INTRODUCTION: A fracture classification system should provide a reliable and reproducible means of communication between different parties. It should be logical and understandable, with few categories to memorize. The aim of this study was to determine the intra- and interobserver reliability of the Schatzker and Mayo classification systems for the assessment of proximal ulna fractures.
MATERIALS AND METHODS: Intra- and interobserver reliability studies were conducted on 39 X-rays of injured elbows drawn randomly from 74 cases previously used in a series on predictors of ulnohumeral osteoarth…

INTRODUCTION: Open reduction and internal fixation of crista supinatoris fractures is required when the elbow is unstable despite appropriate nonoperative management and when a patient is undergoing surgical treatment of a periarticular elbow fracture-dislocation.
STEP 1 SKIN INCISION AND SURGICAL APPROACH: Use a posterior or lateral skin incision according to your preference and then utilize the Kocher interval to access the joint, lateral collateral ligament, and crista supinatoris or, in the setting of a proximal ulnar fracture, use the Boyd interval.
STEP 2 MANAGEMENT OF ASSOCIATED INJUR

OBJECTIVE: To determine the biomechanical properties of plating options for distal ulna fractures.
METHODS: Fourth-generation ulna artificial bones were osteotomized and fixed with 4 different constructs: 2 locking compression plates (a straight 2.7-mm plate and a 2.4-mm T-plate) with both nonlocking and locking screws. The artificial bones underwent nondestructive tests to determine construct stiffness in flexion/extension and lateral bending. The final testing consisted of cyclical loading in axial torsion until implant failure.
RESULTS: The straight plate fixation construct was significan…

At the Campbell Clinic and City of Memphis Hospital from 1960 to 1970, 244 patients (216 with closed and twenty-eight with open fractures) had 330 acute diaphyseal fractures of the radius and ulna which were treated with ASIF compression plates and followed for from four months to nine years. One hundred and twelve patients had fractures of both bones of the forearm; fifty, single fractures of the ulna; and eighty-two, single fractures of the radius. In all, 193 fractures of the radius and 137 fractures of the ulna were treated by compression plating. Sixty-three patients (25.9 per cent) with

A 25-year-old female mandrill (Mandrillus sphinx - a primate and part of the Old World monkey group) was presented with a mildly comminuted, diaphyseal, radial fracture associated with a transverse ulnar fracture. Minimally invasive plate osteosynthesis techniques were used to achieve fixation of both the radial and the ulnar fractures. First, closed fracture reduction was achieved with a distraction frame consisting of a motorized circular external skeletal fixator. Next, dual percutaneous radio-ulnar plating was performed using a 2.7 limited-contact dynamic compression plate on the cranial a

Most patients with a nightstick fracture of the ulna are treated conservatively. Various techniques of immobilisation or early mobilisation have been studied. We performed a systematic review of all published randomised controlled trials and observational studies that have assessed the outcome of these fractures following above- or below-elbow immobilisation, bracing and early mobilisation. We searched multiple electronic databases, related bibliographies and other studies. We included 27 studies comprising 1629 fractures in the final analysis. The data relating to the time to radiological uni

Elbow dislocation with ipsilateral diaphyseal fracture of both radius and ulna is a rare injury. However, elbow dislocation with either radius or ulnar diaphyseal fracture is commonly reported. We report an uncommon injury of a 16 years old male who had his elbow dislocation with ipsilateral both bones forearm fractures. Elbow dislocation was reduced and fracture both bones were fixed with dynamic compression plate under brachial block.

Isolated volar dislocation of the distal ulna without forearm fracture is very rare; however, this injury is incorrectly diagnosed in approximately 50% of cases. This injury can lead to a significant functional disability if left untreated. This report presents a case of isolated volar dislocation of the distal ulna with an ulnar styloid fracture. The dislocation was subsequently reduced, the styloid was surgically repaired, and the patient was satisfied at the last follow-up. The importance of a proper clinical examination and an accurate radiographic position of the wrist are stressed. Furth

Complex fractures of the proximal ulna with associated elbow instability represent challenging disorders with a high incidence of complications, including malunion, arthrosis, stiffness, and recurrent instability. If a significant coronoid fragment is present, there is an increased risk of a poor outcome. Proper management of this coronoid fragment can be critical to a successful outcome. Information on identifying, reducing, and fixing the coronoid fragment in complex proximal ulnar fractures will help the treating orthopaedic surgeon maximize results with this difficult injury pattern.

