Neuromuscular hamartoma of the trigeminal nerve in an adult.

A 36-year-old female presented with a long-standing history of headache. Computed tomography scan and magnetic resonance imaging revealed an enhancing lesion occupying the right Meckel's cave, enlarging the trigeminal ganglion and extending through the foramen ovale into the infratemporal fossa. A right frontotemporal extradural approach to the cavernous sinus was performed and a firm, pinkish lesion intermingled with nerve fibers enlarging the trigeminal ganglion and V3 branch was identified. Frozen section was suggestive of a neuromuscular hamartoma. The lesion was sub-totally resected to av

Benign triton tumor of the trigeminal nerve.

Benign triton tumor (neuromuscular hamartoma) is a rare tumor of the peripheral nerves, comprising mature neural and striated muscle elements. We report the third case, according to the literature, of a benign triton tumor affecting the Vth cranial nerve in a 4-year-old male child. The tumor was seen to involve the mandibular division of the trigeminal nerve in the infratemporal fossa and extend intracranially by eroding the base of middle cranial fossa. The mass was totally excised. In the tumor, bundles of axons intermingling with fascicles of relatively mature skeletal muscle were found. Th…

Probable trigeminal nerve schwannoma in a dog.

A 7-year-old male Husky dog developed atrophy of the right masseter muscle and pruritus of the right side of the face. A myogenic origin was excluded using muscular biopsy. Electrophysiologically, there was involvement of the motor and sensory fibers of the trigeminal nerve, suggesting a lesion located between the brainstem and the trigeminal ganglion. On MRI examination, a nodular mass was detected in the right caudal fossa. This mass was characterized by intense enhancement after injection of contrast medium. Because of the progressive clinical signs, electrophysiology, and MRI results, a pr…

[Involvement of the trigeminal nerve in fractures of the face].

It is an anatomical particularity of the trigeminal nerve that his three main branches evolve in its peripheral section via a relatively long passage up to an osseous canal. We have tried to provide, apart from a quantitative and qualitative analysis of the lesions of the trigeminal nerve in case of facial fractures, a statement as to their topography in order to enable prognosis as to such lesions. For the study, we used the data of 328 patients with a total of 468 fractures. All patients were asked to provide detailed information about any pre- and postoperative loss of sensibility in the ar…

Diagnosis and treatment of trigeminal nerve injuries.

Microneurosurgery has become a valuable technique in the field of oral and maxillofacial surgery. It is now possible to microsurgically repair injuries to the inferior alveolar and lingual nerves resulting from routine oral surgery or dental or oncologic treatment. Sensation may be improved or restored, and painful pares thesias relieved, by timely, well-performed microsurgery.

[Condylar fractures and involvement of the trigeminal nerve].

Although fractures of the condylar neck belong to the most frequent injuries of the jaws, and various nerves are found in close proximity to the temporomandibular joints, only little mention is made in the literature of the neurological complications. An unusual case of V2 and V3 nerve injury is presented. The literature is reviewed and the mechanism of the nerve injuries are discussed, 21 cases are reported.

The trigeminal (V), an oft-neglected nerve.

Corneal denervation is one of the most serious insults that can occur to an eye; disease symptoms can be suppressed, and epithelial breakdow … cranial nerve defect, cranial autonomic dysfunction, or unexplained corneal epithelial defect. The trigeminal nerve is often neglected, but corneal anesthesia aids in the diagnosis of intracranial disease and dictates the need for corneal observation and protection.

Case report: Primary ependymoma of the trigeminal nerve presenting as trigeminal neuralgia.

BACKGROUND: Ependymomas are usually found in the posterior fossa originating from the fourth ventricle. Primary ependymomas arising from cranial nerves are rare with only a handful of reported cases. Trigeminal neuralgia (TN) is rarely due to space occupying lesions. CASE DESCRIPTION: A 20-year-old female presented with TN with a rare presentation of a pure extra-axial ependymoma involving the right trigeminal nerve in the cerebellopontine angle. CONCLUSION: It is essential to explore the possibility of a mass arising from the trigeminal nerve when investigating the cause of TN.

[Trigeminal nerve lipoma presenting with trigeminal neuralgia: case report and literature review].

Cerebellopontine angle lipomas are benign mass lesions and rarely result trigeminal neuralgia. A 61-year-old male with right-sided trigeminal neuralgia in V2 and V3 divisions without sensory disturbances is reported in the article. MRI revealed mass lesion 11´11´4 mm on the lateral pontine surface spreading to the right trigeminal nerve root entry zone. No signs of neurovascular compression were found. Microsurgical exploration of the cerebellopontine angle showed a fatty mass adherent to the brainstem with incorporation of inferior part of trigeminal nerve root. Fatty tissue resection was fol…

Cavernous malformation of the trigeminal nerve manifesting with trigeminal neuralgia: case report.

OBJECTIVE AND IMPORTANCE: We describe a patient with a cavernous malformation within the trigeminal nerve at the nerve root entry zone who presented with trigeminal neuralgia. CLINICAL PRESENTATION: A 52-year-old woman sought treatment after experiencing dizziness and lancinating left facial pain for almost a year. Neurological examination revealed diminished sensation in the distribution of the trigeminal nerve on the left. Magnetic resonance imaging demonstrated a minimally enhancing lesion affecting the trigeminal nerve. INTERVENTION: The patient underwent a retrosigmoid craniotomy. At th…

MRI of the Trigeminal Nerve in Patients With Trigeminal Neuralgia Secondary to Vascular Compression.

OBJECTIVE: Trigeminal neuralgia is a debilitating facial pain disorder, frequently caused by vascular compression of the trigeminal nerve. Vascular compression that results in trigeminal neuralgia occurs along the cisternal segment of the nerve. CONCLUSION: Imaging combined with clinical information is critical to correctly identify patients who are candidates for microvascular decompression. The purpose of this article is to review trigeminal nerve anatomy and to provide strategies for radiologists to recognize important MRI findings in patients with trigeminal neuralgia.

Surgical skills of adhesions and transposition of trigeminal nerve for primary trigeminal neuralgia.

OBJECTIVE: The aims of this study were to investigate disposal of adhesions and transposition of the trigeminal nerve during microvascular decompression and evaluate its surgical effect. METHODS: By collecting the clinical data of typical trigeminal neuralgia patients we treated from January 2013 to May 2013, we chose 120 patients with adhesions and transposition of trigeminal nerve, analyzed their preoperative imaging features and surgical procedures, and evaluated postoperative effect after 3-month follow-up. RESULTS: Among these 120 patients, 113 cases showed positive effect in magnetic r…

Intrinsic arteriovenous malformation embedded in the trigeminal nerve of a patient with trigeminal neuralgia.

A 66-year-old man presented with typical right trigeminal neuralgia. Neuroimaging showed a small arteriovenous malformation (AVM) in the right cerebellopontine angle. Suboccipital craniotomy verified that the AVM was almost completely embedded in the root entry zone of the trigeminal nerve and the nerve axis was tilted infero-posteriorly. The patient obtained complete pain relief without sequelae after surgery by transposition of the superior cerebellar artery and correction of the tilted nerve axis. The nidus of the unresected AVM was obliterated by gamma knife radiosurgery.
