Testicular function in varicocele.

Testicular testosterone concentration serum testosterone, LH and FSH, sperm count and testicular histology were evaluated in 17 patients with varicocele. Testicular testosterone was either normal or high (mean 906 +/- 723 ng/g of tissue), and serum testosterone was within the normal range in most patients. Serum LH was elevated in half of the patients. The degree of testicular damage observed was extremely variable and correlated with sperm analysis. Testicular testosterone tended to be higher in patients with severe microscopic lesions of the testis. It is concluded that even though Leydig ce…

Effect of varicoceles on spermatogenesis.

Varicoceles are dilated veins within the spermatic cord and a relatively common occurrence in men. Fortunately, the large majority of men are asymptomatic, however, a proportion of men with varicoceles can suffer from infertility and testosterone deficiency. Sperm and testosterone are produced within the testis, and any alteration to the testicular environment can negatively affect the cells responsible for these processes. The negative impact of varicoceles on testicular function occurs mainly due to increased oxidative stress within the testicular parenchyma which is thought to be caused by …

The varicocele as related to fertility.

Brown JS, Dubin L, Hotchkiss RS. The varicocele as related to fertility. Fertil Steril 1967;18:46-56. "The fact that some men with varicoceles are fertile should not minimize the potentially detrimental role of this abnormality. It would be reasonable to claim that varicocele is uniformly detrimental. One might postulate that a fertile man with a varicocele would have better semen without this condition."

Intratesticular varicoceles: are they significant?

OBJECTIVE: Varicoceles occur in 15% of adult and adolescent males and are generally considered to be an extratesticular phenomenon. However, an intratesticular component has been reported in up to 2% of adult and 2% of adolescent varicoceles. We sought to determine the incidence of intratesticular varicoceles (ITV) in adolescents in our practice, its significance, associated findings and response to treatment. MATERIALS AND METHODS: We retrospectively reviewed 684 adolescent males who were diagnosed with varicoceles and had at least one Doppler ultrasound (DUS) prior to any surgery to identif…

Varicoceles: the diagnostic dilemma.

Clinical examination of the scrotum remains the most commonly used technique to diagnose varicoceles. However, scrotal anatomy (eg, thick scrotum, scarring, hydrocele) in some men may make physical examination alone unreliable. In these situations, imaging (eg, ultrasound, Doppler imaging, venography) may be used to diagnose a varicocele. The dilemma is that there are no widely accepted or used criteria to diagnose a varicocele based on imaging. This paper reviews the different imaging techniques used and the accuracy of each in diagnosing a varicocele.

[The treatment of varicocele in adolescence].

Varicocele treatment in adolescents should be considered as a prophilactic procedure. It is known, in-fact, that the venous reflux is responsible for damage to the testis that worsen by time. For this reason patients with grade II or III varicocele and reduced size of the testis must be considered for treatment. A right approach to varicocele should consider the different etiopathogenetic factors. Type I varicocele can be easily treated with embolization of the spermatic vein during the diagnostic phlebographic procedure. For type II and III in which is present an iliac hypertension must be co…

Ultrasonographic diagnosis of varicoceles.

OBJECTIVE: To assess the ability of color duplex scrotal ultrasonography to detect subclinical varicoceles and confirm the diagnosis of clinical varicoceles. DESIGN: Physical examination, color duplex scrotal ultrasonography and internal spermatic venography was performed on 64 testicular units in 33 men. SETTING: Male fertility center. PATIENTS: Two hundred sixty-two consecutive men being evaluated for male factor infertility of whom 33 agreed to undergo venography. MAIN OUTCOME MEASURES: Ultrasonographic measurement of scrotal vein diameter of patients in the supine and upright position,…

Current management of varicoceles.

In spite of all that has been written about the varicocele, it is still not clear who will benefit from occlusion of the spermatic vein or why improvement in semen takes place in some patients but not in others. Because fertility is usually assessed in the framework of a couple who are trying to establish a pregnancy, it is important to evaluate both partners before making any recommendations regarding therapy. If the man is found to have a varicocele and semen analyses that indicate deficits in either the quantity or quality of the sperm, surgical ligation or transvenous occlusion should be c…

[Varicocele in the child and adolescent].

Based on a series of 31 cases of children and adolescents with varicocele, the authors stress the high incidence of left testicular atrophy (71%). Other authors have emphasised the almost constant presence of histological lesions which are generally bilateral. These findings suggest that the harmful action of varicocele on spermatogenesis is more marked in children and adolescents than in adults, encouraging early surgical treatments which is almost always effective.

Asymptomatic varicocele in an infant.

Varicoceles are a common cause of male infertility. The incidence of varicoceles at puberty is approximately 16 percent, but in prepubertal males, they are rare. We present a case of a varicocele in a 1 1/2-year-old male and discuss whether early recognition and surgical correction may preserve male fertility potential.
