Alterations in immune competence in outcome of coma following severe head trauma.

The homeostasis established by the interaction among the central nervous system, the immune system and the endocrine system, is radically altered by prolonged comatose state following severe head trauma. This must be taken into account when deciding on therapy and rehabilitation. In a group of post-comatose patients, we studied peripheral lymphocyte subpopulations, defined by monoclonal antibodies, and delayed hypersensitivity. Our findings are indicative of an immunological impairment originating at glioneuronal level, following head trauma. Such a temporary immune deficiency may in turn affe

Immunophilin dysfunction and neuropathology.

In case of nervous damages, like nervous system trauma or various neurodegenerative diseases such as dementia or Parkinson, several treatments are available to restore neurological function. In spite of these treatments, results are often insufficient or not satisfactory in many neurologic diseases, especially for central nervous system (CNS) lesions. To minimize neurological dysfunction, it is critical to reduce neuronal death, avoiding loss of the synaptic connections, and securing viable neurons to extend axons. Unfortunately, there are no effective strategies to fulfill these basic needs e

Prophylactic antibiotic use for penetrating trauma in prolonged casualty care: A review of the literature and current guidelines.

Prolonged casualty care (PCC), previously known as prolonged field care, is a system to provide patient care for extended periods of time when evacuation or mission requirements surpass available capabilities. Current guidelines recommend a 7- to 10-day course of ertapenem or moxifloxacin, with vancomycin if methicillin-resistant Staphylococcus aureus is suspected, for all penetrating trauma in PCC. Data from civilian and military trauma have demonstrated benefit for antibiotic prophylaxis in multiple types of penetrating trauma, but the recommended regimens and durations differ from those use

[Management of chronic postoperative pelvic and perineal pain due to parietal somatic nerve damage].

INTRODUCTION: All surgical procedures require an incision with a risk of nerve damage at the site of the scar or as a result of fibrotic scar tissue. The purpose of this article is to describe the management of chronic postoperative pelvic and perineal pain due to parietal somatic nerve damage. PATIENTS AND METHODS: A comprehensive review of the literature was performed by searching PUBMED for articles on the management of chronic postoperative pelvic and perineal pain due to parietal somatic nerve damage. RESULTS: Postoperative lesions of parietal somatic nerves (ilioinguinal, iliohypogastr

General metabolism in head injury.

Seventy-six patients with closed head injuries alone were studied to define the relation between the severity of the head injury and secondary alterations of general metabolism. The effect of metabolic changes on neurological outcome and the importance of nutritional support on nutritional status and neurological outcome were also evaluated. Using a powerful statistical tool, convergence analysis, it was possible to take into consideration the effects of a number of confounding factors that obviously affected general metabolism. Most of the patients were hypermetabolic for prolonged periods. I

Minimizing the risk of neurologic injury in gynecologic surgery.

OBJECTIVE: The association of intraoperative neurologic injuries with gynecologic surgical procedures is well established. The sequelae of such injuries are usually transient and resolve with minimal intervention, although long-term disability can and does occasionally occur. The purpose of this study was to examine the mechanisms by which these injuries occur in order to reduce the risk of their occurrence. DATA SOURCES: A MEDLINE search was performed cross-referencing the index terms "neurological injury" and "gynecological surgery," from January 1, 1960 to December 31, 2002. METHODS OF ST

Severe posttraumatic craniocervical instability in the very young patient. Report of three cases.

Children younger than 3 years of age represent a distinct subpopulation of patients at particular risk for high cervical and craniovertebral injuries. There are few descriptions of survivors of severe craniocervical trauma among the very young, and scarce data exist regarding management after initial emergency stabilization. The authors describe three children, age 1 to 32 months, who presented with craniocervical junction injuries. Variable neurological findings were observed at presentation (cranial nerve deficits, obtundation, and moderate-to-severe quadriparesis). All three were treated wi

[Neuropathic pain: experimental advances and clinical applications].

