Thiamin deficiency in a colony of cats.

Twenty-eight cats belonging to a cat rescue society developed clinical signs consistent with thiamin deficiency after being fed a proprietary canned food containing inadequate amounts of the vitamin. Five of the cats died but the others recovered after appropriate therapy. The diagnosis was based on clinical signs, histopathological findings, food analysis and response to treatment.

[Thiamin deficiency is common in Israel].

Diseases due to vitamin B1 (thiamin) deficiency are considered to be uncommon in Israel. We present 13 patients admitted during the course of a year who were found to have thiamin deficiency. 5 had classic syndromes such as beri-beri, Wernicke's encephalopathy and cerebellar degeneration. In another 8 the thiamin deficiency was found during workup for less specific neurological abnormalities. Only a minority were alcoholics. Mainly at risk for developing nutritional deficiency were the solitary elderly of low socio-economic status with poor nutrition. Other reasons for thiamin deficiency were

Hemorrhage of thiamine-deficient encephalopathy.

Hemorrhagic lesions of pyrithiamine-induced acute thiamine-deficient encephalopathy of the mouse (PIATDEM) consisted of petechiae, which often coalesced to form small hematomas. Electron microscopy showed the typical petechial lesion to be composed of a perivascular necrotic zone containing fibrin-platelet clot surrounded by a ring of erythrocytes. Endothelial cells were intact and tight junctions were closed. A study of permeability to horseradish peroxidase (HRP) revealed only a slight increase in the number of transport vesicles in the endothelial cells. A large amount of HRP present in the

Induced thiamin deficiency in lambs.

By following a feeding regimen which consistently induced polioencephalomalacia in pre-ruminant lambs it was possible to study certain characteristics just before the terminal stage in polioencephalomalacia. There was always a marked deficit in erythrocyte precursors in bone marrow and this preceded any pathological changes in the brain. Erythrocyte transketolase activity decreased in control lambs, and decreased to an even greater extent in thiamin-deficient lambs. Glucose was the only substrate of those measured which was used by the brain, and its rate of use was not affected by thiamin def

The thiamine-dependent hysteretic behavior of human transketolase: implications for thiamine deficiency.

We have investigated the hysteretic properties of human transketolase with emphasis on its dependency on thiamine pyrophosphate concentratio … l type variation in the lag period suggest mechanisms for the loss of transketolase activity during thiamine deficiency and may explain, at least in part, the differential sensitivity to deficiency demonstrated by tissues and individuals.

Thiamine deficiency disorders: a clinical perspective.

Thiamine is an essential water-soluble vitamin that plays an important role in energy metabolism. Thiamine deficiency presents many challenges to clinicians, in part due to the broad clinical spectrum, referred to as thiamine deficiency disorders (TDDs), affecting the metabolic, neurologic, cardiovascular, respiratory, gastrointestinal, and musculoskeletal systems. Concurrent illnesses and overlapping signs and symptoms with other disorders can further complicate this. As such, TDDs are frequently misdiagnosed and treatment opportunities missed, with fatal consequences or permanent neurologic …

Thiamine deficiency in tachypnoeic Cambodian infants.

BACKGROUND: Beriberi is endemic in South-east Asia. Diagnosis is based on clinical findings, but correlation of clinical features with blood … tial TDP levels. Use of infant formula was associated with higher initial TDP levels. CONCLUSIONS: Thiamine deficiency is common in tachypnoeic Cambodian infants, but routine clinical assessments do not accurately identify those with low blood TDP concentrations. Parenteral thiamine administration markedly increases TDP levels. Empirical thiamine treatment should be …

Mitochondrial myopathy and familial thiamine deficiency.

We studied two siblings with a mitochondrial myopathy, familial thiamine deficiency, and an A3243G mutation of the mitochondrial DNA (mtDNA). The elder brother (patient 1, now 36 years old) developed myopathy and beriberi heart at 20 years of age. Thiamine therapy resolved the cardiac symptoms and hyperpyruvicemia and improved the myopathy. The younger brother presented aged 19 years with a myopathy (patient 2, now 35 years old). Thiamine deficiency was present in the siblings and parents, and ragged-red fibers (RRFs) were noted in muscle biopsies from the siblings. Analysis 17 years later dem…

Thiamine deficiency in hospitalized elderly patients.

Necropsy studies suggest that thiamine deficiency is underdiagnosed in life, in part because the classical clinical presentations are uncommon. Anecdotal reports suggest that thiamine deficiency may contribute to the development of delirium, heart failure and peripheral neuropathy in elderly patients, but little systematic research has been reported. We examined thiamine levels in 36 consecutive non-demented, community-dwelling patients admitted to an acute geriatric unit. Marginal thiamine deficiency [thiamine pyrophosphate effect (TPPE) 15-24%] was present in 11 (31%) and definite thiamine d…

Thiamine deficiency in the critically ill.

There have been recent reports of hospitalised patients developing clinical thiamine deficiency, combined with much debate on the optimal supplementation of thiamine for the parenterally fed patient, particularly in the intensive therapy environment. We performed a retrospective study on 158 patients admitted to the Intensive Care Unit who required nutritional support. Patients who survived had significantly higher body thiamine status than those who died (p less than 0.01). There was no difference between serum albumin concentrations of the two groups. Twenty percent of the patients had bioch…

[Glucose-induced hyperlactatemias in thiamine deficiency].

Parenteral nutrition with carbohydrates is limited, inter alia, by the occurrence of a dose-dependent hyperlactataemia. Thiamine deficiency, which can be provoked by an eight weeks' diet which is deficient in thiamine, will produce clearly elevated lactate levels in rats, leading eventually to lactate acidosis as a result of glucose load, compared with a control group on a normal diet. It is recommended to initiate a high-dosage level thiamine therapy before parenteral feeding with carbohydrates, if thiamine deficiency appears possible on the grounds of previous history of the case (alcohol ab…

Thiamine deficiency in general hospital patients.

The production of sedoheptulose-7-phosphate in the red blood cells of 15 patients was found to be below the suggested normal value. This intermediate product of glucose metabolism is the result of transketolase activity for which thiamine pyrophosphate is one of the co-factors. Without thiamine, the transketolase reaction is inhibited. Some nonlocalizing symptoms, perhaps previously thought due to hypoglycemia, may be occasioned by faulty glucose metabolism at the cellular level from a relative lack of thiamine.

Polioencephalomalacia of dogs with thiamine deficiency.

A naturally occurring neurological disease occurred in six dogs fed cooked meat. Clinical signs were anorexia, progressive spastic paraparesis, recumbency, convulsions and death. The disease was characterized by bilaterally symmetrical spongy change and necrosis of brainstem nuclei with a lesion distribution pattern similar to that in thiamine deficient foxes and cats. An associated thiamine deficiency was evidenced by decreased thiamine levels in the blood of one dog and in the food of another, and rapid remission of clinical signs in a dog given thiamine hydrochloride. Thermal destruction of…