We have reported three cases of fatigue fracture of the ulna in male pitchers of fast-pitch softball. To elucidate the etiology of injury, we first selected three healthy male and three healthy female pitchers from a well-trained college team and analyzed their forearm movement by high-speed cinematography. This showed slight flexion of the elbow joints during wind-up motion, dorsal flexion of the hand joints upon releasing the ball, and extreme pronation of the forearms during the follow-through. We then took 8 mm CT scanning sections of the forearms. Using these images, we investigated shape

An exceptional case of posterior dislocation of the ulna at the elbow, associated with fracture of the radial shaft, is reported. Diagnosis was established with anteroposterior and lateral radiographs of the forearm, elbow and wrist. Treatment consisted of closed reduction of the ulnar dislocation and open reduction and internal fixation of the radius with an AO plate. A long arm cast was applied for 3 weeks with the elbow held at 90 degrees. Six months later full function of the limb had been achieved.

We reviewed the cases of fifteen children who were treated for a Monteggia injury. Transverse and metaphyseal fractures of the ulna were stable after closed reduction, but oblique fractures of the shaft redisplaced and required intramedullary pinning. Five patients who were seen late had open reduction and internal fixation of the fracture. Four of these children had reconstruction or repair of the annular ligament, with excellent results. But in the child in whom the ligament was not repaired the radius subluxated.

Fourteen cases of fracture of the shaft of ulna and radius with associated dislocation of the radial head in adults are presented. The dislocation was initially missed in 42 per cent of cases. Several closed and open therapeutic procedures were performed either for the dislocation or the shaft fractures. In these patients, with follow-up averaging 5.9 years, the incidence of complications (redislocation, mal-union, pseudarthrosis, radio-ulnar synostosis, peri-articular ossification) was noteworthy. Final evaluation according to the range of motion of the forearm, showed a high incidence of uns

BACKGROUND: Distal ulnar fractures can be challenging to manage, and noncompressive intramedullary screws may be a potential solution to this problem. The purpose of this study was to review our institutional experience with retrograde screw fixation of distal ulna fractures compared with conventional plating methods.
METHODS: A retrospective cohort study was designed to compare adult patients who had undergone retrograde intramedullary screw fixation for a distal third ulna fracture in a single health system between June 2021 and February 2024 with controls who underwent conventional plate a…

BACKGROUND: Concomitant distal radius and distal ulna metaphysis or head fractures (DRUF) are uncommon and acceptable results have been reported from cast immobilisation and internal fixation.
METHODS: We reviewed the charts of 1094 patients treated for distal radius fracture at our institution in a two year period from 2009 to 2010. 24 patients with concomitant DRUF with were treated by cast immobilisation (group 1, n = 11), internal fixation of both bones (group 2, n = 7), internal fixation of radius alone (group 3, n = 2), and internal fixation of radius with distal ulna resection (group 4

PURPOSE: To evaluate the clinical and radiological outcomes of distal radius fractures treated by percutaneous fixation using distal radius-ulna pinning and to assess its effectiveness especially for preventing fracture settling.
METHODS: We retrospectively reviewed 18 distal radius fractures (15 AO type A2 and 3 AO type C1). Range of motion and Disabilities of the Arm, Shoulder, and Hand scores were evaluated. We measured radiographic parameters at the final follow-up and compared them with those on immediate postoperative x-rays.
RESULTS: All fractures united and average time to initial he

Background Isolated distal ulna fractures are rare injuries and are commonly associated with distal radius fractures. Though most of them can be managed conservatively, few of them require open reduction due to the interposition of various structures. Case Description In this case report, we report two cases of irreducible isolated distal ulna fractures in adolescents due to the interposition of the extensor retinaculum with its underlying tendons requiring open reduction to achieve bony union and distal radioulnar joint stability. These distal ulna fractures can be best reduced by open redu…

Isolated distal ulna epiphyseal plate injuries are very rare and are often associated with early epiphyseal plate arrest. A 13-year-old boy sustained an isolated minimally displaced Salter-Harris type II fracture of the left distal ulna following a fall from a bicycle. The fracture was reduced, and a long arm plaster cast was applied for four weeks. At the six-month follow-up, the patient presented with a painless, full range of movement of the left wrist, but on radiological examination, a mild shortening of the ulna was detected. We plan to regularly evaluate this patient until distal epiphy

A 10-year-old girl presented to the emergency department having sustained a fall onto an outstretched left hand while playing soccer. Clinical and radiographical assessment identified a Salter-Harris I distal ulna fracture, as well as a buckle fracture of the distal radius. The injury was closed, and she had no neurovascular deficits on examination. She was brought to the operating theatre the following morning for closed reduction under general anaesthesia. Image intensification was used to confirm anatomical reduction, and an above-elbow moulded plaster-of-paris cast was applied. Follow-up c
