Positional blood pressure change and the risk of hypotension during spinal anesthesia for cesarean delivery: an observational study.

BACKGROUND: We investigated whether preoperative positional arterial blood pressure change predicted hypotension and ephedrine requirement during spinal anesthesia for cesarean delivery. METHODS: Arterial blood pressure was measured in 66 women undergoing cesarean delivery in the supine and the right lateral positions. Positional blood pressure change was defined as the difference between mean blood pressure in the right lateral and supine positions. Hypotension (<80% baseline) was recorded, and severe hypotension (<70% baseline) was treated with ephedrine. RESULTS: The mean (range) position

Intracranial subdural hematoma post-spinal anesthesia: report of two cases and review of 33 cases in the literature.

BACKGROUND AND OBJECTIVES: Intracranial subdural hematoma is a rare complication of spinal anesthesia. We report two cases of women who developed chronic subdural hematoma post-spinal anesthesia diagnosed after prolonged clinical evolution of post-dural puncture headache (PDPH) and we analyze other 33 cases found on literature review. CASE REPORTS: In 35 patients (ages 20-88 years, 19 males), 14 were older than 60 years (40%), of which 12 (86%) were males. The relationship is inverted in the group of younger patients (< 60 years), in which we observed twice as many women (14:7). Two peaks of

Hetastarch co-loading is as effective as pre-loading for the prevention of hypotension following spinal anesthesia for cesarean delivery.

BACKGROUND: Pre-loading with hetastarch decreases the incidence and severity of hypotension after spinal anesthesia for cesarean delivery. However, pharmacokinetic studies with crystalloid predict that fluid loading should be more efficacious if rapidly administered immediately after induction of spinal anesthesia. The aim of this study was to compare pre- and co-loading of hetastarch for the prevention of hypotension following spinal anesthesia for cesarean delivery. METHODS: Forty-six healthy term parturients scheduled for cesarean delivery were randomized to receive 500 mL of 6% hetastarch

[Effects of induction of anesthesia on hemodynamics in children with deformities of the spinal cord when turning to the position lying on the belly].

The investigation was devoted to assessment of the reaction of blood circulation to turning to the position "lying on the belly" in different variants of induction of anesthesia to children aged from 7 to 17 years having deformities of the spinal cord. The following combinations were compared: Ketamin, Fentanyl, and inhalation with nitrous oxide; Propofol and Fentanyl; Thiopental, Clofelin, Fentanyl and inhalation with nitrous oxide; Propofol, Clofelin and Fentanyl. The minimal changes in blood circulation were found to occur when using the combination of Thiopental, Clofelin and Fentanyl.

Failed regional anesthesia with reduced spinal bupivacaine dosage in a parturient with achondroplasia presenting for urgent cesarean section.

A 36-year-old patient with a history of previous back surgery, asthma, latex allergy and achondroplasia presented for urgent cesarean delivery at 31 weeks' gestation for worsening nonimmune fetal hydrops. The fetus was diagnosed with trisomy 21 and achondroplasia. Because of the urgent clinical situation, the patient was given a spinal anesthetic, which required supplemental intravenous sedation after delivery of the fetus. This case report discusses the controversies in anesthetic management of this complicated patient and compromised fetus regarding general anesthesia, epidural, spinal and c

Prevention of spinal anesthesia-induced hypotension in the elderly: comparison between preanesthetic administration of crystalloids, colloids, and no prehydration.

The practice of routinely prehydrating patients by infusing a crystalloid or colloid solution (up to 1.0 L/70 kg) for prevention of spinal anesthesia-induced hypotension has been challenged recently, after several reports of failure to demonstrate its efficacy in young women. We compared the incidence and frequency of hypotension and vasopressor therapy after spinal anesthesia and no prehydration with crystalloid and colloid prehydration in elderly patients. Eighty-five ASA grade I or II patients (aged 60-89 yr) for elective total hip replacement were randomized to receive 500 mL crystalloid s

In vitro modeling of spinal anesthesia. A digital video image processing technique and its application to catheter characterization.

