Trabeculectomy fails to control the intraocular pressure (IOP) adequately in some cases. The effect of the enzyme hyaluronidase--300 IU Hylase 'Dessau' (commercially available ampuls)--applied in subconjunctival injection in the region above the filtering bleb in case of postoperative rise in IOP following trabeculectomy was studied. Congenital glaucoma is one of the main causes of blindness in children and is more common in Saudi Arabia than in Western countries, perhaps owing to the prevalence of consanguinity with families. Trabeculotomy has given poorer in Saudi Arabia than elsewhere. This study compared the results of trabeculotomy (67% success rate) with trabeculectomy (54% success rate), and reports fewer complications following trabeculotomy.

Two cases of a benign form of optic disc edema after successful trabeculectomy are reported. In both patients, optic disc edema was noted 2 to 4 weeks after trabeculectomy. The edema occurred without loss of visual acuity or field. The absolute intraocular pressure and intracranial pressure were normal--that is, the edema was not a syndrome of hypotony or pseudotumor cerebri. The anterior chamber depth was measured with the Haag-Streit Pachymeter in 26 cases (33 eyes) of primary angle-closure glaucoma preoperatively and postoperatively on days 1, 2, 3, 4, 5, 6, 8, 10, 12, 14, 30, 45 and 60. After successful trabeculectomy, the anterior chamber depth became shallower, particularly on days 2 to 4 postoperative, and then gradually deepened after day 5. By day 14 after surgery, the anterior chamber depth was recovered up to 91% of the preoperative level, and by day 30 up to 93%, to remain essentially stable thereafter.

Five cases of delayed nonexpulsive suprachoroidal hemorrhage after trabeculectomy in aphakic eyes are reported. Four cases were done under general anesthesia and in three there was straining and bucking at extubation. The most common presentation was sudden severe ocular pain one day postoperatively, associated with marked decrease in vision and low intraocular pressure.
