A young woman who was experiencing repeated convulsions was admitted. The patient's brain magnetic resonance image revealed reversible posterior leukoencephalopathy. Blood pressure fluctuated at times to more than 200 mmHg, and the measurement of the right and left upper arms differed by approximately 70 mmHg. Enhanced computed tomography revealed stenotic lesions of some arteries including the left renal artery. Such findings led to an initial diagnosis of Takayasu arteritis and hypertensive encephalopathy caused by renovascular hypertension. A percutaneous transluminal renal angioplasty was …

Giant cell arteritis (GCA) is the most common systemic vasculitis affecting people over 50 years. This disease is a diagnostic challenge with a range of clinical symptoms and findings due to different affected vessels. Because of this, the initial diagnosis can be tricky, and some of the patients present at first time with a real unusual initial manifestation. One of these can be tongue necrosis, which is according to the literature in accordance with scalp necrosis, the rarest initial manifestation of GCA We describe a patient who presented with tongue necrosis as initial symptom of GCA. The …

A 51-year-old woman presented with crusting rhinitis, bilateral serous otitis, inflammatory arthralgias, fever, weight loss and signs of temporal arteritis. Temporal arteries were increased in size, painful, with inflammatory signs. There was microscopic hematuria and inflammatory parameters were increased. The renal function was normal. Anticytoplasmic neutrophils antibodies were detected (anti-PR3). Temporal artery biopsy did not show signs of giant cell arteritis. A diagnostic of Wegener's granulomatosis was established and steroid treatment allowed disappearance of clinical and biologic fe…

BACKGROUND AND OBJECTIVE: The aim of this study was to investigate the accuracy of color Doppler-ultrasonography (CDU) compared with biopsy for the diagnosis of temporal arteritis (TA).
PATIENTS AND METHOD: Twenty-three patients with suspected TA on the basis of clinical criteria were evaluated with CDU prior to temporal artery biopsy. The presence of a hypoechoic halo, suggesting edema of the inflamed vessel, and inflammatory stenoses were registered. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and eficiency or global test value (GTV) were evalu…

Ischemic colitis is one of the most common intestinal ischemic injury in which more than 90% of patient are over 60 year-old. It results from impaired perfusion of blood to the bowel and is rarely caused by vasculitis such as systemic lupus erythematosus, polyarteritis nodosa, and Takayasu's arteritis. Takayasu's arteritis affects the aortic arch, medium-sized and large arteries but rarely involves inferior mesenteric artery. We report a case of Takayasu's arteritis involving inferior mesenteric artery which developed ischemic colitis in a 70 year old female. To the author's knowledge this is …

We report on a 29-year-old male patient with collapse and recurrent vertigo. Physical examination showed a bruit of the left common carotid artery on auscultation and a pulselessness of both arms. The diagnosis of Takayasu's arteritis was based on the results of duplex sonography, MRI and angiography (stenosis and occlusion of extracranial arteries). Laboratory results did not reveal any specific changes. Combined treatment with prednisolone and azathioprine was initiated. After 12 days we saw an improvement of duplexsonographical and laboratory findings. Takayasu's disease should be considere…

PURPOSE OF REVIEW: Takayasu arteritis remains a therapeutic challenge. In spite of current treatments, progression of vascular lesions is observed frequently. The purpose of this article is to describe advances in therapeutic strategies for Takayasu arteritis.
RECENT FINDINGS: Immunosuppressive agents including methotrexate, mycophenolate mofetil, and azathioprine added to corticosteroids can bring Takayasu arteritis into remission in many patients. Unfortunately, relapse is common when prednisone is tapered to dosages of 15 mg/day or less. A better understanding of pathogenesis has lead to t…

A woman with Takayasu arteritis is reported who presented with constitutional symptoms and persistent thrombocytosis documented since 3 years before the diagnosis. Disease-specific symptoms such as arm claudication, transient loss of vision, and self-remitting eye ptosis present at the time were apparently missed, because she is a non-English-speaking Hispanic woman whose history was obtained through an interpreter. Extensive workup done at the time failed to reach a definite etiology. A computed tomography scan of the chest done because of midthoracic back pain and an elevated erythrocyte sed…

