Video-based system for intraoperative facial nerve monitoring: comparison with electromyography.

OBJECTIVE: To validate a recently developed intraoperative facial nerve monitoring system that is based on video control of facial movements. STUDY DESIGN: In a single-subject design study, involving 15 otoneurosurgical patients, the relationship between intensity of neural stimulation, facial movements, and electrophysiologic voltage were measured. The analysis was performed by measuring the ipsilateral oral commissure displacement in relation to different levels of current administered to the nerve during surgical procedures. SETTING: Electromyography and video system intraoperative facial

[Neurophysiological criteria for the diagnosis and prognosis of facial nerve neuropathy in children].

Electromyography bindings of blink reflex, visual and acoustic evoked potentials have been studied in 35 children with facial nerve neuropathy (FNN), 20 children with FNN being manifested for the first time and 15 children being affected with recurrent type. Blink reflex analysis in FNN has revealed trifacial and controlateral facial nerve pathology in 10-20% cases. Factors essentional for rehabilitation and development of complications were the following: (1) duration of conduction block along trigeminis-facial nerve arch, (2) conduction block association, according to the blink reflex, with

Continuous retrograde monitoring of the facial nerve during cerebellopontine angle surgery: normative data.

An alternative technique for the continuous monitoring of the facial nerve, monopolar recording of facial nerve antidromic potentials (FNAPs), on 10 subjects undergoing retrosigmoid vestibular neurectomy for Meniere's disease is described. To elicit FNAPs bipolar electrical stimulation of the marginalis mandibulae was performed. Stimulus intensity ranged from 0 to 10 mA with a delivery rate of 7/second. Antidromic potentials were recorded with a silver wire monopolar electrode positioned intracranially on the proximal portion (root entry zone) of the acoustic-facial bundle. Bipolar recordings

Comparison of Responses to DCN vs. VCN Stimulation in a Mouse Model of the Auditory Brainstem Implant (ABI).

The auditory brainstem implant (ABI) is an auditory neuroprosthesis that provides hearing to deaf patients by electrically stimulating the cochlear nucleus (CN) of the brainstem. Whether such stimulation activates one or the other of the CN's two major subdivisions is not known. Here, we demonstrate clear response differences from the stimulation of the dorsal (D) vs. ventral (V) subdivisions of the CN in a mouse model of the ABI with a surface-stimulating electrode array. For the DCN, low levels of stimulation evoked multiunit responses in the inferior colliculus (IC) that were unimodally dis

Hydroxyapatite-mineralized metal-organic frameworks as sonosensitizer modulate tumor microenvironment to potentiate high-intensity focused ultrasound therapy.

High-intensity focused ultrasound (HIFU) has emerged as a game-changing breast-conserving therapeutic modality owing to its dual functions of non-invasive tumor ablation and immunity activation. However, its low focal gain in the target region results in residual tumor tissue after ablation. Inspired by the exceptional acoustic response of bone tumor to HIFU, we developed bone-derived hydroxyapatite (HA)-mineralized metal-organic frameworks (designated as AZSH MOFs) as sonosensitizer that co-encapsulated Zn2+, hypoxic-activated prodrug AQ4N, and STING agonist SR-717 for acoustic-immunological

Sketches of chimpanzee (Pan troglodytes) hoo's: vowels by any other name?

In human speech, the close back rounded vowel /u/ (the vowel in "boot") is articulated with the tongue arched toward the dorsal boundary of the hard palate, with the pharyngeal cavity open. Acoustic and perceptual properties of chimpanzee (Pan troglodytes) hoo's are similar to those of the human vowel /u/. However, the vocal tract morphology of chimpanzees likely limits their phonetic capabilities, so that it is unlikely, or even impossible, that their articulation is comparable to that of a human. To determine how qualities of the vowel /u/ may be achieved given the chimpanzee vocal tract, we

Removal of large acoustic neurinomas (vestibular schwannomas) by the retrosigmoid approach with no mortality and minimal morbidity.