Neuropathic pain is a clinical entity designating the different types of pain associated with a lesion of the nervous system including a wide range of pathological conditions from painful peripheral lesions (for example diabetic neuropathy, post-zoster pain, trauma-induced nerve injury) and central pain (particularly stroke-induced pain, spinal lesions, and multiple sclerosis). Despite this wide range of etiologies, neuropathic pain has well characterized clinical features which generally allow distinction from other types of pain: continuous often burn-like pain, paroxysmal pain (electrical d

[Problems of perinatal neurology].

An analysis of more than 1000 cases of early organic brain lesions has demonstrated that in 54% of them the etiology of neurological disturbances was an antenatal pathology, in 38%-intranatal and in 8%-postnatal. The prevention of perinatal disorders of the nervous system is considered as a complex problem including measures of health prevention for the future mothers, control over the development of pregnancy, perfection of obstetric aid, effective measures for the prevention of tissue hypoxia and its sequalae, elaboration of methods of rehabilitative therapy, work with personnel.

The Role of Neuro-Immune Interaction in Chronic Pain Conditions; Functional Somatic Syndrome, Neurogenic Inflammation, and Peripheral Neuropathy.

Functional somatic syndromes are increasingly diagnosed in chronically ill patients presenting with an array of symptoms not attributed to physical ailments. Conditions such as chronic fatigue syndrome, fibromyalgia syndrome, or irritable bowel syndrome are common disorders that belong in this broad category. Such syndromes are characterised by the presence of one or multiple chronic symptoms including widespread musculoskeletal pain, fatigue, sleep disorders, and abdominal pain, amongst other issues. Symptoms are believed to relate to a complex interaction of biological and psychosocial facto

Ultrasound evaluation of focal neuropathies in athletes: a clinically-focused review.

Focal neuropathies represent a rare but clinically important and potentially challenging aetiology of pain in athletes. Diagnostic ultrasound is commonly used in the evaluation of nerve entrapments, and has several advantages over other imaging modalities, including high resolution, portability, lack of ionising radiation, low cost, point-of-care access, ease of contralateral comparison and capability of Doppler and dynamic imaging techniques. In this review, we discuss the use of ultrasound for the evaluation of injuries to the brachial plexus including 'stingers,' suprascapular nerve, ulnar

Laser vs. suture nerve anastomosis.

The repair of injured or transected nerves is frequently encountered in head and neck surgery. Recently, CO2 lasers with milliwatt capability and micrometer spot sizes have been developed that may be used in nerve anastomosis. A comparative study was performed between microsuture and CO2 laser repair of transected sciatic nerves in rats. Nerve regeneration was measured in terms of morphology, electrophysiology, and function. Histologic studies revealed no difference in the size and number of regenerated axons, although there was less scar tissue formation at the anastomotic site with the laser

Prevention of injury to the great auricular nerve during rhytidectomy.

The great auricular nerve is the one most commonly injured during rhytidectomy. The main trunk is closest to the surface over the midtransverse belly of the sternocleidomastoid muscle. At this point, the nerve is 6.5 cm below the caudal edge of the bony external auditory canal. The external jugular vein is parallel and 0.5 cm ventral to the nerve at the same point. A simple technique using these fixed anatomic landmarks helps the surgeon avoid injury to the great auricular nerve and the jugular vein.

Risk factors for postoperative cognitive dysfunction in geriatric trauma: a dual-pathway perspective.

Postoperative cognitive dysfunction (POCD) is common in geriatric trauma patients and involves complex underlying mechanisms. Most existing studies have focused on isolated risk factors, with limited attention to how these factors interact. In this perspective article, we propose a dual-pathway theoretical model-central nervous system vulnerability and peripheral stress amplification-to reframe the understanding of POCD pathogenesis. In older adults, POCD rarely stems from a single cause. Instead, it arises from a "second hit" process, in which pre-existing central nervous system vulnerabiliti