Takayasu arteritis is an uncommon disease with a variety of presentations. We report a case of Takayasu arteritis with a presentation of a pulmonary-renal syndrome in a 22-year-old woman. She presented in acute respiratory failure with hemoptysis and acute renal failure; interestingly, however, the renal biopsy was normal. Magnetic resonance angiography (MRA) showed significant narrowing in the distal abdominal aorta with bilateral renal and common iliac artery occlusions. Thoracic and abdominal angiogram confirmed MRA findings of type IV Takayasu arteritis. Percutaneous transluminal angioplas…

A 29-year-old patient presented with Takayasu's arteritis which was revealed by heart failure, epilepsy, right hemiparesis and fever. Transient abnormalities of MRI and CSF (raised protein and cell content) were initially observed. The hypothesis of a hypertensive encephalopathy is suggested.

Temporal arteritis, the most common form of systemic vasculitis in adults, is a panarteritis that chiefly involves the extracranial branches of the carotid artery. The condition is illustrated in this article by the case of a 79-year-old woman with a dry cough, toothache, tongue infarction, and vision loss. The mean age of onset is 72 years and the disease rarely occurs in persons younger than 50 years. The most common presenting manifestations are headache, jaw claudication, polymyalgia rheumatica, and visual symptoms. Eighty-nine percent of patients have an erythrocyte sedimentation rate gre…

OBJECTIVE: To evaluate the incidence and characteristics of musculoskeletal manifestations in polymyalgia rheumatica (PMR) and temporal arteritis (TA).
METHODS: The records of 163 cases of PMR or TA diagnosed over a 15 year period in one area of Spain were reviewed for the presence and type of musculoskeletal manifestations.
RESULTS: Of 163 patients, 90 had isolated PMR and 73 had TA. Eighteen of the 90 patients (20%) with isolated PMR developed distal peripheral arthritis either at diagnosis or during the course of the disease. When it occurred, synovitis was mild, monoarticular or pauci-ar…

BACKGROUND: Failure of response of giant cell arteritis (GCA) to corticosteroid therapy has invariably been attributed to the delay in diagnosing the disease or the use of inadequate corticosteroid dosage. Following our observation of progressive deterioration following the introduction of prednisolone use in a patient, we examined the possibility that worsening of the condition might be due to corticosteroid therapy rather than coincidence.
OBJECTIVE: To determine whether corticosteroid therapy may exacerbate GCA. DESLGN: Case report and an analysis of similar cases reported in the medical l…

Polymyalgia rheumatica (PMA) and temporal arteritis (TA) are common clinical syndromes that affect the elderly population. Both syndromes may include similar constitutional complaints, an increase in acute phase reactants, a rapid response to corticosteroids, and the presence of anteritis giant cells upon temporal artery biopsy. Differential diagnosis is one of exclusion. A case presentation is included to assist practitioners in recognizing presenting symptoms, identifying diagnostic testing, and proceeding with appropriate therapy and follow-up.

Giant cell arteritis and polymyalgia rheumatica are closely related disorders that affect persons more than 50 years of age and cause substantial morbidity. Patients with giant cell arteritis typically have a localized headache, nonspecific systemic symptoms, temporal artery tenderness and a high erythrocyte sedimentation rate (ESR). The diagnosis is confirmed by characteristic pathologic findings on temporal artery biopsy. Patients with polymyalgia rheumatica usually have similar nonspecific systemic symptoms, proximal muscle pain and stiffness, and an elevated ESR. The diagnosis is based on …

Only two years separate the initial descriptions of polymyalgia rheumatica (PMR) by Bruce in 1888 and giant cell arteritis (GCA) by Hutchinson in 1890. However, the existence of an association between these two conditions was definitely accepted only in 1964. Even if PMR is generally accepted as a different disease from GCA, some authors deem PMR to be a manifestation of a generalised arteritis and use the term GCA to define PMR/GCA as a whole. The time has come to clarify this issue.

In this report a woman with giant-cell arteritis presenting as an orbital pseudotumor is described. Other causes of orbital pseudotumor were excluded in this patient. The pathogenesis of orbital pseudotumor caused by giant-cell arteritis is discussed. The concurrent arteritis of the superficial temporal artery and the rapid resolution of signs and symptoms after prednisone therapy supports the causal relationship between orbital pseudotumor and giant-cell arteritis in this patient.
