Surgical outcomes of vascular reconstruction in soft tissue sarcomas of the lower extremities.

BACKGROUND: Limb-sparing procedures are currently considered the standard treatment for lower limb soft tissue sarcoma (STS). Surgical excision combined with vascular resection may be necessary to provide an adequate safety margin and to improve the oncologic outcomes. In this scenario, vascular reconstruction is required to preserve limb function. We evaluated the long-term patency and survival outcomes of arterial and venous reconstruction after resecting lower limb STS in the largest single-center case series to date. METHODS: Between November 1995 and July 2014, 25 patients with lower lim

The brachiobasilic arteriovenous fistula: effect of patient variables.

PURPOSE: The hemodialysis population is constantly expanding as patients on dialysis have increased longevity and the number of kidneys available for transplantation remains static (1). After radiocephalic and brachiocephalic fistulas have been exhausted the use of the autologous brachiobasilic fistula (BBAVF) should be considered prior to use of a synthetic graft. We present our single center experience of 140 brachiobasilic fistulas in a five-year period and examine any factors that influence patency and long-term function. METHODS: Patients who had undergone formation of a BBAVF between Ja

Regional vs. Local Anesthesia for Arteriovenous Fistula Creation in Adults With End-Stage Kidney Disease: A Grade Assessed Systematic Review and Meta-Analysis.

Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis in patients with end-stage renal disease (ESRD), yet high rates of primary failure and suboptimal patency remain significant challenges. The choice of anesthetic technique may influence vascular dynamics and AVF outcomes. This study aims to compare the effectiveness of regional anesthesia (RA) versus local anesthesia (LA) on clinical outcomes following AVF creation in adults with ESRD. A systematic review and meta-analysis was conducted in accordance with PRISMA 2020 guidelines. PubMed, Cochrane Library, Scopus, and

Long-term outcomes of extra-anatomical venous bypass for central venous occlusion in haemodialysis patients: A single-centre experience.

OBJECTIVE: Central venous occlusion is a recognised complication of haemodialysis vascular access that can render ipsilateral arteriovenous fistulas dysfunctional. When endovascular options have been exhausted, extra-anatomical vein-to-vein bypass offers a surgical strategy for access preservation. This study evaluated long-term patency outcomes, bypass thrombosis natural history and baseline predictors of primary patency loss. METHODS: A retrospective review was performed of all patients undergoing extra-anatomical venous bypass to the external iliac vein (EIV) for central venous occlusion a

Arteriovenous fistulas are associated with superior outcomes in very elderly hemodialysis patients: a nationwide cohort study.

BACKGROUND/AIMS: The optimal vascular access strategy for very elderly patients initiating hemodialysis (HD) remains unclear. Arteriovenous fistulas (AVFs) offer long-term benefits but may be limited due to vascular aging. This study evaluated vascular access outcomes in patients aged ≥ 80 years. METHODS: We conducted a retrospective cohort study using data from the Korean National Health Insurance Service between 2008 and 2019. Patients aged ≥ 80 years who initiated HD with a newly created AVF or arteriovenous graft (AVG) were included. Primary outcomes were primary, assisted primary, and se

Efficiency and safety of microvascular anastomotic coupler for wrist revascularization in traumatic injuries.

INTRODUCTION: Effective vascular anastomosis is crucial for successful hand surgery, particularly in traumatic injuries where rapid restoration of blood flow is essential. The Synovis microvascular anastomotic coupler system presents a novel approach by potentially offering faster and more reliable outcomes than traditional suturing methods. This study was conducted to assess the effectiveness and safety of the Synovis coupler for microvascular anastomoses in the wrist, evaluating anastomosis time, patency rates, and complication rates. METHODS: This retrospective study examined 25 microvascu

Induction of neointima formation by platelet derived angiogenesis fraction in a small diameter, wide pore, PTFE graft.

Enhancement of endothelialization and patency of a small diameter (2 mm), wide pore, PTFE graft was attempted by coating the luminal surface with a platelet derived angiogenesis fraction (PDAF) and implanting it in a rat model. PDAF was delivered to the grafts by combining it with a carrier polymer. PDAF-treated grafts were initially implanted in the retroperitoneum for 21 days followed by removal of one for histology and in situ end to side bypass to the infrarenal aorta for the other. Vascularized grafts were examined at 14 days for patency and 100 days for patency and histology. Significant

Brachiobasilic fistula formation - single versus two stage procedure.

BACKGROUND: Brachial basilic (BB) fistulae are a form of vascular access for patients requiring dialysis. They are indicated when the cephalic vein is unsuitable for use. This fistula can be created with either a single stage or a two stage procedure. We aimed to compare the two techniques. METHODS: 73 BB fistulae (29 single and 44 two-stage) were created over a 5-year period (2003-2008). Data including sex, age, dialysis and diabetic status was collected from the case notes. Patency and time to maturity data was collected prospectively on an electronic database within the dialysis unit. RES

An update of the effect of far infrared therapy on arteriovenous access in end-stage renal disease patients.

