This study examined the implications of left ventricular (LV) dilatation and increased pulmonary thallium uptake during adenosine-induced coronary hyperemia. A case of single left coronary artery is presented with the correlation between the results of thallium-201 (T1-201) myocardial single photon emission computed tomography (SPECT) and coronary angiographic findings. The chest pain in this case might be partly secondary to the myocardial ischemia, under stress, which could result from the steal phenomenon to normal perfusion myocardium due to anatomical anomaly of single left coronary artery. Our findings suggest that T1-201 myocardial SPECT could be a useful method for detecting myocardial ischemia in patients with single left coronary artery.

Five patients with AIDS with Pneumocystis carinii pneumonia (PCP) showed diffuse bilateral lung uptake on sequential thallium and gallium scans. This creates a problem in the sequential use of these tests for the differentiation of Kaposi's sarcoma from acute inflammatory or other malignant diseases. In Kaposi's sarcoma, thallium uptake is usually focal and not diffuse unless associated with other pathology. However, in PCP the diffuse thallium lung uptake may be due to congestion, increased vascular permeability, and extravasation of thallium into the lung parenchyma.

We performed exercise stress scintigraphy with thallium-201 in 88 patients including 31 patients of effort angina and nine patients of old myocardial infarction without angina. Sensitivity of exercise stress scintigraphy to detect effort angina was 77%, whereas sensitivity of exercise stress ECG was only 44.5%. The combination of stress scintigraphy with stress ECG enhanced sensitivity up to 87%. Thallium-201 has been evaluated for myocardial imaging by determining its distribution and assessing its imaging properties. Organ distribution with time was studied in goats, chosen for their large size and easy operability. Daphnia magna was used to evaluate the aquatic toxicity of Tl(III) compounds including Tl(III) nitrate, Tl(III) chloride, and Tl(III) acetate. The results clearly show that Tl(III) is extremely toxic to daphnids. The 48-h LC50 values for Tl(III) nitrate, Tl(III) chloride, and Tl(III) acetate are 24, 61, and 203 microg/L, respectively. Tl(III) is much more toxic than Tl(I) and many other metals such as Cd(II), Cu(II), and Ni(II); it is similar to the toxicity that of Hg(II). The formation of Tl(III)-complexes would significantly reduce Tl(III) toxicity.

This case report describes a patient with thallium poisoning caused by repeated exposure to low doses of thallium. Alopecia and nail changes were the most prominent features of this case. There was dystrophy of nails in the form of whitish lunular stripes. This is the first report of complete erosion of proximal parts of nails following thallium poisoning. This case is the first report of thallium poisoning from India occurring from repeated low dose exposure.
