BACKGROUND Trigger-point injection (TPI) therapy is an effective modality for symptomatic treatment of myofascial pain. Serious adverse effects are rarely observed. In this report, we present the case of a 39-year-old man who experienced severe, transient hypokalemic paralysis in the context of TPI therapy with methylprednisolone, bupivacaine, and epinephrine. He was successfully treated with electrolyte replacement in a closely monitored setting. CASE REPORT A 39-year-old man with no past medical history except for chronic left hip pain from a work-related injury received a TPI with methylpre

Recent evidence suggests that active trigger points (TrPs) in neck and shoulder muscles contribute to tension-type headache. Active TrPs within the suboccipital, upper trapezius, sternocleidomastoid, temporalis, superior oblique and lateral rectus muscles have been associated with chronic and episodic tension-type headache forms. It seems that the pain profile of this headache may be provoked by referred pain from active TrPs in the posterior cervical, head and shoulder muscles. In fact, the presence of active TrPs has been related to a higher degree of sensitization in tension-type headache.

Adolescents who participate in athletics or have abnormal musculoskeletal anatomy have higher incidences of back pain than non-athletic peers with normal anatomy. Significant time and money spent in diagnostic evaluations for axial back pain can result in treatment delay causing a subsequent decrease in quality of life. Myofascial trigger points are a commonly overlooked reason for axial back pain. They develop due to an abnormal myoneural connection in the setting of muscle overuse. Trigger point injections are a technically simple intervention that is both diagnostic and therapeutic in allev

Cervicogenic somatic tinnitus is a subtype of subjective tinnitus and is defined as tinnitus in which forceful contractions of jaw and neck muscles modulate its psychoacoustic attributes. Various physical therapies have been proposed for the treatment of somatosensory tinnitus although there is no definitive cure for it. This report describes the use of acupuncture in the treatment of a 71-year-old woman with chronic neck pain who suffered from a left-sided tinnitus for 2 years as well. The tinnitus and neck pain severity was rated as 7 and 6, respectively, on a numeric rating scale of 10. On

BACKGROUND: Cervicogenic headache (CGH) is a secondary headache linked to cervical spine disorders and musculoskeletal dysfunction. Recent research suggests that the posterior myofascial chain, extending from the cervical region to the lower limbs, may contribute to CGH. Muscle tightness and trigger points (TrPs) in this chain can exacerbate headache symptoms. However, limited research on the prevalence of tightness and TrPs in the posterior kinetic chain highlights the need for further investigation to inform targeted treatments.
OBJECTIVE: The main aim of this study is to determine the prev

PURPOSE: Myofascial trigger points (MTPs) in the tibialis anterior muscle (TAM) are located in the upper third of the muscle and can be activated by various pathologies such as sprains, fractures, and muscle overload. Although the pathophysiology of MTPs is unclear, they often coincide with motor plates in the innervation zone. This study aimed to describe the innervation of the deep fibular nerve (DFN) into the TAM and relate it to clinically described MTPs.
METHODS: The TAMs of 12 cadavers were dissected from their origins to observe the exact point at which the DFN penetrates the muscle be

This study investigated the ultrasound and histological characteristics of myofascial trigger points (MTrPs) in the upper trapezius (UT) following dry needling (DN) therapy. Forty-nine participants with active MTrPs in one UT were included, with the contralateral healthy side serving as control. Ultrasound assessments (UT thickness, Young's modulus, vascular parameters including peak systolic velocity [PSV], resistance index [RI], and vascularity index [VI]) were performed: (1) pre-intervention, (2) 1-hour after DN. Pain levels were measured using the visual analog scale (VAS). Key findings sh

BACKGROUND: Myofascial trigger points (MTrPs) in the upper trapezius are prevalent in individuals with shoulder pain and may interfere with neuromuscular control and kinematics during shoulder movements. The purpose of this study was to compare shoulder muscle activation patterns and kinematic parameters, particularly scapulohumeral rhythm (SHR), between individuals with upper trapezius MTrPs and the healthy group during shoulder abduction.
METHODS: An observational study was conducted with participants assigned to either a trigger point group (n = 15) or a healthy group (n = 13). Surface ele

