[Management of urinary incontinence after orthotopic urinary diversion].

Orthotopic urinary diversion (OUD) is performed in almost half of all radical cystectomies. This review presents an overview of the incidence, pathophysiology, and management of urinary incontinence (UI) after OUD. Daytime and nighttime UI are reported in up to 15% and 45% of cases after OUD, respectively. UI after OUD is more frequent in women. Stress incontinence is the most common reason for daytime urinary leakage, while an absent vesicourethral reflex with reduced external sphincter muscle tone is associated with nighttime UI. Conservative management has limited therapeutic value in UI af…

Prevalence of faecal incontinence among women with urinary incontinence.

Four hundred and sixty-five women attending a urodynamic clinic were interviewed, and completed a detailed bowel questionnaire, about their urinary and bowel symptoms. All the women underwent video-cystourethrography with pressure and flow studies. The reported incidence of faecal incontinence was 15.3% (n = 71) on direct questioning and 26% (n = 121) on the postal questionnaire. Faecal incontinence was more common in women with a urodynamic diagnosis of detrusor instability (30% (n = 26)) than among women diagnosed as having genuine stress incontinence (21% (n = 38)). Denervation and myogenic

[Urogynecology II: urinary incontinence in men and women: surgical treatment of urinary incontinence and prolapse].

Numerous surgical procedures are available for the treatment of stress urinary incontinence in women and men. On a par with classical therapy options (e.g. colposuspension and artificial sphincter prosthesis) suburethral tape procedures have become established as the minimally invasive standard of care. Regarding comorbidities and recurrent urinary incontinence, therapeutic procedures should be modified on an individual basis. It is crucial to involve patients in therapeutic decision-making and counseling should be given with respect to all conservative and operative alternatives.

Classification of female urinary incontinence by the scored incontinence questionnaire.

OBJECTIVES: Our purpose was to investigate whether urinary incontinence (UI) severity could be graded by UI scores. METHODS: The study included 168 UI patients diagnosed by conventional procedures as stress incontinence (SI, n=108) or urge incontinence (URI, n=60). A questionnaire containing 15 items scored for stress-score (s-s) and urge-score (u-s) was administered to patients to assess the correlation between pad test values (PTVs) and s-s in SI and u-s in URI. RESULTS: Significant correlations were found between log[PTVs] and both s-s and u-s: log[PTV(SI)]=0.051s-s-0.207 (r=0.830, P<0.00…

Urodynamics of female urinary incontinence with emphasis on stress incontinence.

More than 500 female patients with urinary incontinence were studied in our urodynamic laboratory by simultaneous recordings of urethral pressure at 2 levels, bladder pressure, intra-abdominal pressure, and by uroflowmetry and cine-fluoroscopy. Urethral pressure profile under basal conditions was compared to that in patients with a full bladder, under the stress of sharp or sustained increase in intra-abdominal pressure, voluntary perineal muscle activity and patient's position. From these studies stress and urge incontinence, and neurogenic varieties (or a combination of several types) could …

Ultrasound in Female Urinary Incontinence.

Urinary incontinence (UI) is a common health condition that may interfere with the quality of life. A comprehensive evaluation of female UI helps with effective and safe treatments. Ultrasound has gained popularity to explore UI recently because it can collect crucial information for treatment planning and counseling. Translabial and introital approaches are commonly and reliably applied to ultrasound. The images can be obtained using two-dimensional and three-dimensional ultrasounds. Ultrasound is the only modality capable of confirming the presence or absence of a mid-urethral sling (MUS) an…

Pharmacological treatment of urinary incontinence.

We present an overview of the current pharmacological treatment of urinary incontinence (UI) in women, according to the latest evidence available. After a brief description of the lower urinary tract receptors and mediators (detrusor, bladder neck, and urethra), the potential sites of pharmacological manipulation in the treatment of UI are discussed. Each class of drug used to treat UI has been evaluated, taking into account published rate of effectiveness, different doses, and way of administration. The prevalence of the most common adverse effects and overall compliance had also been pointed…
