PURPOSE: To evaluate atypical magnetic resonance (MR) imaging features of spinal tuberculosis.
MATERIALS AND METHODS: Between 1990 and 1993, five of 11 consecutive patients with spinal tuberculosis (two men and three women, aged 30-57 years) had MR findings more suggestive of neoplasm than infection. One patient, a European immigrant, had acquired immunodeficiency syndrome (AIDS).
RESULTS: Areas involved with tuberculosis were hypointense on T1-weighted images and hyperintense on T2-weighted images, and became enhanced with gadopentetate dimeglumine. In two patients, tuberculosis affected on

Disseminated tuberculosis is characterised by concurrent tubercular involvement of two or more non-contiguous organs. It is an unusual presentation of tuberculosis, especially in the absence of immunodeficiency. We describe a young, immunocompetent patient who presented with fever, cough, headache, diplopia and paraparesis. On examination, the patient had positive Kernig's sign, right third cranial nerve palsy and bilateral sixth cranial nerve palsy, bilateral lower limb weakness and crepititions on lung auscultation bilaterally. Chest radiogram revealed bilateral pulmonary tuberculosis. CT of

We present a patient with spinal intradural tuberculosis in the absence of both vertebral and meningeal tuberculosis. Diagnosis was made based on intra-operative findings and was confirmed by histopathology. Early surgical decompression along with a combination of steroid and antitubercular therapy resulted in a good outcome. At 26-month follow-up, the patient regained bladder control and was able to walk with support. Clinical features, magnetic resonance imaging, and intra-operative findings are described. Pathology and the relevant literature are discussed.

We reviewed 31 cases (19 males and 12 females) of spinal tuberculosis seen at the National Tuberculosis Centre from 1985 to 1989. The mean age was 35.4 years. The predominant clinical feature was backache (90.3%), while neurological features were found in 30.9%. An elevated erythrocyte sedimentation rate (in 80.0%) and a positive Mantoux test (in 70.9%) served as useful investigations. Spinal x-ray was abnormal in all cases, the lumbar spine being most commonly involved. Bacteriological or histopathological confirmation was obtained in only 29.0% of cases. The mainstay of treatment was anti-tu

Schistosomiasis is a chronic parasitic disease acquired through freshwater exposure, in which cercariae penetrate the skin and mature into adult worms. Disease manifestations largely result from granulomatous inflammation and fibrosis surrounding retained eggs. A 65-year-old widow presented with six months of lower back pain, weight loss, paraplegia (power 1/5), sphincter dysfunction, a T12 sensory level, and T9-T10 gibbus. She had received empirical antitubercular therapy for two months without improvement. Thoracolumbar MRI demonstrated vertebral and disc collapse at T10-T11 and L1-L2, with

BACKGROUND: Mycobacterium tuberculosis is a major cause of myelopathy and radiculopathy in settings with a high prevalence of tuberculosis/human immunodeficiency virus (HIV) coinfection. However, a paucity of publications exists on the spectrum of neurological and magnetic resonance (MR) imaging findings of spinal tuberculosis in these populations.
METHODS: We conducted a retrospective study of adults with spinal tuberculosis at a referral center in South Africa for patients with spinal disease without bony involvement seen at plain film radiography. We report the clinical, laboratory and spi

Craniovertebral junction tuberculosis is a rare lesion in which treatment remains controversial. Options range from conservative treatment to surgery, independently of any associated neurological threat. We here report the first case of pathologic odontoid fracture in a context of spinal tuberculosis, complicated by unusual neurological evolution. The patient presented with non-contiguous multifocal tuberculosis, of which there have previously been only 6 reported cases.

