Biodistribution of mRNA COVID-19 vaccines in human breast milk.

BACKGROUND: COVID-19 mRNA vaccines play a vital role in the fight against SARS-CoV-2 infection. However, lactating women have been largely excluded from most vaccine clinical trials. As a result, limited research has been conducted on the systemic distribution of vaccine mRNA during lactation and whether it is excreted in human breast milk (BM). Here, we evaluated if COVID-19 vaccine mRNA is detectable in BM after maternal vaccination and determined its potential translational activity. METHODS: We collected BM samples from 13 lactating, healthy, post-partum women before and after COVID-19 mR…

mRNA vaccines: a new opportunity for malaria, tuberculosis and HIV.

The success of the first licensed mRNA-based vaccines against COVID-19 has created a widespread interest on mRNA technology for vaccinology. As expected, the number of mRNA vaccines in preclinical and clinical development increased exponentially since 2020, including numerous improvements in mRNA formulation design, delivery methods and manufacturing processes. However, the technology faces challenges such as the cost of raw materials, the lack of standardization, and delivery optimization. MRNA technology may provide a solution to some of the emerging infectious diseases as well as the deadli…

Seroconversion to mRNA SARS-CoV-2 Vaccines in Hematologic Patients.

Hematologic patients show lower responses to SARS-CoV-2 vaccines, but predictors of seroconversion are lacking. In this prospective cohort study, hematologic patients undergoing SARS-CoV-2 mRNA vaccination at a single center in Milan, Italy, were sampled for anti-Spike and anti-Nucleocapsid IgG titer at 5 ± 1 weeks and at 3 months from the second vaccine dose. Patients (N = 393) received either BNT162b2 (Pfizer-BioNTech, 48%) or MRNA-1273 (Moderna, 52%), and 284 (72%) seroconverted and 100% persisted at 3 months. Non-response was higher in chronic lymphocytic leukemia (CLL) and lymphoma patien

The new generation of messenger RNA (mRNA) vaccines against influenza.

Today there are multiple types of flu vaccines. The emergence of nucleic acid technology used in vaccines against SARS-CoV-2 suggests its future application against this infection. Against influenza, two types of vaccines have been developed based on messenger RNA (mRNA): conventional or non-replicative and self-amplifying or replicative (auRNA), both included in lipid nanoparticles. Animal studies carried out with the former have shown their strong capacity to induce Th-1 antibodies and cellular immunity against influenza haemagglutinin (HA) with few side effects. Human trials have shown 87%

Chimeric spike mRNA vaccines protect against Sarbecovirus challenge in mice.

The emergence of severe acute respiratory syndrome coronavirus (SARS-CoV) in 2003 and SARS-CoV-2 in 2019 highlights the need to develop univ … ruses, but SARS-CoV and WIV-1 challenge in mice resulted in breakthrough infections. Chimeric spike mRNA vaccines efficiently neutralized D614G, mink cluster five, and the UK B.1.1.7 and South African B.1.351 variants of concern. Thus, multiplexed-chimeric spikes can prevent SARS-like zoonotic coronavirus infections with pandemic potential.

Comparative safety of mRNA COVID-19 vaccines to influenza vaccines: A pharmacovigilance analysis using WHO international database.

Two messenger RNA (mRNA) vaccines developed by Pfizer-BioNTech and Moderna are being rolled out. Despite the high volume of emerging evidence regarding adverse events (AEs) associated with the COVID-19 mRNA vaccines, previous studies have thus far been largely based on the comparison between vaccinated and unvaccinated control, possibly highlighting the AE risks with COVID-19 mRNA vaccination. Comparing the safety profile of mRNA vaccinated individuals with otherwise vaccinated individuals would enable a more relevant assessment for the safety of mRNA vaccination. We designed a comparative saf…

Progress and Prospects of Nanodelivery Systems in Metabolism-Regulated mRNA Vaccines.

mRNA vaccines face two major challenges in tumor immunotherapy: inefficient delivery and an immunosuppressive tumor microenvironment (TME) driven by metabolic reprogramming. This review proposes that nanodelivery systems can overcome both obstacles by protecting and targeting mRNA while simultaneously reshaping tumor metabolism and immunity. We summarize how nanocarriers act as "metabolism-immunity dual regulation centers" through three mechanisms: co-delivery of metabolic regulators, intrinsic modulation of immune cell metabolism via material properties, and TME-responsive spatiotemporal rele…

Individualized mRNA vaccines evoke durable T cell immunity in adjuvant TNBC.

Triple-negative breast cancer (TNBC) is frequently associated with metastatic relapse, even at an early stage1. Here we assessed an individualized neoantigen mRNA vaccine in 14 patients with TNBC following surgery and after neoadjuvant or adjuvant therapy. In peripheral blood of nearly all patients, high-magnitude, vaccine-induced, mostly de novo T cell responses to multiple neoantigens were detected that remained functional for several years. Characterization of individual patients revealed that a large proportion of these T cells developed into two subsets: a late-differentiated phenotype wi

mRNA vaccines for HBV: Mechanisms, preclinical advances, and therapeutic clinical progress.