Myofascial trigger points (MTPs) are frequently identified in clinical practice, yet their underlying physiology remains conceptually fragmented across biochemical, mechanical, and neurophysiological domains. Commonly reported features, including spontaneous electrical activity, increased stiffness, nociceptive sensitization, hypoxia, and elevated inflammatory mediators, are well documented but are not consistently interpreted within established frameworks of tissue stress and repair biology. This paper proposes a systems-level integrative framework, the Protective Model of Myofascial Trigger

Joint hypermobility (JH) conditions suggest dysfunction in the autonomic nervous system (ANS) (dysautonomia), associated with multifactor non-articular local musculoskeletal pain, and remains a complex treatment. This study aims to determine the effects of musculoskeletal interfiber counterirritant stimulation (MICS) as an innovative treatment of myofascial trigger points (MTrPs) on the upper trapezius muscle in JH patients. We evaluate the ANS activity by wavelet transform spectral analysis of heart rate variability (HRV) in sixty women, equally divided: MTrP, MTrP + general joint hypermobili

Background: Chronic neck pain (NP) attributed to myofascial pain syndrome is one of the particularly common skeletal muscle disorder associated with the hyperirritable zone in the taut band of muscle. Trigger points (TPs) are the physical interpretation of the myofascial pain syndrome. In the United States, 30%-85% of pain patients have been affected by myofascial TPs. Objectives: To reveal preliminary evidence on the clinical efficacy of ischemic compression therapy, dry cupping, and their combination on improving the TPs' pressure pain threshold (PPT), neck range of motion (NROM), and neck d

The underlying etiology of tension type headache (TTH) is not understood. The current paper highlights the etiologic role of muscle trigger points (TrPs) to the development and maintenance of central sensitization in TTH and its clinical repercussion for proper management of these patients. Areas covered: A literature search on Pub Med for English-language published papers between 1990 and May 2017 to provide the most updated data on the topic was conducted. Current literature suggests that the referred pain elicited by active trigger points (TrPs) contributes to the manifestations of TTH. The

Existing treatment methods of zygomatic complex fractures, which are complicated by contrac- ture of the masseter as a result of displaced bone fragments, have to be improved. Lack of muscle relaxation leads to the formation of local hypertonicity. In spasmodic muscle fibers varies perfusion and hypoxia occurs, which is accompanied by the release of inflammatory mediators and activation of pain receptors. Over time, areas formed local hypertonicity specific trigger points that contain multiple sensory loci and include one or more sensitive nerve endings. A device for the effective electromyogr

BACKGROUND: Shoulder disorders are a common health problem in western societies. Several treatment protocols have been developed for the clinical management of persons with shoulder pain. However available evidence does not support any protocol as being superior over others. Systematic reviews provide some evidence that certain physical therapy interventions (i.e. supervised exercises and mobilisation) are effective in particular shoulder disorders (i.e. rotator cuff disorders, mixed shoulder disorders and adhesive capsulitis), but there is an ongoing need for high quality trials of physical t

Background/Objectives: Muscle tissues are a common source of symptoms related to low back pain (LBP), with myofascial trigger points (MTrPs) being a significant contributor. Since previous meta-analyses support interventions targeting MTrPs for reducing pain and improving functional disability in patients with LBP, this review aimed to synthesize current knowledge on the prevalence of MTrPs in LBP patients. Methods: To conduct this systematic review, data were collected from PubMed, Cochrane, and Web of Science. Published articles at any time up to February 2025 that comprised descriptive, obs

Trigger point dry needling (TrpDN) has emerged as a promising intervention for the treatment of myofascial pain. This involves insertion of a sterile filament needle directly into the trigger point (Trp), with the aim of eliciting a local twitch response (LTR). This sudden, involuntary contraction of the muscle fibers confirms the breakdown and success of TrpDN intervention. Not all LTRs are perceptible through visual observation or tactile sensation during the intervention. In search of noticeable LTR, clinicians perform Hong's technique (back-and-forth movements of needle in muscle). This re

BACKGROUND: Myofascial trigger points (TrPs) are hypersensitive points located in a tight band of muscle that, when palpated, produce not only local pain but also referred (distant) pain. The role of TrPs in patients with cervical dystonia (CD) has not been investigated.
OBJECTIVE: To identify the presence of TrPs in patients with isolated idiopathic CD and their association with pain.
METHODS: Thirty-one patients (74.2% women; age: 61.2 years, SD: 10.1 years) participated. TrPs were explored in the sternocleidomastoid, upper trapezius, splenius capitis, levator scapulae, anterior scalene, s
