Uterine incarceration is a rare complication of pregnancy, usually presenting between 12 and 20 weeks' gestation, when the gravid uterus normally ascends through the pelvic brim. Conditions such as pelvic inflammatory disease, endometriosis, postoperative adhesions, posterior wall leiomyomas, and pelvic contraction may lead to fixed retroflexion of the uterus, predisposing to this condition. A case of incarceration is presented. Clinical features, diagnosis, treatment, and potential complications are discussed.

The clinical and pathological features of four cases of the rare entity diffuse leiomyomatosis of the uterus are described. The patients, who ranged in age from 28 to 35 years, presented with a six month to ten year history of abnormal uterine bleeding, dysmenorrhoea and an enlarged uterus. Hysterectomy in each case revealed a symmetrically enlarged uterus containing numerous small ill-defined leiomyomatous nodules. In a 60-year-old woman because of postmenopausal haemorrhage diagnostic curettage was performed. The histopathologic finding was the adenocarcinoma of the endometrium. The clinical stage was Ib, grade 2 or respectively presumptive stage IV with regard to ultrasound and radiographic findings. Rupture of the uterus is an uncommon obstetric emergency that usually occurs after 36 weeks gestation in a woman with a previous cesarean section. Complete rupture of the uterus with extrusion of the fetus into the peritoneal cavity is associated with high fetal mortality and with hypovolemic shock in the mother. Extraovarian Brenner tumors are extremely rare. Three paratesticular lesions have been observed in men. The only extraovarian Brenner tumor in a woman previously reported was located in the broad ligament. This paper describes a small tumor with histologic features of Brenner tumor within the wall of the uterus. The possible histogenesis is briefly discussed.

Acute puerperal inversion of the uterus is an uncommon but potentially fatal obstetric complication. In the 15 year period ending December 31, 1974, 11 inversions have been managed at the University of Virginia Hospital. All were recognized immediately and manually replaced and there was no significant postpartum morbidity. Successful management appears to depend largely upon prompt recognition, which can be achieved by routine postpartum examination of the vagina and cervix and manual exploration of the uterus.

A case is reported of a 180 degrees levorotation of the gravid uterus with successful outcome for mother and child. Delivery was by cesarean section.Only 108 cases of rotation of the gravid uterus have been reported in the world's literature. A uterine tumour was associated in almost one third of cases. The condition usually presents as an acute abdomen. Complications include uterine rupture and pulmonary embolism. Treatment is by laparotomy and de-torsion, with cesarean section if at term or near term.
