Future directions. Augmentation of osteoporotic vertebral bodies.

Because current medical and surgical treatments of vertebral body fractures are less than adequate, there is a need for interventions that decrease the likelihood of occurrence of these fractures and improve the treatment options once they have occurred. One such broad category of intervention involves the fortification or augmentation of the vertebral bodies. In addition to prophylactically stabilizing osteoporotic vertebral bodies at risk for fracture, augmentation of vertebral bodies that have already fractured may prove to be useful by reducing pain, improving function, and preventing furt…

[The arterial supply of the cervical vertebral body in newborns].

The cervical vertebral column is mainly supplied by both vertebral arteries. A regular ladder pattern of arteries is on the dorsal plane as … rtery may have no effect because of the good anastomoses. Some decades of aa. nutriciae come in the vertebral body from all sides, but only 2 up to 4 rr. centrales anteriores and 2 rr. centrales posteriores join in the middle of the vertebral body. There are parallels to the arterial blood supply of the thoracal and the lumbar vertebral bodies. The longitudinal …

Is restoration of vertebral body height after vertebral body fractures and minimally-invasive dorsal stabilization with polyaxial pedicle screws just an illusion?

INTRODUCTION: Thoracolumbar spine fractures often require surgical treatment as they are associated with spinal instability. Optimal operati … follow-up examination after 8.5 ± 8 months. The kyphotic deformity was measured by determining the vertebral body angle, the mono- and bi-segmental wedge angle at three time points. The patients' subjective outcome was evaluated by the VAS spine score. RESULTS: After surgical therapy, a significant reduction of the traumatic kyphotic deformity was shown with a …

The intravertebral vacuum phenomenon ("vertebral osteonecrosis"). Migration of intradiscal gas in a fractured vertebral body?

STUDY DESIGN: Retrospective cohort. OBJECTIVES: To compare the prevalence of the association between contiguous intervertebral disc and vertebral collapses with or without an intravertebral vacuum phenomenon. SUMMARY OF BACKGROUND DATA: The mechanism of occasional gas accumulation within some vertebral collapses is poorly known. The current hypothesis is that this phenomenon is indicative of bone ischemia. In fact, avascular necrosis as the main pathologic event remains speculative, and should not explain per se the presence of gas within a vertebral body. METHODS: Comparison of the prevale…

Growth modulation increases clinical success in vertebral body tethering.

PURPOSE: The relationship between skeletal maturity and postoperative growth modulation (GM) in patients treated with Vertebral Body Tethering (VBT) is poorly understood. We aimed to: (1) identify preoperative skeletal maturity factors associated with GM, and (2) assess the relationship between GM and postoperative success in patients treated with VBT. METHODS: We retrospectively reviewed radiographic data from 55 patients with a minimum follow-up of 2 years (2.6 ± 0.5). Changes in standing height and skeletal maturity (Sanders Stage [SS], Triradiate Cartilage [TRC], and Risser Stage [RS]) we…

Vertebral Body Tethering in AIS Management-A Preliminary Report.

Vertebral Body Tethering (VBT) is a recently developed surgical technique for the treatment of progressive and severe scoliosis in patients with significant growth potential. It has been used since the first exploratory series, which showed encouraging results on the progressive correction of the major curves. This study reports on a retrospective series of 85 patients extracted from a French cohort, with a follow-up at a minimum of two years after a VBT with recent screws-and-tether constructs. The major and compensatory curves were measured pre-operatively, at the 1st standing X-ray, at 1 ye…

Disc and Vertebral Body Morphology From Birth to Adulthood.

STUDY DESIGN: Cross-sectional. OBJECTIVE: The aim of this study was to describe the morphology of intervertebral discs and vertebral bodies … e was analyzed. Custom made software was used to semi-automatically measure intervertebral disc and vertebral body morphology, corrected for orientation in all 3 planes. RESULTS: Vertebral body height increased from birth up to adulthood, from 4-to-14 mm in the cervical, 6 to 20 mm in the thoracic, and 9 to 28 mm in the lumbar spine. This increase was 0.70 mm/y …

Utilizing vertebral body autograft for anterior cervical corpectomy/fusion.

BACKGROUND: There are numerous ways to reconstruct cervical vertebral bodies and achieve arthrodesis following anterior cervical corpectomy and fusion (ACF). However, device and donor site complications abound. Here, we describe a novel technique for performing ACF using the vertebral body itself as a structural autograft. METHODS: The anterior cervical spine was accessed and discectomies were performed rostral and caudal to the corpectomy. Five millimeter troughs were drilled on the lateral borders of the vertebral body, and it was removed en bloc. The autograft was rotated 90°, and an anter…

Vertebral fractures following stereotactic body radiotherapy for spine metastases.

Stereotactic body radiotherapy has emerged as one of the preferred treatments for patients with spine metastases, with the potential for long-term control from lesion irradiation. Post-treatment vertebral compression fractures are a known complication of this therapy, contributing to worsening pain and reduced quality of life, sometimes requiring surgical intervention. This review explores the current knowledge of post-radiotherapy fractures, in terms of the rates and associated predictive factors. A search of databases including Medline, Embase and the Cochrane Library was conducted using key

[Vertebral Body Height after Balloon Kyphoplasty vs. Radiofrequency Kyphoplasty].

BACKGROUND: Kyphoplasty is used to alleviate pain and to restore the initial height of osteoporotic vertebral fractures (OVF). One of the most recent procedures is radiofrequency-targeted vertebral augmentation (RFTVA). We investigated whether restoration with this method is similar and as adequate as with the established procedure of balloon kyphoplasty (BKP), as assessed by the anatomical height of the vertebral body. The aim of our study was to compare the intravertebral angles (base-endplate) post- and preoperatively with these two procedures. PATIENTS AND METHODS: The base and endplate a…

Advanced lung cancer: aggressive surgical therapy vertebral body involvement.

The NOMS considerations provide a dynamic decision framework to determine the optimal combination of systemic and radiation therapies and surgery. Generally, NSCLC metastases to the spine require SRS because cEBRT usually fails to provide consistent long-term local control. Patients with spinal cord compression secondary to NSCLC require surgical decompression to safely undergo SRS and to reduce the risk of radiation-induced spinal cord injury. Separation surgery allows spinal cord decompression and spinal stabilization using the posterior approach and, in combination with SRS, has been shown

Surgical vertebral body cementoplasty as spinal cancer metastasis management.

Spinal metastases of cancer are frequent and their treatment strategy remains a problem. The aim of the present paper is to present the technique of cementoplasty and its originality. Spine corporeal cementoplasty techniques were performed in 9 patients with spine metastasis treated at Neurosurgery Department between 2002 and 2005. This series is composed of four women and five men. The average age of patients was 54 years. All patients were admitted for treatment of metastasis of breast carcinoma, metastasis of breast adenocarcinoma (three cases), colon cancer (two cases), pulmonary adenocarc