The life qualities of end-stage renal disease (ESRD) patients rely largely on adequate dialysis, and a well-functioning vascular access is indispensable for high quality hemodialysis. Despite the advancement of surgical skills and the optimal maintenance of arteriovenous fistula (AVF), malfunction of AVF is still frequently encountered and has great impact on the life of ESRD patients. Several medical, mechanical and genetic prognostic factors are documented to affect the patency of AVF and arteriovenous graft (AVG). Heme oxygenase-1 (HO-1) is one of the genetic factors reported to play a role

Surgical infrainguinal revascularization for peripheral arterial disease: factors affecting patency rate.

BACKGROUND: Peripheral arterial disease is a source of morbidity and mortality. Surgical vascular reconstruction is a treatment option but probability of failure and complications are important concerns. In this study, we evaluated outcome of surgical infrainguinal reconstruction and factors affecting graft patency for a period of one year. METHODS: In this cohort study, 85 consecutive patients with chronic ischemia who underwent lower extremity surgical vascular reconstruction (including 52 femoropopliteal and 25 femorofemoral bypass) from March 2007 to Feb 2009 were recruited. Graft patency

Fully no-touch technique without inflating the cephalic vein increase the patency rate of arteriovenous fistulas.

INTRODUCTION: One of the most important problems for patients undergoing hemodialysis due to chronic renal failure is the need for a long-lasting vascular access. The patency of vascular access is affected by many factors. Factors related to surgery are also among these factors. No-touch techniques theoretically and practically prevent endothelial damage due to minimal contact on vascular structures. It is a well-known fact that surgeries performed with these methods also increase long-term survival. METHODS: Patients who underwent distal radiocephalic arteriovenous fistula (AVF) surgery for

New Absorbable Microvascular Anastomotic Devices Representing a Modified Sleeve Technique: Evaluation of Two Types of Source Material and Design.

The purpose of this study was to create a new absorbable vascular anastomotic coupler and evaluate the patency and degradation degree. Vascular anastomosis was performed in the jugular vein in 31 New Zealand white female rabbits. The coupler consisted of an inner and outer ring. One side of the jugular vein was passed through and overlapped the inner ring. The opposite side of the jugular vein overlapped the everted jugular vein on the inner ring. Then, the outer ring engaged with the inner ring and completed the anastomosis. The outer rings were also constructed with two shapes including an O

Brachial Vein Transposition Versus Arteriovenous Graft - Two-year Results.

BACKGROUND: Proper vascular access is essential for effective hemodialysis. There are three main access modalities: arte- riovenous fistula (AVF), arteriovenous graft (AVG), and central venous catheter. AVF has better patency and fewer complications, with lower morbidity and mortality rates. Some patients have limited superficial venous patrimony, and the best vascular access remains undetermined, with AVG and brachial vein transposition (BVT) representing upper limb alternatives. Our aim is to inves- tigate BVT and AVG followed by our institution regarding patency and need for intervention.

Comparative evaluation of prosthetic, reversed, and in situ vein bypass grafts in distal popliteal and tibial-peroneal revascularization. Veterans Administration Cooperative Study Group 141.

In 1983, the Veterans Administration initiated a cooperative study in 18 medical centers to perform a prospective comparative evaluation of vascular grafts. Five hundred ninety-six patients have had femoropopliteal below-knee or femorotibial-peroneal (polytetrafluoroethylene, umbilical vein, in situ, or reversed saphenous vein) bypasses. Patency evaluation at two years revealed that for femoropopliteal below-knee reconstruction, the vein patency (76%) was better than prosthetic patency (64%). For femorotibial-peroneal reconstructions, the two-year patency for vein bypasses (73%) was significan

Application of intraoperative infrared thermography in bypass surgery for adult moyamoya syndrome: A preliminary study.

BACKGROUND AND OBJECTIVES: Cerebral revascularization surgery is the mainstay of treatment for moyamoya syndrome (MMS) today, and intraoperative determination of the patency of the revascularized vessel is a critical factor in the success of the procedure. Currently, major imaging modalities include intraoperative indocyanine green (ICG) videoangiography (ICG-VA), digital subtraction angiography (DSA), and vascular ultrasound Doppler. Infrared thermography is a modern imaging modality with non-contact devices for the acquisition and analysis of thermal data. We aimed to investigate the feasibi

Outcomes of arteriovenous fistulae compared with heparin-bonded and conventional grafts for hemodialysis access.

PURPOSE: Arteriovenous fistulae (AVF) are the preferred vascular access for hemodialysis patients. However, patients who do not have suitable veins require prosthetic arteriovenous graft (AVG) placement. We analyzed the patency and complication rates of upper extremity brachiocephalic AVF compared to brachioaxillary tapered heparin-bonded AVG and conventional AVG. METHODS: We performed a retrospective analysis of patients who underwent a permanent vascular access procedure at our tertiary referral center from 2006 to 2008. The First Microsurgery-assisting Robot Equipped With an Automatic Instrument Exchange System and a Miniature Tool Kit.

Microsurgery requires high precision, stability, and effective teamwork. However, current robotic-assisted surgery faces challenges such as inefficient instrument exchange and dependence on surgical assistants, contributing to prolonged operative times and increased costs. To address these issues and enhance surgical efficiency and surgeon autonomy, we developed a microsurgery-assisting robot equipped with an automated instrument exchange system and a specialized micro tool kit. The robot was tested using chicken-wing arteries and rat vessels with diameters ranging from 0.35 to 0.8 mm. Evaluat
