Re-laparoscopy for the treatment of complications after laparoscopic appendectomy: is it possible to maintain the minimally invasive approach?

Despite laparoscopy is considered an adequate tool for the diagnosis and management of postoperative surgical complications, its role after laparoscopic appendectomy (LA) remains uncertain. The aim of this study was to evaluate whether laparoscopy is useful for treating complications after laparoscopic appendectomy. A retrospective analysis of a prospectively collected database of patients undergoing LA, who needed a reoperation for postoperative complications during the period 2006-2020, was performed. Demographics, operative variables, and postoperative outcomes were analyzed. A total of 201…

Successful perioperative management of a patient with idiopathic thrombocytopenic purpura undergoing emergent appendectomy: Report of a case.

INTRODUCTION: Idiopathic thrombocytopenic purpura (ITP) is an autoimmune disease characterized by a low platelet count and normal bone marrow. Patients with ITP undergoing surgery are thought to have increased risk for postoperative complications because of their thrombocytopenia. PRESENTATION OF CASE: we report the case of a 66-year-old woman with ITP who required an emergency operation for acute appendicitis associated with disseminated intravascular coagulation. Preoperative therapy consisted of platelet transfusions only, and intraoperative hemostasis was achieved. Postoperatively, high-d

Securing the appendiceal stump with the Gea extracorporeal sliding knot during laparoscopic appendectomy is safe and economical.

BACKGROUND: Laparoscopic appendectomy (LA) has become very popular. One criticism of this approach is the high cost of the disposable equipment such as the linear stapler. An alternative would be suture ligation of the appendiceal base. To prove the safety of the Gea extracorporeal sliding knot (GESK) for closure of the stump after LA, a retrospective study was conducted. METHODS: For this study, 63 LA procedures performed by one surgeon using the Gea knot (group A) were reviewed and compared with 63 LA procedures performed by two other surgeons (group B) using the linear stapler. The GESK is…

Is early laparoscopic appendectomy feasible in children with acute appendicitis presenting with an appendiceal mass? A prospective study.

BACKGROUND: The management of an appendiceal mass remains controversial with 2 schools of thought; early surgical intervention vs nonoperative management with or without interval appendectomy. The aim is to determine the role and safety of early laparoscopic appendectomy (LA) in children with acute appendicitis presenting with an appendiceal mass. METHODS: This is a prospective study of 88 consecutive pediatric patients who underwent attempted LA for suspected acute appendicitis at KK Women's and Children's Hospital, Singapore, between May and October 2003. RESULTS: A total of 88 patients wi…

Delayed residual intra-abdominal abscess with threatened preterm labor after laparoscopic appendectomy in the second trimester.

Acute appendicitis is the most common non-obstetric surgical emergency in pregnancy. We report a 30-year-old primigravida at 25 weeks' gestation who underwent laparoscopic appendectomy for appendicitis with localized peritonitis. Thirteen days later, she represented with worsening lower abdominal pain, vomiting, leukocytosis, vaginal bleeding, and uterine contractions. Ultrasound showed a right lower abdominal collection and a viable 27-week fetus. Magnetic resonance imaging demonstrated a peri-cecal abscess adherent to the uterus with associated intraperitoneal fluid suspicious for purulent p…

Laryngospasm-Induced Negative-Pressure Pulmonary Edema After Laparoscopic Appendectomy in a Young Adult: A Case Report.

Negative-pressure pulmonary edema (NPPE) is an uncommon postoperative form of noncardiogenic pulmonary edema resulting from forceful inspiratory efforts against an obstructed upper airway. A 20-year-old man (62 kg, 172 cm; body mass index, 21.0 kg/m²) underwent laparoscopic appendectomy under general anesthesia. Preoperative oxygen saturation was 98% on room air. Immediately after extubation, laryngospasm occurred, followed by acute respiratory distress, oxygen desaturation to 74%, tachypnea at 35 breaths/min, and tachycardia at 125 beats/min. Arterial blood gas analysis showed pH 7.33, PaCO₂ …

Postoperative Antibiotics after Appendectomy for Acute Appendicitis: No Evidence of Benefit, Even With Intra-abdominal Fluid.

