Minimally invasive endoscopic-assisted approaches to the posterior fossa.

Modern surgical approaches are becoming more minimalistic, associated with the term "minimally invasive." The endoscope provides a more panoramic anatomical view in addition to the ability to access narrow deep corners with decent illumination and clear visualization. Endoscopic assisted microsurgery through a tailored small craniotomy is the foundation of keyhole surgery. The endoscope can be advanced deeper into the field, thus enhancing an exposure and allowing bimanual dissection, ultimately providing smaller craniotomies and tailored key exposures. The term "minimally invasive" became ass

Evaluation of brain function in severe human head trauma with multimodality evoked potentials. Part 1: Evoked brain-injury potentials, methods, and analysis.

Methods for obtaining multimodality evoked potentials, somatosensory, visual, auditory, and auditory brain-stem potentials in patients with severe head trauma are described. A method of analyzing abnormal multimodality evoked potentials (graded evoked brain-injury potentials) is proposed that defines the degree of abnormality of the electrophysiological data and expresses it simply in four grades per modality. Data from 20 normal subjects are given for comparison with the abnormal data obtained from 51 patients with head trauma.

Translabyrinthine removal of cerebellopontine angle meningiomas.

The authors report two cases of the microscopic total removal of meningiomas in the region of the cerebellopontine angle using the translabyrinthine approach. Although both tumors were larger than 4 cm, the only resulting neurological deficit was ipsilateral hearing loss. The translabyrinthine approach may be an alternative measure in dealing with these difficult lesions.

Inhibition of the Adenosine A2A Receptor Mitigates Excitotoxic Injury in Organotypic Tissue Cultures of the Rat Cochlea.

The primary loss of cochlear glutamatergic afferent nerve synapses due to noise or ageing (cochlear neuropathy) often presents as difficulties in speech discrimination in noisy conditions (hidden hearing loss (HHL)). Currently, there is no treatment for this condition. Our previous studies in mice with genetic deletion of the adenosine A2A receptor (A2AR) have demonstrated better preservation of cochlear afferent synapses and spiral ganglion neurons after noise exposure compared to wildtype mice. This has informed our current targeted approach to cochlear neuroprotection based on pharmacologic

Prevention of Superior Petrosal Vein Injury during Microvascular Decompression for Trigeminal Neuralgia: Operative Nuances.

Background  The superior petrosal vein (SPV) often obscures the surgical field or bleeds during microvascular decompression (MVD) for the treatment of trigeminal neuralgia. Although SPV sacrifice has been proposed, it is associated with multiple complications. We have performed more than 4,500 MVDs, including approximately 400 cases involving trigeminal neuralgia. We aimed to describe our operative technique and nuances to avoid SPV injury. Methods  We have provided a detailed description of our institutional protocol, including the anesthesia technique, neurophysiologic monitoring, patient po

The role of radiation in delayed hearing loss in nasopharyngeal carcinoma.

Although radiation damage to the inner ear can be induced experimentally in animals, its incidence in humans as a complication of therapy for nasopharyngeal carcinoma (NPC) remains a matter of conjecture due to inadequate histological validation. A case of advanced NPC with a mixed conductive and neurosensory hearing loss is presented that at necropsy showed tumour invasion of the VIIIth cranial nerve in the internal auditory meatus with associated infection. The architecture of the organ of Corti was well-preserved despite heavy doses of radiation but degeneration, the cause of which could no

Vestibular evoked myogenic potentials: review.

BACKGROUND: Disorders of balance often pose a diagnostic conundrum for clinicians, and a multitude of investigations have emerged over the years. Vestibular evoked myogenic potential testing is a diagnostic tool which can be used to assess vestibular function. Over recent years, extensive study has begun to establish a broader clinical role for vestibular evoked myogenic potential testing. OBJECTIVES: To provide an overview of vestibular evoked myogenic potential testing, and to present the evidence for its clinical application. REVIEW TYPE: Structured literature search according to evidence-

Medical history, cigarette smoking and risk of acoustic neuroma: an international case-control study.

Acoustic neuroma (vestibular schwannoma) is a benign tumor of the vestibulocochlear nerve. Its recorded incidence is increasing but risk factors for this tumor have scarcely been investigated. We conducted a population-based case-control study of risk factors for acoustic neuroma in the UK and Nordic countries, including 563 cases and 2,703 controls. Tumor risk was analyzed in relation to medical history and cigarette smoking. Risk of acoustic neuroma was significantly raised in parous compared with nulliparous women (OR = 1.7, 95% CI: 1.1-2.6), but was not related to age at first birth or num

[Effect of impulse noise exposure on the endocochlear potentials].

Effects of impulse noise exposure (25 impulses, peak level 165 dB SPL) on endocochlear potentials (EP) and CAP threshold were investigated in guinea pigs. Eight hours after exposure, EP recorded from the second turn of the cochlea was very low (19.0 +/- 1.7 mV) and the negative EP resulted from prolonged anoxia was markedly diminished (-EPmax = -6.4 +/- 1.3 mV). The return rate of EP after reventilation was also reduced. The positive EP returned to normal values 7 days after exposure but the negative EP and the CAP threshold did not. The results suggest that the stria vascularis was also damag

Intraoperative auditory monitoring in vestibular schwannoma surgery: new trends.

OBJECTIVE: To investigate the efficiency of a new method of brainstem auditory-evoked potential (BAEP) monitoring during complete vestibular schwannoma (VS) resection with attempted hearing preservation. MATERIAL AND METHODS: Dedicated software providing near real-time recording was developed using a rejection strategy of artifacts based on spectral analysis. A small sample number (maximum 200) is required and results are obtained within 10 s. Fourteen consecutive patients with hearing class A operated on for VS, in an attempt to preserve hearing, participated in the investigation. Postoperat

Guinea pig auditory neurons are protected by glial cell line-derived growth factor from degeneration after noise trauma.

For patients with profound hearing loss, cochlear implants have become the treatment of choice. These devices provide auditory information through direct electrical stimulation of the auditory nerve. Prosthesis function depends on survival and electrical excitability of the cochlear neurons. Degeneration of the auditory nerve occurs after lesions of its peripheral target field (organ of Corti), specifically, including loss of inner hair cells (IHCs). There is now evidence that local treatment of the cochlea with neurotrophins may enhance survival of auditory neurons after aminoglycoside-induce

Unexpected Complications Following Accidental Petrosal Vein Damage during Standard Retrosigmoid Surgery for a Large Vestibular Schwannoma: Introspection and Lessons Learned.

We present the case of a patient with a vestibular schwannoma (VS) who developed vascular complications following surgery and discuss the potential mechanisms. Additionally, we systematically searched the literature to identify citations on vascular and brain stem complications following VS surgery. We excluded the articles related to facial and vestibulocochlear nerve-related complications and other complications, such as headache, tinnitus, and ataxia. We also excluded the articles related to recurrent vestibular schwannoma because our article focuses on primary VS surgery-related complicati

Hearing preservation and facial nerve function after microsurgery for intracanalicular vestibular schwannomas: comparison of middle fossa and retrosigmoid approaches.

PURPOSE: Therapeutic options for vestibular schwannomas (VS) include microsurgery, stereotactic radiosurgery and conservative management. Early treatment of intracanalicular vestibular schwannomas (IVS) may be advisable because their spontaneous course will show hearing loss in most cases. Advanced microsurgical techniques and continuous intraoperative monitoring of cranial nerves may allow hearing preservation (HP) without facial nerve damage. However, there are still controversies about the definition of hearing preservation, and the best surgical approach that should be used. METHODS: In t