Hepatitis B virus (HBV) remains a leading cause of cirrhosis and hepatocellular carcinoma, while functionally curative therapies remain scarce. Durable remission is hindered by the persistence of covalently closed circular DNA (cccDNA) and viral genome integration, both of which contribute to impaired immune responses. mRNA-based technologies provide a versatile platform for prevention and treatment, owing to their rapid development cycle and intrinsic immunostimulatory properties. In preclinical HBV-carrier mouse models, lipid-nanoparticle (LNP) mRNA vaccines encoding hepatitis B surface anti…

pH-Isomerizable Acylhydrazone-Based Ionizable Lipids for Spleen-Targeted mRNA Vaccines.

Effectively and safely amplifying systemic immunity against heterogeneous and immunosuppressive cancers remains highly challenging. Here, we report a library of acylhydrazone-based ionizable lipids (AHzILs) that exploit pH-responsive E/Z isomerization to dynamically tune the membrane properties of mRNA-encapsulated lipid nanoparticles (LNPs), thereby achieving spleen targeting, efficient endosomal escape, and robust immune activation. These isomerizable mRNA vaccines acquired a protein corona enriched in mannose-binding protein A and vitronectin, promoting preferential uptake by antigen-presen…

SARS-CoV-2 mRNA vaccines sensitize tumours to immune checkpoint blockade.

Immune checkpoint inhibitors (ICIs) extend survival in many patients with cancer but are ineffective in patients without pre-existing immunity1-9. Although personalized mRNA cancer vaccines sensitize tumours to ICIs by directing immune attacks against preselected antigens, personalized vaccines are limited by complex and time-intensive manufacturing processes10-14. Here we show that mRNA vaccines targeting SARS-CoV-2 also sensitize tumours to ICIs. In preclinical models, SARS-CoV-2 mRNA vaccines led to a substantial increase in type I interferon, enabling innate immune cells to prime CD8+ T ce…

Key Clinical Frontiers of mRNA Loaded Lipid Nanoparticles in Cancer Vaccines.

Cancer vaccines are promising, but clinical translation is constrained by inefficient antigen delivery and suboptimal immune activation. Lipid nanoparticles (LNPs)-validated for potency and safety in COVID-19 mRNA vaccines-offer a versatile, scalable, and immunogenic platform. Key barriers persist: precise targeting of tumors or lymphoid tissues, efficient intracellular mRNA release, and the immunosuppressive tumor microenvironment. This review synthesizes design principles for mRNA-loaded LNPs, emphasizing lipid chemistry, organ-selective biodistribution, and nano-engineering strategies that …

Advancements in breast cancer mRNA vaccines: Current development and future prospects.

Messenger RNA (mRNA) vaccines have become a transformative approach in immunotherapy and have attracted significant attention owing to their unprecedented success in controlling COVID-19. With their ability to flexibly and specifically encode tumour-associated antigens, along with their favorable safety profiles and scalable manufacturing, mRNA vaccines represent a highly promising platform for cancer treatment. Breast cancer is a heterogeneous disease and many of its subtypes are immunologically cold tumours, which has limited the progress of immunotherapy in this field. Recent studies have h…

Novel mRNA vaccines induce potent immunogenicity and afford protection against tuberculosis.

INTRODUCTION: Mycobacterium tuberculosis (Mtb) is the causative agent of tuberculosis (TB), a disease with a severe global burden. The intractability of Mtb has prevented the identification of clear correlates of protection against TB and hindered the development of novel TB vaccines that are urgently required. Lipid nanoparticle (LNP)-formulated mRNA is a highly promising vaccine platform that has yet to be thoroughly applied to TB. METHODS: We selected five Mtb antigens (PPE15, ESAT6, EspC, EsxI, MetE) and evaluated their potential as LNP-formulated mRNA vaccines, both when each antigen was…

Next-generation mRNA vaccines eliciting robust protection against multidrug-resistant Enterobacteriaceae.

Antibiotics are essential for treating bacterial infections, but the growing problem of antimicrobial resistance (AMR) undermines their effectiveness. Vaccines targeting multidrug-resistant (MDR) bacteria are urgently needed. Here, we developed next-generation mRNA vaccines encoding two novel target antigens: phosphate-specific transport protein (PstS) and DUF3748 domain-containing protein (YidR). The resulting fusion proteins exhibited high expression and secretion in vitro and provided strong protective efficacy in mice against Klebsiella pneumoniae (K. pneumoniae) and enterohemorrhagic Esch…

Current innovations in mRNA vaccines for targeting multidrug-resistant ESKAPE pathogens.

The prevalence of multidrug-resistant (MDR) ESKAPE pathogens, including Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, and Pseudomonas aeruginosa, represents a critical global public health challenge. In response, mRNA vaccines offer an adaptable and scalable platform for immunotherapy against ESKAPE pathogens by encoding specific antigens that stimulate B-cell-driven antibody production and CD8+ T-cell-mediated cytotoxicity, effectively neutralizing these pathogens and combating resistance. This review examines recent advancements and ongoing chal…