OBJECTIVE: To evaluate the safety and efficacy of removing large acoustic neurinomas (> or =3 cm) by the retrosigmoid approach. METHODS: Large acoustic neurinomas (mean (SD), 4.1 (0.6) cm) were removed from 50 consecutive patients by the retrosigmoid suboccipital approach while monitoring the facial nerve using a facial stimulator-monitor. Excision began with the large extrameatal portion of the tumour, followed by removal of the intrameatal tumour, and then removal of the residual tumour in the extrameatal region just outside the porus acusticus. The last pieces of tumour were removed by sha

Effect of coupler tip size on round window stimulation with the floating mass transducer under controlled preload.

OBJECTIVE: Mechanical stimulation of the cochlear round window (RW) extends the indication of active middle-ear implants to mixed and conductive hearing loss, yet outcomes vary considerably due to transducer-RW size mismatch and uncontrolled preload. The Hannover Coupler version 2 (HC2) addresses these issues with a small tip and a force indicator. This study further evaluates the effect of HC2 tip size on RW stimulation efficiency under controlled preload force. METHODS: Stapes displacements were measured in six human temporal bones using laser Doppler vibrometry during acoustic and RW stimu

Effect of deep brain stimulation in rats selectively bred for reduced prepulse inhibition.

BACKGROUND: Sensorimotor gating, measured as prepulse inhibition (PPI) of the acoustic startle reaction (ASR), is disturbed in certain neuropsychiatric disorders, such as schizophrenia, obsessive compulsive disorder, and Tourette's syndrome (TS). Deep brain stimulation (DBS) of the centromedian-parafascicular complex (CM-Pf), globus pallidus internus (in rats the entopeduncular nucleus - EPN), and the ventral striatum (in rats the nucleus accumbens - NAC) has been used for treatment in TS. OBJECTIVE: We tested whether DBS of these regions would alleviate breeding-induced low PPI in rats. MET

The effect of transcutaneous vagus nerve stimulation on cortical excitability.

There is great interest about the therapeutic potentialities of transcutaneous vagus nerve stimulation (tVNS) applied to neuropsychiatric disorders. However, the mechanisms of action of tVNS and its impact on cortical excitability are unclear. To this regard, transcranial magnetic stimulation (TMS) can be useful because it is able of evaluating non-invasively excitatory and inhibitory circuitry of the human cortex. Aim of the present study is to investigate the effects of tVNS on cerebral cortex excitability in healthy volunteers by means of TMS. Ten healthy subjects participated in this rando

The timbre of the voice as perceived by the singer him-/herself.

Fifteen professional singers sang simple vocal exercises at different pitches. The sung excerpts were recorded, and seven modified versions were created from each recording. The modifications were then played to the participant and (s)he had to assess the similarity of these stimuli to the perception of his/her own voice during the act of singing. Participants rated as most similar those stimuli which were modified by the filter whose frequency response most closely resembled a trapezoid, which was created by taking into account 1) the diffracting air conduction component from the mouth of the

Improving non-native duration contrast with dichotic training in dyslexic and non-dyslexic individuals.

Perceiving and producing English phonemic vowel length contrasts is challenging for non-native speakers. According to multi-time resolution models, endogenous slow/fast rhythms contribute, respectively, in the right/left hemispheres, to long/short acoustic cue processing. This study introduced a perceptual training method implementing dichotic stimulation to improve /i:/-/ɪ/ processing by promoting hemispheric complementarity. Twenty non-dyslexic and 20 dyslexic French adults received 1 hr-training over 3 days. Productions were evaluated with pre-/post-tests. Training enhanced vowel duration c

Breathing detection from tracheal sounds in both temporal and frequency domains in the context of phrenic nerve stimulation.

Electrical stimulation of the phrenic nerves via implanted devices allows to counteract some disadvantages of mechanical ventilation in patients with high tetraplegia or Ondine's syndrome. Existing devices do not allow to monitor breathing or to adapt the electroventilation to patients' actual needs. A reliable breathing monitor with an inbuilt alarm function would improve patient safety. In our study, a real-time acoustic breathing detection method is proposed as a possible solution to improve implanted phrenic stimulation. A new algorithm to process tracheal sounds has been developed. It com

Influence of medial olivocochlear efferents on the sharpness of cochlear tuning estimates in children.