Tuberculosis remains one of the most important infectious disease worldwide and skeletal form has an incidence between 3% and 10%, depending on geographic area. The thoracolumbar junction is the most commonly involved in vertebral tuberculosis; some radiologic features are specifics, but other could be indistinguishable from that of lymphoma or metastases. We discuss the case of a 80-year-old man presented with a 2-month history of thoracolumbar pain, fatigability and fever. The vertebral radiography showed narrowing of the intervertebral disc space between T1 and T12, adjacent irregularity, e

OBJECTIVE: To evaluate the clinical efficacy of one-stage surgical management for spinal tuberculosis.
METHODS: Fifty-seven patients with thoracic, thoracolumbar and lumbar spinal tuberculosis were treated surgically by a single stage procedure from Jan 1996 to Jan 2001, including anterior resection, interbody outografting and anterior instrumentation in 35 cases, posterior debridement and posterior instrumentation in 16 cases, and posterior instrumentation and anterior resection plus interbody autografting in 6 cases. Titanium-alloy spinal implants were used in 38 cases and stainless steel i

Esophageal tuberculosis is rare and is usually due to secondary extension from contiguous structures. We report a patient who presented with dysphagia and was found to have esophageal stricture. Endoscopic biopsy was not suggestive of malignancy or tuberculosis. CT scan of the thorax revealed involvement of the fourth thoracic vertebra with paratracheal lymphadenopathy. The patient responded to anti-tubercular therapy.

Spinal tuberculosis constitutes 50% of all musculoskeletal tuberculosis. However, literature regarding congenital spinal tuberculosis is very scanty. Congenital spinal tuberculosis was diagnosed in a two-month-old child on the basis of age at presentation (gibbus since three weeks of age), hepatomegaly, raised ESR, radiological destruction of D10-D11 vertebrae, asymptomatic maternal endometrial tuberculosis and tuberculous histopathology from CT guided biopsy specimen from D10 vertebra. Both child and mother were treated by antitubercular treatment. Child improved symptomatically, gibbus becam

Multifocal tuberculosis in a child is rare event. Here we report a case of multifocal tuberculosis revealed by spinal tuberculosis in an eleven-year-old French Guianese girl. This observation underlines the difficulties, the consequences of delay, and the necessity of an early diagnosis of this disease for children.

Tuberculosis (TB) is a worldwide disease which seriously affects the global public health. Spinal TB is the most common extra-pulmonary TB and may cause vertebral bone destruction, collapse, kyphosis and even paralysis. Anti-TB chemotherapy is considered the cornerstone treatment of spinal TB and surgery is often required for patients with severe kyphosis, impaired neurological function or spinal instability. Debridement of TB lesions, bone grafting and internal fixation are the key procedures of spinal TB surgery. However, the selection of surgical approach, the extent of TB lesion debridemen

Spinal tuberculosis is the most common form of extrapulmonary tuberculosis. It is of great importance to neurologists because of the potentially devastating complication of paraplegia, which may set in during active disease or the healed phase. Due to the deep-seated nature of the disease, definitive diagnosis is often challenging. There is no clear consensus on the appropriate duration of therapy for spinal tuberculosis, with various guidelines recommending treatment from as short as 6 months to up to 18 months. In this article, we present a critical appraisal of the evidence on the same. In

China is a country with a high tuberculosis (TB) burden. Spinal TB is one reason for the resurgence of TB in China. The objective of this study was to investigate the clinical and epidemiological features of patients with spinal TB in a teaching hospital of Southwest China. The study group comprised 967 patients with spinal TB who were admitted to the authors' institution from 1999 to 2013. Demographic characteristics, prevalence of symptoms and clinical manifestations, prevalence of multidrug resistance and extensive drug resistance of spinal TB, management and outcome, and incidence of compl

OBJECTIVE: To study the value of digital tomosynthesis (DTS) applied in diagnosing spinal tuberculosis.
METHODS: Images of digital radiology (DR) and DTS were retrospectively analyzed in patients with spinal tuberculosis, and image quality and ratio of detection of lesions were compared.
RESULTS: Excellent ratio was higher for DTS images than DR images; ratios of detection of bone destruction, sequestration, and paraspinal abscess were higher for DTS than DR.
CONCLUSIONS: DTS had better image quality and ratios of detection of lesions and could be applied in diagnosing and following spinal
