A Review of External Cervical Resorption.

External cervical resorption (ECR) is a relatively uncommon yet aggressive form of dental hard tissue destruction. It is initiated at the cervical aspect of the root surface and extends apicocoronally and circumferentially inside the dentin. Despite the large number of case reports and clinical studies that have investigated ECR, its etiology remains unclear. Recent advancements in clinical assessment measures, such as the use of cone-beam computed tomographic imaging, have provided additional insights into the nature of this lesion. This has facilitated the continued development and improveme

Reestablishing biologic width with forced eruption.

Maintenance of gingival health is one of the keys for the longevity of teeth, as well as for the longevity of restorations. In this context, the biologic width functions as a barrier against the entrance of microorganisms into the internal medium of the periodontal ligament and into the gingival and osseous connective tissue. This clinical case describes a technique to reestablish the biologic width of a central incisor using forced extrusion and done without post-treatment corrective surgery.

Diagnostic Accuracy of Cone-Beam Computed Tomography and Periapical Radiography in Detecting External Cervical Resorption: A Systematic Review and Meta-Analysis.

This study assessed the diagnostic accuracy of periapical radiography and cone-beam computed tomography for detecting external cervical resorption. An electronic search was conducted in seven databases. The protocol was registered in PROSPERO, and the risk of bias was assessed with QUADAS-2. Retrospective studies using human dental images were included. A bivariate random-effects model estimated pooled diagnostic values using MetaDisc v1. Four studies met the inclusion criteria. Three demonstrated a high risk of bias in patient selection. The meta-analysis revealed specificity values of 0.93 f

Esthetic crown lengthening: appropriate diagnosis for achieving gingival balance.

A gummy smile poses a restorative challenge for dentists attempting to achieve ideal esthetics. Many have advocated the use of a gingivectomy or laser-assisted gingival contouring procedure. However, this simplistic approach can potentially create a mucogingival defect or a biological width violation. To avoid these periodontal-restorative complications, it is important during treatment planning to assess the anatomical relationship that resulted in the gummy smile and choose the appropriate surgical treatment to eliminate this condition.

Talon cusp: an overview with case reports of 3 clinical variants.

Talon cusp is a dental anomaly, which is present as an accessory cusp and is seen both in the deciduous and permanent dentition. It may occur unilaterally or bilaterally and has been reported mostly in maxillary teeth, with a few cases occurring in the mandible. Talon cusp is usually asymptomatic; however, there are many clinical problems associated with it. Talon cusp is clinically manifested in three forms: talon, semi-talon and trace talon. In this article, we present four case reports of talon cusp with an overview of different clinical variants of talon cusp, classification, etiopathogene

Accessory foramina in the furcation area of primary molars. A SEM investigation.

AIM: The accessory canals in the furcation of primary molars can explain the frequent presence of radiolucency in the furcation area in case of pulpitis and necrosis of teeth. In fact these canals can represent a communication between periodontium and pulp tissue allowing bacteria to enter. The aim of this study was to determine the prevalence of foramina in the furcation area of primary molars. MATERIALS AND METHODS: The presence of foramina was assessed using extracted deciduous molars examined with Scanning Electron Microscope (SEM) evaluation. Thirty extracted human primary molars (15 upp

External Cervical Resorption Treatment: A Single-Center Retrospective Cohort Study of Cases Treated Over a 20-Year Period.

AIM: External cervical resorption is a clinically challenging destructive process, with its etiology poorly understood. This study aimed to evaluate the survival of external cervical resorption treatments performed by a single operator over 20 years. MATERIALS AND METHODS: A dataset of recalled external cervical resorption cases treated from 1996 to 2023 was created. The measured metrics included Heithersay classification, tooth type, historical treatment period, treatment modality, and pre-treatment pulp status. Adverse sequelae such as extractions, revisions, and surgeries were analyzed. A

Orthodontic extrusion of subgingivally fractured incisor before restoration. A case report: 3-years follow-up.

Orthodontic forced eruption may be a suitable approach without risking the esthetic appearance in tooth fracture below the gingival attachment or alveolar bone crest. Extrusion of such teeth allows elevating the fracture line above the epithelial attachment and so the proper finishing margins can be prepared. Restoration after orthodontic eruption may present a more conservative treatment choice in young patients compared with the prosthetic restoration after extraction. This case describes a multidisciplinary approach using the orthodontic forced eruption facilitating the composite restoratio

Coronal displacement of cementum: correlation between age and coronal movement of cementum in impacted teeth.

