Beyond the binary: a nationwide study of physician decision orientations toward artificial hydration in terminal cancer care.

BACKGROUND: Artificial hydration (AH) in terminally ill cancer patients remains ethically and clinically controversial. Although evidence suggests limited benefit near the end of life, AH is frequently administered, particularly in East Asian settings where it may carry symbolic meaning. This study conceptualized physician decision-making as a clinical spectrum and examined demographic, clinical, and ethical factors associated with AH decision orientations. MATERIALS AND METHODS: A nationwide cross-sectional survey of palliative care-trained physicians in Taiwan assessed clinical practices an

Prognostic factors in patients in the terminal phase of haematological malignancies who are receiving home medical care.

Although there are many prognostic models for patients in the terminal phase of solid tumours, a reliable prognostic scoring system in patients in the terminal phase of haematological malignancies (HM) has not been established. We retrospectively evaluated 180 patients in the terminal phase of HM who were receiving home medical care (HMC). Multivariate analyses revealed that clinician's estimate, consciousness, loss of appetite, dyspnoea, neutrophil count, lymphocyte count, and lactate dehydrogenase were associated with overall survival (OS). Based on this result, we developed a novel prognost

Economic analysis of terminal care for patients with malignant osteolytic bone disease and pain treated with pamidronate.

The goals of this study were the assessment (1) of all costs of terminal care of patients with osteolytic bone disease and pain and (2) of the economic consequences of the pamidronate treatment as observed in a prospective clinical trial on the effectiveness of pamidronate. A total of 70 patients were recruited, who were all suffering from advanced tumour diseases (60% breast cancer, 21% multiple myeloma, and 19% other tumours). In a single-institution study the patients were randomly assigned to receive, in a double-blinded setting, pamidronate 60 mg i.v. or 90 mg i.v. every 3 weeks for a max…

[Anesthetic care for fibrobronchoscopy in patients with chronic terminal respiratory insufficiency undergoing evaluation for isolated lung transplantation].

During the first 18 month operation of the isolated lung transplantation program at or Institution, eight patients with terminal chronic respiratory failure underwent fiberoptic bronchoscopy and broncho-alveolar lavage as part of their evaluation for isolated lung transplantation. Four patients had severe obstructive, three restrictive, and one mixed, obstructive and restrictive, disease; all of them were on continuous supplemental oxygen. Procedures were performed under topical anaesthesia, with either light sedation or simple monitored anaesthesia care. Monitoring included non-invasive blood

How different is the care of terminal pancreatic cancer patients in inpatient palliative care units and acute hospital wards? A nationwide population-based study.

BACKGROUND: Inpatient palliative care is important for patients with terminal pancreatic cancer. However, the differences between inpatient palliative care and acute hospital care for inpatients with pancreatic cancer have not been explored in a population-based study. METHODS: This population-based nationwide study was conducted using data from the Taiwan National Health Insurance database to analyze the differences between inpatient palliative care and acute hospital care for inpatients with pancreatic cancer. We identified 854 patients with terminal pancreatic cancer, who had received in-h

Barriers to Palliative Care for Terminally Ill Children in a Developing Country Cancer Hospital: A Qualitative Study.

INTRODUCTION: Pediatric palliative care (PPC) aims to improve the quality of life for children with life-limiting illnesses and support their families by addressing physical, psychological, social, and spiritual needs. Globally, significant barriers to PPC delivery include limited integration into health systems, inadequate provider training, restricted access to pain medications, and communication challenges. These issues are particularly pronounced in low- and middle-income countries, where cultural, infrastructural, and resource constraints further complicate care. Despite growing recogniti

Plasma L-carnitine levels in terminally ill cancer patients receiving only palliative care.

BACKGROUND: Several studies indicated that plasma L-carnitine (LC) levels are significantly decreased during chemotherapy or chemoradiation and that LC supplementation can improve the fatigue score in some cancer patients. However, the LC levels in end-stage cancer patients treated only with palliative care remained unclear. The present study was performed to examine the plasma LC levels of terminally ill and hospitalized patients. METHODS: Twenty-one terminally ill cancer patients in our hospital, with expected survival of several months, were enrolled in the present study. Blood samples wer

BACKGROUND: To ensure coherence and continuity of care for individual patients, it is necessary to improve collaboration between healthcare systems as well as between healthcare systems and patients. The objective of the study was to gain knowledge about the experiences of terminally ill patients and their relatives with the healthcare system to identify facilitators of and barriers to high-quality care in the last stage of patients' lives. METHODS: Semi-structured interviews were conducted with terminally ill patients and their relatives. The data were analysed using content analysis. RESUL

Integrating Swanson's Caring Theory into End-of-Life Nursing for Terminal Tongue Cancer: A Case Report.

Patients with terminal tongue cancer often experience severe physical pain, oral mucosal breakdown, and psychological distress that significantly diminishes quality of life. This case report describes the care of a 47-year-old man with recurrent terminal tongue cancer, demonstrating how Swanson's Caring Theory guided holistic and empathetic end-of-life nursing practice. Over a 16-day hospice admission in September 2020, assessment was based on Gordon's functional health patterns and interventions aligned with Swanson's 5 caring processes. Key issues included excruciating tumor pain (rated 10/1

Simultaneous occurrence of transient global amnesia and Takotsubo syndrome triggered by caring for a terminally ill relative.

Takotsubo syndrome and transient global amnesia can occur simultaneously, not only in the context of acute but also long-standing emotional stress. Probably, hypothyroidism and migraine make the patient more susceptible to both of these disorders.

Factors influencing palliative care decision-making participation among elderly patients in the terminal stage: a cross-sectional study.

BACKGROUND: Against the global ageing backdrop, the demand for palliative care among terminally ill elderly patients is rising, but their participation in related decision-making remains generally low. Identifying influencing factors is urgent to improve resource utilization efficiency and care quality. METHODS: A convenience sampling method was used to select 426 elderly patients in the terminal stage of life as study subjects. Surveys were conducted using a general information questionnaire, the palliative care Decision-Making Participation Scale, the Palliative Care Attitude Scale, the Thr

Dry Eyes among Visual Display Terminal Users Visiting the Outpatient Department of Ophthalmology of a Tertiary Care Centre.

INTRODUCTION: Visual display terminal usage has increased substantially in recent years across all age groups and is considered one of the major risk factors for dry eye disease. Early assesment of dry eyes and prevention is important. The aim of this study is to find the prevalence of dry eyes among visual display terminal users visiting the Outpatient Department of Ophthalmology of a tertiary care centre. METHODS: A descriptive cross-sectional study was conducted among patients visiting the Department of Ophthalmology in a tertiary care centre after receiving ethical approval from the Insti

End-of-life care among terminally ill patients in the emergency department: a best practice implementation project.

INTRODUCTION: The increasing number of older adults visiting emergency departments (EDs) near the end of life highlights the need for palliative and end-of-life care in this setting, despite the ED's focus on acute care. AIM: This study implemented evidence-based end-of-life care for terminally ill adult patients in the ED in a medical center in Shanghai, China. METHODS: This project followed JBI Evidence Implementation Framework. A baseline audit was conducted to measure current practices against best practice recommendations. Barriers to evidence-based practices were identified, improvemen