BACKGROUND: Maldistribution of intrathecal local anesthetic has recently been implicated as a contributor to neurotoxic injury. In vitro modeling can be used to understand the distribution of anesthetic agents within the subarachnoid space. We describe an in vitro modeling technique that uses digital video image processing and its application to catheter injection of local anesthetic. METHODS: A clear plastic model of the subarachnoid space, including a simulated spinal cord and cauda equina, was filled with lactated Ringer's solution. Phthalocyanine blue dye of known concentration was inject

Continuous Spinal Anesthesia in a High-Risk Patient With Pulmonary Hypertension Undergoing Hip Fracture Repair: A Case Report.

This report describes the successful use of continuous spinal anesthesia (CSA) in a 70-year-old woman with severe pulmonary hypertension (PH), right ventricular (RV) dysfunction, and obstructive sleep apnea (OSA) undergoing hip fracture repair. The perioperative challenge was to manage a patient at high risk of hemodynamic decompensation and severe gas exchange impairment. CSA allowed preservation of spontaneous ventilation and incremental titration of local anesthetic, while hemodynamic stability was maintained with early norepinephrine support. This case illustrates the importance of individ

Hemodynamic effects of plain versus hyperbaric bupivacaine during spinal anesthesia for total knee arthroplasty: a randomized clinical trial.

BACKGROUND: Spinal anesthesia (SA) is widely used for elective total knee arthroplasty (TKA) but is frequently associated with episodes of low blood pressure. Because baricity influences intrathecal spread, plain bupivacaine (PB) and hyperbaric bupivacaine (HB) may differ in their early hemodynamic effects. This trial compared the early hemodynamic consequences of PB versus HB for SA in elective TKA. METHODS: This prospective, double-blinded, single-center randomized controlled superiority trial enrolled adult patients undergoing elective TKA under SA, randomized 1:1 to receive 2 mL of 0.5% P

Spinal Anesthesia After Thoracolumbar Fixation for Scoliosis: A Case Report Emphasizing the Role of Chest X-ray Reassessment.

Spinal anesthesia (SA) is widely favored for lower limb orthopedic surgeries due to its rapid onset, effectiveness, and lower risk compared to general anesthesia (GA), particularly in patients with comorbidities. Nevertheless, prior spinal surgery presents significant challenges to SA because of deformed anatomy, scarring, and altered surface landmarks. In this article, we report successful landmark-based SA using radiographic reassessment, without ultrasound, in an elderly patient with multiple comorbidities and previous thoracolumbar fixation for scoliosis who was scheduled for elective tota

A machine learning model for automated anesthesia risk classification in lumbar spinal stenosis patients: development and multicenter validation.

BACKGROUND: Patients with lumbar spinal stenosis are typically elderly with multiple comorbidities, necessitating accurate preoperative anesthetic risk assessment. The American Society of Anesthesiologists (ASA) classification quantifies functional reserve and disease burden, serving as a widely used tool for risk stratification. However, ASA classification is often influenced by subjective factors including physician experience and varies among clinicians with different seniority, while the assessment process remains time-consuming. This study aimed to develop an automated model for anestheti

Spinal Versus General Anesthesia for Acute Kidney Injury and Transfusion in One-Week-Staged Bilateral Total Knee Arthroplasty.

Background/Objectives: Evidence on spinal versus general anesthesia in unilateral total knee arthroplasty (TKA) may not extend to one-week-staged bilateral surgery, where older patients receive two anesthetics in a short interval and intra-operative spinal-to-general conversion is common but rarely reported transparently. We compared peri-operative acute kidney injury (AKI) and transfusion between strategies in this setting. Methods: We retrospectively analyzed 207 patients (414 surgeries) undergoing one-week-staged bilateral primary TKA at one center. Co-primary endpoints were creatinine-base

Tourniquet-Induced Atypical Lower Back Pain Under Spinal Anesthesia: A Case Report and Mechanistic Exploration of Referred Pain.

Tourniquet-associated complications typically manifest as localized limb symptoms or systemic reactions. Here, we report a rare case of acute, severe lower back pain directly induced by a tourniquet during lower limb surgery under spinal anesthesia. A 32-year-old male developed intense lower back pain (VAS: 4-10) approximately 1.5 h after tourniquet application to the right thigh, accompanied by elevated blood pressure and agitation, while bilateral lower limb sensorimotor blockade remained intact (T11-S5 level). Intravenous analgesics provided only transient and limited relief. Crucially, wit