This study was conducted on 48 impacted and 51 erupted maxillary permanent canine teeth extracted from healthy patients aged 13-73 years. Longitudinal buccolingual ground sections were prepared. The distance between the edges of cementum and enamel in each specimen was measured with an eyepiece micrometer. A correlation test was applied between age and the distance between enamel and cementum measurements. In impacted teeth, depending on age, cementum had a tendency to overlap the enamel; there was a linear correlation between age and coronal displacement of cementum (n-2 = 46, r = 0.69, dista

An in vitro evaluation of radicular penetration of hydrogen peroxide from bleaching agents during intra-coronal tooth bleaching with an insight of biologic response.

OBJECTIVES: External root resorption is the complication of intra-coronal bleaching done with 30% H2O2 alone or mixed with sodium perborate but not with sodium perborate mixed with water. The study was done to comparatively evaluate the H2O2 leakage from three H2O2 liberating bleaching agents. STUDY DESIGN: Fifty one single rooted human teeth were used. After root canal therapy gutta percha was removed below cemento-enamel junction. Three bleaching agents: sodium perborate mixed with water, sodium perborate mixed with 30% H2O2 and 30% H2O2 alone were used. Teeth without defect, with cervical

Local opioids in a model of periodontal disease in rats.

There is growing evidence for the participation of opioid receptors in the development of inflammation. The present study was designed to clarify the role played by opioid receptors in periodontal disease. Periodontal disease was induced by placing a sterile silk ligature around the cervix of the second maxillary tooth on day 0. Morphine was administered either systemically or locally before and after the onset of periodontal disease. The results showed that in both patterns, morphine treatment reduced fiber attachment and alveolar bone loss, without affecting the increased leukocyte count in

Treatment of severe localized periodontal destruction associated with a cemental tear: a case report and review of the literature.

Cemental tears have been described as a fragment of cementum becoming detached from the root surface, which can lead to severe periodontal breakdown. A 50-year-old woman with no history of periodontal attachment loss had an asymptomatic, isolated deep pocket on the mesial aspect of tooth No. 14. The radiographic examination revealed a thin, chip-like radiopacity along the lateral aspect of tooth No. 14. Periodontal flap surgery revealed a cemental tear on the apical part of the tooth. The root fragment was removed and the intrabony lesion was treated with enamel matrix derivative. A clinical e

Treatment of External Cervical Resorption via Surgical Crown Lengthening and Concomitant Placement of a Resin-Modified Glass-Ionomer Restoration: A Case Series with 1- to 4-Year Follow-up.

External cervical resorption is a disease entity with an idiopathic etiology by which osteoclastic invasion cavitates teeth near the cementoenamel junction. The lesion, if left untreated, has the potential to progress and compromise tooth maintainability. Treatment of these lesions requires access (often surgical), removal of the affected tissue, and placement of a restorative material. This case series documents 10 external cervical resorption lesions that were treated by surgical crown lengthening with concurrent placement of a resin-modified glass-ionomer restoration and followed up for a p

Multiple Cervical Root Resorption Involving 22 Teeth: A Case with Potential Genetic Predisposition.

A rare case of extensive multiple idiopathic cervical root resorption with potential genetic predisposition was presented. A heathy 19-year-old Chinese male with no contributory medical or family/social history complained of pain during mastication that lasted for several months. Oral examination identified 7 missing teeth and external cervical root resorption involving 9 teeth. Comparison of orthopantomograms taken in May 2021 and February 2022 identified that cervical root resorption occurred in 22 teeth. Resorption commenced at the cementoenamel junction and progressed rapidly over the 9-mo

An evidence-based system for the classification and clinical management of non-proximal gingival recession defects.

Gingival recession defect (GRD) may be defined as an apical migration of the gingival margin respective to the cementoenamel junction resulting in partial exposure of the root surface to the oral cavity, which may have important esthetic, functional, and periodontal health implications. A novel system for the classification and management of non-proximal GRDs is proposed in this article. This evidence-based system consists of two essential components: (1) Establishment of the GRD type based on the midbuccal/midlingual attachment level respective to the interproximal bone level, and (2) Assessm

[Failure in the orthodontic treatment of impacted maxillary canines].

Maxillary canine impaction occurs in 1-3% of most Western populations and its orthodontic treatment is often difficult. Failure to resolve the impaction is not uncommon and may lead to malpractice lawsuits. The information in the literature about the reasons for non-resolution of this condition is sparse and unsubstantiated. This encourages the dispensing of inappropriate treatment that may result in severe tooth, soft and hard tissue damage as well as a prolonged treatment period. This review describes the epidemiology, pathogenesis and the wide range of reasons that may lead to failure to re