OBJECTIVE: To evaluate the necessity of postoperative antibiotics following appendectomy for acute appendicitis (AA), particularly in patients with intra-abdominal fluid, and to identify factors associated with postoperative infections. BACKGROUND: Postoperative antibiotic use after appendectomy remains controversial, especially in the presence of intra-abdominal fluid. While some surgeons prescribe antibiotics empirically, there is no consensus on whether they reduce the risk of postoperative infections in patients with intra-abdominal fluid accumulation. METHODS: A retrospective study was …

Implementation of artificial intelligence-based computer vision model in laparoscopic appendectomy: validation, reliability, and clinical correlation.

BACKGROUND: Application of artificial intelligence (AI) in general surgery is evolving. Real-world implementation of an AI-based computer-vision model in laparoscopic appendectomy (LA) is presented. We aimed to evaluate (1) its accuracy in complexity grading and safety adherence, (2) clinical correlation to outcomes. METHODS: A retrospective single-center study of 499 consecutive LA videos, captured and analyzed by 'Surgical Intelligence Platform,' Theator Inc. (9/2020-5/2022). Two expert surgeons viewed all videos and manually graded complexity and safety adherence. Automated annotations wer…

Incidental Appendiceal Mucinous Neoplasm Found During Appendectomy in a 15-Year-Old Patient: A Case Report.

Appendiceal mucinous neoplasms (AMNs) are rare gastrointestinal tumors, often underdiagnosed due to their variable presentation. Low-grade appendiceal mucinous neoplasms (LAMNs) are particularly significant because of their association with pseudomyxoma peritonei (PMP), a condition that increases the risk of abdominal recurrence. This report presents the case of a 15-year-old female with no prior medical history who developed nonspecific abdominal symptoms. Imaging revealed features consistent with appendicitis, leading to a laparoscopic appendectomy. Histopathological analysis confirmed a low…

[A Case Report of Appendiceal NET G1 Diagnosed by Histopathological Examination after Appendectomy for Acute Appendicitis].

A 36-year-old woman was diagnosed with acute appendicitis after a close examination of her abdominal pain and nausea. Laparoscopic appendectomy was performed, and pathological examination revealed a NET G1. They were localized lesions on the tips and body of the appendix, with no additional resection because the tumor size was less than 2 cm and no risk factors for recurrence and metastasis(vascular invasion, NET G2 or higher, or invasion of the mesentery). The patient was followed up with contrast-enhanced CT every 6 months and has been recurrence-free for 2 years postoperatively.

Applying machine learning to consumer wearable data for the early detection of complications after pediatric appendectomy.

When children are discharged from the hospital after surgery, their caregivers often rely on subjective assessments (e.g., appetite, fatigue … activity, heart rate, and sleep, in detecting abnormal recovery early in children recovering after appendectomy. One hundred and sixty-two children, ages 3-17 years old, who underwent an appendectomy (86 complicated and 76 simple cases of appendicitis) wore a Fitbit device on their wrist for 21 days postoperatively. Abnormal recovery events (i.e., abnormal sy …

Postoperative narcotic utilization in single incision laparoscopic-assisted extracorporeal appendectomy (SILEA): a single-institution retrospective review.

PURPOSE: Laparoscopic appendectomy for pediatric acute appendicitis is commonly performed by pediatric surgeons. A single incision approach has been investigated, but the lack of technical uniformity has resulted in conflicting data. We hypothesized that an initial single incision laparoscopic-assisted extracorporeal appendectomy (SILEA) approach is associated with similar complication rates as compared to the standard three-incision laparoscopic appendectomy (TILA). METHODS: Approximately 1300 laparoscopic appendectomies were retrospectively reviewed for acute appendicitis over a 5 year peri…

Unusual histopathological findings in appendectomy specimens with clinical diagnosis of acute appendicitis: A retrospective cohort analysis.

INTRODUCTION: While appendicitis is considered one of most common acute surgical conditions, several studies have reported abnormal histopathological findings in appendectomy specimens; however, sending all appendices to histopathology is not yet routinely done. Here we report many unusual findings. Those unusual findings played a role not only in confirming acute appendicitis as a cause of the presentation in some cases but also discovering etiologies that mimic it with great impact on its management. METHODS: Between January 2011 and December 2017, a total of 1510 patients were operated with…
