Forty-four patients, with a mean age at surgery of 10 years, were followed for 5 to 16 years (mean 9.7 years) after relief of left ventricular outflow tract obstruction. There were no early deaths, but 5 late deaths occurred, 3 following reoperation. Twenty-five patients were recatheterized from 1 to 16 years later (mean 6.6 years). In 21 of 32 patients (66 per cent), a new diastolic murmur followed relief of valvular stenosis; 25 (78 per cent) of these patients had a postoperative diastolic murmur. Seventeen of these 25 (68 per cent) were recatheterized, and 11 of the 17 (65 per cent) had mod

A 56-year-old woman with presumed hypertrophic cardiomyopathy was diagnosed with Fabry disease (GLA: p.Glu358del) and a concomitant PLN variant. Imaging revealed advanced Fabry cardiomyopathy with dynamic LVOT obstruction. Despite optimized medical and enzyme replacement therapy, symptoms progressed (NYHA-class III) with a marked increase in LVOT gradient. Mavacamten was initiated and uptitrated under echocardiographic monitoring. At three months, LVOT gradient significantly decreased and functional status improved (NYHA-class I), while systolic function remained preserved. This case highlight

Dynamic left ventricular outflow tract obstruction (LVOTO) is an underrecognized, load dependent cause of hemodynamic instability in critically ill patients. Although classically associated with hypertrophic cardiomyopathy, it frequently occurs in the ICU in settings such as septic shock, hypovolemia, postoperative states, and right ventricular failure. Standard shock therapies, including inotropes and afterload reduction, may paradoxically worsen obstruction and lead to escalating hemodynamic instability. Bedside echocardiography is central to diagnosis, typically revealing a small hyperdynam

BACKGROUND: Left ventricular outflow tract (LVOT) obstruction is a rare but fatal complication after transcatheter aortic valve implantation (TAVI). Early detection and timely intervention are essential for favorable clinical outcomes.
CASE SUMMARY: We report a case of LVOT obstruction diagnosed and dynamically monitored via a SavvyWire (Haemonetics). A 78-year-old woman underwent transfemoral TAVI for severe valve stenosis, and continuous hemodynamic monitoring by the SavvyWire identified a significant aorto-left ventricular pressure gradient with systemic hypotension immediately after valve

A subaortic membrane is an uncommon cause for left ventricular outflow tract obstruction. Hypertrophic cardiomyopathy with dynamic left ventricular outflow tract obstruction would mask the presence of the subaortic membrane on transthoracic echocardiography and cause a false diagnosis. We report a patient with subaortic stenosis due to flail subaortic membrane misdiagnosed as obstructive hypertrophic cardiomyopathy on transthoracic echocardiography, identified on transesophageal echocardiography and cardiac catheterization.

Accessory mitral valve is a rare congenital abnormality and an unusual cause of subvalvular obstruction of the left ventricular outflow tract. Accessory mitral valves are usually detected in children due to symptomatic obstruction; isolated nonobstructive accessory mitral valve is rarely seen in adults. We describe the echocardiographic diagnosis of accessory mitral valve as an isolated congenital anomaly not associated with a substantial degree of obstruction of the left ventricular outflow tract in an asymptomatic adult patient. This case highlights the importance of transthoracic and transe

The surgical relief of complex multilevel left ventricular outflow tract (LVOT) obstruction remains a challenging surgical problem. We present a new operation which combines the concepts of aortoventriculoplasty, extended aortic root replacement and the use of a pulmonary autograft. Sixteen patients underwent this operation: 11 patients after previous attempts to relieve diffuse subvalvular stenosis and five patients who presented excessive gradients over an outgrown aortic valve prosthesis. All patients, except one survived the operation. One patient developed massive right ventricular infarc

Between March 1986 and April 1990, 22 consecutive fetuses (at gestational ages of 21 to 38 weeks) with a suspected diagnosis of critical (ductus-dependent) left ventricular outflow tract obstruction on fetal echocardiogram were referred to our center for delivery and surgical treatment. Diagnoses were hypoplastic left heart syndrome (n = 16), valvular aortic stenosis (n = 2), common atrioventricular canal with subaortic stenosis (n = 3), and single ventricle with subaortic stenosis (n = 1). Postnatal echocardiography revealed that fetal echocardiography was correct in predicting left ventricul

Recurrent tunnel stenosis of the left ventricular outflow tract following operation for subaortic stenosis and hypoplastic aortic annulus remain a challenge for pediatric cardiac surgeons. We have recently applied a new technique of extended aortic root replacement using an aortic allograft to treat three patients who had previously been operated upon for subaortic stenosis and three who had aortic stenosis with a hypoplastic aortic annulus. This new procedure combines the concept of aortoventriculoplasty with allograft aortic root replacement and coronary artery reimplantation. The valved aor

Combined forms of left ventricular outflow obstruction are being recognized with increasing frequency. A patient who had coarctation of the aorta, valvular aortic stenosis, and fixed and dynamic subvalvular obstruction is described. The importance of recognizing multiple levels of obstruction in such patients is emphasized.

Arteriovenous fistulas (AVFs) are the preferred vascular access for hemodialysis due to their durability and lower risk of complications than alternative access methods. However, AVFs can significantly impact cardiac function, particularly in patients with preexisting cardiovascular conditions. We present a case of a 56-year-old female with a history of hypertension and end-stage renal disease who developed recurrent hypotension, dizziness, and dyspnea following AVF reopening. The echocardiographic evaluation revealed severe left ventricular hypertrophy, systolic anterior motion of the mitral

AIMS: Many factors cause left ventricular outflow tract obstruction (LVOTO) in hypertrophic cardiomyopathy (HCM). Previous studies reported that left ventricular basal muscle bundle (BMB) may be associated with LVOTO. We aimed to evaluate the role of BMB in LVOTO by echocardiography.
METHODS AND RESULTS: Two hundred fifty-six patients diagnosed with HCM were recruited. The morphologic characteristics of left ventricular outflow tract (LVOT) were analysed. BMB was detected in 178 (69.5%) patients by echocardiography. Patients were separated by a resting or provocative LVOT gradient ≥30 mmHg or

Operative relief of congenital tunnel subaortic stenosis by means of local incision or excision, or both, has generally been unsatisfactory. The use of a valve-bearing conduit between the left ventricular apex and thoracic aorta offers a predictable means of bypassing the left ventricular outflow obstruction. The procedure was used in a 17 year old girl with an excellent hemodynamic result. The history of operative management with diverting plantation of valved conduits in this position have not been defined, but use of these prostheses appears advisable in severe subvalvular, valvular and sup

A woman in her 70 s presented with fatigue and dyspnoea during exertion. Six years ago, she had been implanted with a dual chamber pacemaker with the lead placement at the high septal site of the right ventricle. Echocardiography demonstrated the presence of the previously detected left ventricular septal bulge along with a newly observed systolic turbulent mosaic at left ventricular outflow tract (LVOT) and a systolic anterior motion (SAM) of the mitral valve. Electrophysiological pacing study and simultaneously performed echocardiography demonstrated that the apical pacing decreases the pres

BACKGROUND: Pregnancy exacerbates left ventricular outflow tract (LVOT) gradients due to increased cardiac output (CO). This is amplified in patients with LVOT obstruction (LVOTO). Ascertaining whether gestational elevations in LVOT gradients are transient haemodynamic responses that regress postpartum, or predictors of disease progression or adverse outcomes, is critical.
METHODS: A retrospective, observational study was conducted between 2009 and 2023 in two tertiary centres. Clinical and echocardiographic data were collected preconception, antenatally, and postpartum. Key measurements incl
