We developed a rapid and accurate method for quantifying total and differential white blood cell (WBC) counts by pretreating synovial fluid with hyaluronidase and using an automated hematology analyzer. Forty-seven samples of synovial fluid that had been placed in blood-collection tubes containing ethylenediamine- N,N,N',N' -tetraacetic acid as an anticoagulant were treated with hyaluronidase at 37 degrees C for 10 minutes, and then the total and differential WBC counts were determined by means of the automated hematology analyzer. Infections due to pneumococci are frequent in patients infected with the human immunodeficiency virus (HIV), but joint infections are rare. We observed two cases of septic arthritis due to pneumococci in two non-haemophilic HIV seropositive patients. The authors report the case of a young 17 year-old girl with acute systemic lupus erythematosus who presented with purulent arthritis due to Group A hemolytic streptococcus in the knee, and perhaps other joints. OBJECTIVE: In order to fit with National Institute for Health and Care Excellence (NICE) guidelines on arthritis, we established a 'very early arthritis clinic' (v-EAC) to evaluate patients within 48 h of a medical referral for any suspected arthritis of <1-month duration. We describe the characteristics of patients referred to the v-EAC from 2 October 2023 to 1 October 2024.
METHODS: Delay of referral was recorded. A systematic and standardized clinical, biological and ultrasound evaluation of all consecutive patients was conducted. Patients and disease characteristics are described.

We report a case of disseminated sporotrichosis in a 49-year-old man who was treated with multiple immunosuppressants, including tumor necrosis factor (TNF)-alpha antagonists (etanercept and infliximab), for presumed inflammation arthritis. Felty's syndrome is a rare but serious extra-articular manifestation of rheumatoid arthritis. Morbidity as well as mortality are increased on account of greater susceptibility to infectious agents. We report on two patients suffering from Felty's syndrome who were successfully treated by cyclophosphamide. A review of the literature with special regard to treatment of Felty's syndrome is given.
