Functional analysis of the human rib cage over the vital capacity range in standing position using biplanar X-ray imaging.

Pathologies of the respiratory system can by accompanied by alterations of the biomechanical function of the rib cage, as well as of its morphology and movement. The assessment of such pathologies could benefit from rib cage kinematic analysis during breathing, but this analysis is challenging because of the difficulties in observing and quantifying bone movements in vivo. This work explored the feasibility of using biplanar x-rays to study rib cage modifications at different lung volumes and evaluated the potential of the method to characterize rib cage kinematic patterns in patients. Chest wall muscle atrophy as a contributory factor for forced vital capacity decline in systemic sclerosis-associated interstitial lung disease.

OBJECTIVE: To investigate the potential contribution of accessory respiratory muscle atrophy to the decline of forced vital capacity (FVC) in patients with SSc-associated interstitial lung disease (ILD). METHODS: This single-centre, retrospective study enrolled 36 patients with SSc-ILD who underwent serial pulmonary function tests and chest high-resolution CT (HRCT) simultaneously at an interval of 1-3 years. The total extent of ILD and chest wall muscle area at the level of the ninth thoracic vertebra on CT images were evaluated by two independent evaluators blinded to the patient informatio…

A Low Forced Vital Capacity (FVC)/Diffusing Capacity of the Lung for Carbon Monoxide (DLCO) Ratio Increases Clinical Suspicion for Fibrotic Hypersensitivity Pneumonitis (FHP) Over Idiopathic Pulmonary Fibrosis (IPF).

Background and objective Fibrotic Hypersensitivity Pneumonitis (FHP) and idiopathic pulmonary fibrosis (IPF) are interstitial lung diseases (ILDs) that are challenging to differentiate with prognostic and therapeutic implications. Clinical observations suggest that patients with FHP may have a lower baseline ratio of forced vital capacity (FVC) to the diffusing capacity of the lung for carbon monoxide (DLCO), or FVC/DLCO (F/D) ratio, than patients with IPF. In light of this, we aimed to determine whether patients with FHP have a significantly lower baseline F/D ratio than patients with IPF. Me…

Extracting forced vital capacity from the electronic health record through natural language processing in rheumatoid arthritis-associated interstitial lung disease.

PURPOSE: To develop a natural language processing (NLP) tool to extract forced vital capacity (FVC) values from electronic health record (EHR) notes in patients with rheumatoid arthritis-interstitial lung disease (RA-ILD). METHODS: We selected RA-ILD patients (n = 7485) in the Veterans Health Administration (VA) between 2000 and 2020 using validated ICD-9/10 codes. We identified numeric values in proximity to FVC string patterns from clinical notes in the EHR. Subsequently, we performed processing steps to account for variability in note structure, related pulmonary function test (PFT) output…

Asymmetry of thoracic expansion in hemiplegic stroke and its relationship with physical function and percent vital capacity: A cross-sectional study.

BACKGROUND: Thoracic expansion (TE) is associated with respiratory muscle strength and pulmonary function. However, in patients with stroke who present unilateral motor impairment, the relationship between asymmetry in TE measured bilaterally, physical function, and percent vital capacity (%VC) remains unclear. OBJECTIVE: This study aimed to investigate differences in TE between the sound and hemiplegic sides in patients with stroke, and to examine the associations between TE asymmetry, physical function, and %VC. METHODS: Forty male patients with post-stroke hemiplegia were enrolled. TE was…

Body mass index and vital lung capacity as determinants of maximum oxygen volume (VO2 max): assessing fitness in a sedentary population.

PURPOSE: The increasing prevalence of sedentary lifestyles in modern society has become a significant public health concern, correlating with a rise in obesity rates and various chronic diseases, including cardiovascular and respiratory diseases. A critical metric often assessed in health evaluations is maximum oxygen consumption (VO2 max), which is instrumental in gauging an individual's aerobic fitness level. University staff members who engage in limited physical activity are at risk of reduced vital lung capacity (VLC) due to elevated Body Mass Index (BMI). Therefore, this study aimed to d

Combined Forced Expiratory Volume in 1 Second and Forced Vital Capacity Bronchodilator Response, Exacerbations, and Mortality in Chronic Obstructive Pulmonary Disease.

Rationale: The American Thoracic Society (ATS)/European Respiratory Society defines a positive bronchodilator response (BDR) by a composite of BDR in either forced expiratory volume in 1 second (FEV1) and/or forced vital capacity (FVC) greater than or equal to 12% and 200 ml (ATS-BDR). We hypothesized that ATS-BDR components would be differentially associated with important chronic obstructive pulmonary disease (COPD) outcomes. Objectives: To examine whether ATS-BDR components are differentially associated with clinical, functional, and radiographic features in COPD. Methods: We included subje…

Accuracy of Balloons and Handheld Spirometers as Low-Cost Tools for Measuring Vital Capacity and Air Volume: A Laboratory-Based Validation Study.

INTRODUCTION: Vital capacity and air volume are key physiological substrates important for voice, speech, cough, and swallowing. The current gold standard equipment to assess vital capacity and air volume is a pneumotachograph. The aim of this study was to characterize the validity of four low-cost, clinically accessible tools as potential alternative methods to accurately assess vital capacity and air volume. METHODS: This laboratory-based study examined the accuracy of four low-cost tools to assess vital capacity and air volume: two balloon-based circumference methods (<USD 1), one analogue…

Venous Bicarbonate as a Prognostic Biomarker and Proposed Proxy for Vital Capacity to Be Used as an Eligibility Criterion in Amyotrophic Lateral Sclerosis Clinical Trials.

BACKGROUND: Clinical trials for people living with ALS (pALS) all list vital capacity (VC) as an important eligibility criterion. However, VC measures are challenging to perform among pALS, especially bulbar pALS. Additionally, since the disease rapidly changes, the VC can change in a short duration of time, making it unreliable. OBJECTIVE: We aimed to investigate the association of venous bicarbonate with VC as an alternative criterion for trial eligibility. We also wanted to examine whether venous bicarbonate could be used as a prognostic biomarker for survival in ALS. METHODS: We included…

Forced vital capacity trajectories and risk of lung transplant and ILD-related mortality among patients with rheumatoid arthritis-associated interstitial lung disease.

We aimed to determine the prevalence and outcomes for forced vital capacity percent predicted (FVCpp) decline among patients with rheumatoid arthritis-associated interstitial lung disease (RA-ILD). We identified patients with RA-ILD in the Mass General Brigham Healthcare system. RA-ILD diagnosis was determined by review of high-resolution computed tomography (HRCT) imaging by up to three thoracic radiologists. We abstracted FVCpp measurements, covariates, lung transplant, and ILD-related death from the medical record. We employed a relative FVCpp decline cutoff of > 10% within 24 months. We al…
