Pediatric vs Adult or Mixed Trauma Centers in Children Admitted to Hospitals Following Trauma: A Systematic Review and Meta-Analysis.

IMPORTANCE: Adult trauma centers (ATCs) have been shown to decrease injury mortality and morbidity in major trauma, but a synthesis of evidence for pediatric trauma centers (PTCs) is lacking. OBJECTIVE: To assess the effectiveness of PTCs compared with ATCs, combined trauma centers (CTCs), or nondesignated hospitals in reducing mortality and morbidity among children admitted to hospitals following trauma. DATA SOURCES: MEDLINE, Embase, and Web of Science through March 2023. STUDY SELECTION: Studies comparing PTCs with ATCs, CTCs, or nondesignated hospitals for pediatric trauma populations (

Instruments of Insight: Cognitive Tool Use by Occupational Therapy Practitioners at Midwest Trauma Centers.

In the hospital setting, standardized cognitive screening and assessment tools are critical to determine the presence of cognitive changes for occupational therapy discharge planning and intervention approaches. However, there is limited understanding of trends related to standardized tool availability and use across different trauma center designations. This study aimed to identify commonly used standardized cognitive screens and functional performance assessments, and to explore factors influencing their selection and use. Electronic surveys were employed with a total of 28 questions. Two hu

Unique Indications for Covered Stent Grafts in Neuroradiology Departments of Level 1 Trauma Centers.

OBJECTIVE: To describe unique indications for covered stent grafts in trauma-associated cerebrovascular injuries. PATIENTS: Between 2006 and 2018, five patients with cerebrovascular injuries were treated with a covered stent graft. We present a retrospective analysis of technique and outcomes. RESULTS: In all cases stent deployment was successful. Endoleaks occurred in two cases requiring additional transvenous embolization of a carotid cavernous fistula (CCF) in one patient. Two cases of in-stent thrombosis were observed during intervention and 2 days postintervention in a patient with a lo

Evaluation and Implication of Case Volume Variation in Level 1 and 2 Trauma Centers.

OBJECTIVE: To evaluate variation in case volume and procedural volume across level 1 and 2 U.S. trauma centers. BACKGROUND: When trauma center distribution does not fit regional needs, the longstanding volume-outcomes relationship in trauma care is at risk. Case volume variability has important implications for trauma center distribution, patient outcomes, and clinical skills maintenance. METHODS: We placed trauma centers into quintiles based on average annual patient volume meeting American College of Surgery Trauma Quality Improvement Program (ACS TQIP) inclusion criteria from 2017 to 2021

Investigating the Burden of Traumatic Injuries and Access to Trauma Centers in Rural Riyadh.

INTRODUCTION: Trauma is a major cause of death and disability among young adults in Saudi Arabia, with a road traffic fatality rate three times higher than other high-income countries. The vast expanse of the Riyadh region comprises 21 governorates over 156,078 square miles, most of which is rural. Although delays in access to trauma care pose a significant mortality and economic burden, regions at highest risk of delays have not been previously studied. This paper aims to identify the trauma center distribution, trauma burden, and areas with delayed access to trauma care in the rural governor

"Slow" Trauma to Families with the Demise of Family-Centered Care and Trauma-Informed Care.

AbstractIn the article "Slow Trauma: Reframing Concepts of Moral Injury in Trauma-Informed Ethics Consultation," Charles Love argues that family members are at significant risk for "moral injury and trauma" through the experience of the hospitalization of their loved ones. But Love adds an important insight about the type of moral injuries experienced by family members, distinguishing between what psychologists refer to as "fast" trauma and "slow" trauma. Love's detailed exposition of slow trauma is an important refinement of my article "TIEC, Trauma Capacity, and the Moral Priority of Surroga

Age and trauma center-specific disparities in splenectomy rates for pediatric trauma patients.

BACKGROUND: Non-operative management (NOM) of blunt splenic injury (BSI) is the target for pediatric trauma care. Yet, the rates of splenectomy have historically been higher at non-pediatric trauma centers (PTCs). We sought to determine if this difference improved contemporaneously. METHODS: Using the National Trauma Data Bank, 26 707 pediatric patients (age ≤18 years, 2013 to 2021) with BSI were identified and analyzed based on care at adult trauma centers (ATCs), dual trauma centers (DTCs), or pediatric trauma centers (PTCs) using descriptive statistics and logistic regressions.
