The authors analyze the results of early (on days 5-8 postoperation) contact correction with soft contact lenses (SCL), carried out in 126 infants (206 eyes), operated on for congenital cataract extraction at the age of 2-12 mos. Soviet IOZM, Czechoslovak Spofa-lens, and American Bausch and Lomb lenses have been applied for 18-14 hrs daily, in parallel with the treatment for amblyopia (direct occlusion, a total exposure of the retina), carried out at home. A thorough adaptation, observance of the hygiene and schedule of the lens use have been a guarantee of the absence of ocular complications.

In 13 pseudophakic eyes, endothelial cell counts remained stable following pars plana vitrectomy. The posterior chamber pseudophakos apparently had a protective effect on the corneal endothelium, even though the posterior lens capsule was removed during surgery. By contrast, in nine aphakic eyes, a statistically significant (p greater than .05) cell loss of 7.6% occurred after pars plana vitrectomy. We conclude that vitrectomy therefore poses an increased risk to the cornea in aphakic, but not in pseudophakic eyes.

Contrast sensitivity was measured in pseudophakic and aphakic patients. No difference could be demonstrated between the contrast sensitivities of patients with anterior chamber/iris-supported and posterior chamber intraocular lenses (IOLs). Little difference was found between pseudophakics and aphakics. Interocular contrast ratios in five pseudophakic patients with normal fellow eyes revealed a range of contrast attenuation characteristics which may be attributed to the IOL and corneal and posterior capsule transmission.

We used wide-field color specular microscopy of the human corneal surface to study the morphologic changes that occur under extended-wear contact lenses. There was a shift in the frequency distribution of surface cells by size to larger cells, suggesting a prolonged residence time on the ocular surface. Other abnormalities included retained desiccated mucin, coarse mucus plaques, and palisading of cells.

We examined 201 consecutive aphakic and pseudophakic eyes postmortem. Of these, 146 eyes had undergone intracapsular cataract extraction (ICCE) and 55 eyes had undergone extracapsular cataract extraction (ECCE), either with the posterior capsule preserved intact (ECCE-CI, 30 eyes) or having had a surgical discission performed (ECCE-D, 25 eyes). The first 46 patients to undergo surgery for squint with adjustable sutures at Moorfields Eye Hospital, City Road, London, are presented. The indications for surgery were: dysthyroid ophthalmopathy, fourth nerve palsy, monocular aphakia with strabismus and miscellaneous conditions. The technique of surgery is briefly described. The results show a high incidence of alignment, indicating the value of the technique and this is supported by long-term follow-up data where available. Treatment failures are analysed in detail.

Serious corneal complications occurred in an otherwise successful experience with continuous-wear soft contact lenses (SCLs) for aphakic correction. One hundred twenty eyes were fit, and 92% attained visual acuity of 20/40 or better.