The present study objectively quantified the efferent-induced changes in the sharpness of cochlear tuning estimates and compared these alterations in cochlear tuning between adults and children. Click evoked otoacoustic emissions with and without contralateral broadband noise were recorded from 15 young adults and 14 children aged between 5 and 10 yrs. Time-frequency distributions of click evoked otoacoustic emissions were obtained via the S-transform, and the otoacoustic emission latencies were used to estimate the sharpness of cochlear tuning. Contralateral acoustic stimulation caused a sign…

Far field potentials from the brain stem after transcutaneous vagus nerve stimulation.

Recently, the vagus nerve has gained particular interest in neuropsychiatry, as neurodegenerative diseases like Alzheimer's and Parkinson's disease are supposed to affect the brainstem nuclei of the vagus nerve early in their course. In addition, electric stimulation of the vagus nerve has therapeutic effects in otherwise therapy-refractory epilepsies and depressions. So far, no method is available to assess vagus nerve function in this context. On this background and based on the established techniques of early acoustic evoked potentials we investigated if a transcutaneous electric stimulatio

Auditory brainstem implant in auditory rehabilitation of patients with neurofibromatosis type 2: Hannover programme.

An auditory brainstem implant (ABI) is indicated for patients suffering from bilateral neural deafness. The most affected patients are those with neurofibromatosis type 2 (NF2). An implantation is possible either at the same time as, or after, surgical removal of an acoustic neuroma. This paper demonstrates the results of eight out of 11 patients with NF2, seven of whom received an ABI after tumour removal. Pre-operatively, all of them were deaf. Post-operatively, the first fitting served to determine the individual stimulation parameters for each electrode. The stimulation-dependent side-effe

Intracellular recordings in response to monaural and binaural stimulation of neurons in the inferior colliculus of the cat.

The inferior colliculus (IC) is a major auditory structure that integrates synaptic inputs from ascending, descending, and intrinsic sources. Intracellular recording in situ allows direct examination of synaptic inputs to the IC in response to acoustic stimulation. Using this technique and monaural or binaural stimulation, responses in the IC that reflect input from a lower center can be distinguished from responses that reflect synaptic integration within the IC. Our results indicate that many IC neurons receive synaptic inputs from multiple sources. Few, if any, IC neurons acted as simple re…

Auditory threshold evaluation by distortion-product oto-acoustic emissions using decision support system.

Distortion-product oto-acoustic emissions (DPOEs) were measured in 102 normal-hearing and hearing-impaired ears to examine the sensibility, specificity, positive predictive value and negative predictive value of this test in a real clinical setting using two decision support systems. The first model only considered the presence or the absence of DPOEs in response to a 52 dB SPL stimulation intensity. The second model used two consecutive tests: i) the presence or the absence of DPOEs in response to a 52 dB SPL stimulation intensity, and ii) evaluation of the slope of each DPOE input-output fun

Peripheral auditory processing in normal and abnormal ears: physiological considerations for attempts to compensate for auditory deficits by acoustic and electrical prostheses.

The processing of complex acoustic signals, such as speech, by the normal peripheral auditory system is described in terms of the properties of individual cochlear nerve fibres and the effects of lateral inhibition in the cochlear nucleus. The implications of the peripheral filtering of signals is indicated in psychophysical terms. In cochlear pathology, these filtering characteristics deteriorate, and the implications arising from the consequent deterioration in auditory frequency selectivity are outlined for the coding of loudness and for the analysis of speech. These considerations suggest

Electrophysiological investigations on the projections from the cerebral cortex to the vermal posterior lobe of the cerebellum.

The following cerebrocortical areas have been electrically stimulated in cats under Nembutal anaesthesia: forelimb and hindlimb areas of the primary sensorimotor cortex, primary and secondary acoustic areas and visual area. Stimulation of these regions evokes in the vermal portion of lobuli VI and VII of the cerebellum potentials at a short latency (2.8-3.5 ms) and at a longer latency (12-22 ms), which have been identified as due to mossy and climbing fibre inputs respectively. Each point of the cerebellar cortex receives usually projections by some and never by all the stimulated cerebrocorti
