Abdominal imaging after intravenous injection of 99mTc pertechnetate is a simple, safe, and noninvasive diagnostic procedure to show ectopic gastric mucosa, which is present in most cases of symptomatic Meckel's diverticulum. It was performed in 13 infants and children suspected of having a Meckel's diverticulum, mainly because of unexplained rectal bleeding. Two of the three proven Meckel's diverticula were correctly identified by scintiscanning, while meticulous contrast x-ray studies were negative in each case. Chylous effusion is an infrequent complication of abdominothoracic surgery. When conservative therapy cannot cure the patient, surgical closure of the thoracic duct must be performed. If radiological lymphography is impossible to perform, lymphoscintigraphy with Tc-99m human serum albumin is a good alternative. Here, we present two patients who developed chylothorax after abdominothoracic surgery. Successful surgery was performed after lymphoscintigraphy had demonstrated the disruption of the thoracic duct.

We herewith report an unusual case of primary hyperparathyroidism whose parathyroid adenoma strongly accumulated technetium (Tc)-99m pertechnetate. A 41-year-old woman was referred to our department under the tentative diagnosis of primary hyperparathyroidism. Methotrexate is used widely in high doses as a chemotherapeutic agent. Lower doses are given in rheumatoid and psoriatric arthritis. Thyroid nodularity is a common finding. The incidence of thyroid cancer in the general population is estimated at 0.1%. Thyroid nodules are evaluated by palpation, ultrasonography, radionuclide scintigraphy and fine-needle aspiration cytology. Routine thyroid scintigraphy is made by using Tc-99m-pertechnetate or the I-131 and I-123 scintigraphic method. Preoperative localization of ectopic parathyroid lesions is crucial for the correct treatment of patients with primary hyperparathyroidism. Invasive and noninvasive procedures, including selective venography, ultrasound, CT and MRI provide limited sensitivity in the detection of ectopic lesions. We report three patients in whom 99mTc-MIBI scintigraphy accurately detected ectopic parathyroid adenomas and was instrumental in the cure for these patients. Technetium-99m-MIBI scintigraphy provides a simple and accurate noninvasive test for the detection of ectopic parathyroid adenomas.

We report a case in which 99mTc-sestamibi SPECT was used to localize a middle mediastinum parathyroid adenoma that was not detected with planar sestamibi imaging on two previous occasions. UNLABELLED: Our goal was to determine the clinical usefulness of 99mTc-sestamibi to identify breast cancer in patients prior to biopsy.
METHODS: We studied 66 patients who received 20 mCi 99mTc-sestamibi intravenously. Lateral and anterior planar images were gathered within 30 min of the injection. Only focal increased uptake was interpreted as positive. Confirmatory pathologic diagnoses were obtained within 2 mo. The prevalence of breast cancer in our sample was 54%.

We have evaluated five compounds, stannous chloride (SnCl2), 2-mercaptoethanol (2-ME), dithiothreitol (DTT), dithioerythritol (DTE), and ascorbic acid (AA) to reduce monoclonal antibody MoAb (disulfide groups and compared their efficacy for labeling MoAbs with 99mTc. The reduction of 99mTc with dithionite at pH 11 was nearly quantitative. The use of AA, at a molar ratio of 3500:1, for three IgG and three IgM antibodies examined, gave a labeling efficiency greater than 95%. Hence no purification was needed. A method of producing technetium-99m (99mTc) radiopharmaceuticals is described, where reduction of pertechnetate occurs through stannous ions adsorbed on the surface of an infusion catheter. This leads to radiopharmaceuticals containing microgram quantities of stannous ions and that, therefore, results in minimal blood-pool labeling and essentially the elimination of tin colloids. Technetium-99m stannous pyrophosphate (99mTc-PYP) myocardial scintigrams were obtained in 35 acute pericarditis and in three chronic constrictive pericarditis patients.
