and led to less disruption of clinical duties. At our institution we have used the opportunity to embrace more multidisciplinary and interprofessional involvement by opening sessions to include all members of our surgical specialties, anesthesiology, and palliative care teams. This survey aimed to assess the utility of

Structured Nurse-Led PICU Liberation Bundle (ABCDEF) Discussion During Rounds: A Single-Center, Secondary Analysis of Pre- Vs. Post-Implementation Data From 2017.

OBJECTIVE: The Society of Critical Care Medicine PICU Liberation ABCDEF (A-F) Campaign Collaborative aimed to implement best practices for analgesia (A), breathing (B), comfort/consciousness (C), delirium (D), early mobilization (E), and family engagement and empowerment (F). Our post hoc study of Mayo Clinic data sought to compare the pre-implementation rounding process, which used a nursing script organized by organ systems, with a novel, liberation-focused rounding process emphasizing A-F bundle elements. DESIGN: Secondary analysis of pre- vs. post-implementation surveys collected by indep

Interprofessional Team Staffing in U.S. Intensive Care Units.

Rationale: There is a paucity of data, and no consensus, about the composition of interdisciplinary teams of healthcare workers (HCWs) who provide care in intensive care units (ICUs). Objectives: To delineate the nature and variation of HCW staff composition in U.S. adult ICUs before the COVID-19 pandemic. Methods: A national survey of 574 adult ICUs inquired about ICU staffing. Two sets of survey items asked about 1) "availability to provide care" in ICUs for 11 HCW types, collapsed into six groupings; and 2) the presence in formal ICU clinical rounds of nine HCW types, collapsed into six gro

Participation of Parents of Hospitalized Children in Medical Rounds: A Qualitative Study on Contributory Factors.

PURPOSE: Factors that promote parents' participation during medical rounds on their hospitalized child have not been fully addressed. The aim of this study was to identify factors that promote the participation of family members during medical rounds. DESIGN AND METHODS: This was a descriptive qualitative study using elements of analysis from the grounded theory method. Semi-structured interviews and non-participant observations were performed from December 2015 until June 2016 and took place on a general academic pediatric ward where the age of children did not exceed 12 months. RESULTS: In

Cross-disciplinary "General Learning Objectives" for postgraduate medical training: Prioritisation of five competency domains using a Delphi process.

OBJECTIVE: The Swiss postgraduate education regulations stipulate that teaching be guided by cross-disciplinary "General Learning Objectives" (GLOs) based on the CanMEDS roles. Notwithstanding these regulations, teaching activities and performance criteria for GLOs are scarcely evident in postgraduate training programmes. The primary aim of this study was to determine the reasons why too little emphasis is placed on GLOs in postgraduate training programmes. A secondary aim was to define a reduced set of needs-based cross-disciplinary competencies that trainees should develop during their postg

When and How Would You Screen This Patient for Cervical Cancer? : Grand Rounds Discussion From Beth Israel Deaconess Medical Center.

Successful screening programs based on cervical cytology have dramatically reduced the incidence of cervical cancer in the United States. Human papillomavirus immunization is poised to reduce it further as an increasing percentage of vaccinated women reach adulthood. A recent guideline from the American Cancer Society advises that cervical cancer screening begin at age 25 and that high-risk human papillomavirus testing is the preferred screening test. The U.S. Preventive Services Task Force recommends screening begin at age 21 and that cytology remain incorporated into screening. Here, 2 exper

Development of a valid and reliable pterygium surgery assessment scale for ophthalmology residents.

BACKGROUND: Microsurgery training has become an important part of ophthalmology teaching and one of the main topics of examination. Accurate and effective evaluation of microsurgery skills is vital for the training and teaching of residents. In this study, we aimed to establish a pterygium surgery assessment scale for use by ophthalmic residents and evaluate its reliability and validity. METHODS: Based on a literature search, experienced pterygium surgeons developed the preliminary scale according to the standard surgical procedure. The preliminary scale and a questionnaire were sent to teach

POSTOPED: improving surveillance of postoperative delirium in a Scottish tertiary hospital.

Delirium is the most common postoperative complication among patients over the age of 65 years. It is associated with increased morbidity and is a significant financial cost to healthcare systems. We aimed to improve the detection of delirium on the surgical wards of a tertiary surgical centre. This would take the form of completion of 4AT assessments (the 4 AT test for delirium, on admission and 1 day postoperatively). Prior to this project, the 4AT was in use in the surgical admission clerking paperwork for over 65 s, however, 4AT assessments were not routinely performed as part of day 1 post

Our objective was to provide expert consensus on best practices for anatomy teaching and training on ultrasound-guided botulinum neurotoxin type A (BoNT-A) injection for specialists involved in treating spasticity and dystonia. Nine experts (three neurologists; six physical medicine and rehabilitation physicians) participated in a three-round modified Delphi process. Over three rounds, experts reached consensus on 15 of 16 statements describing best practices for anatomy and BoNT-A injection training. They unanimously agreed that knowledge of the target audience, including their needs and curr
