Speech production in Parkinson's disease: I. An electropalatographic investigation of tongue-palate contact patterns.

Previous studies have indicated that consonant imprecision in Parkinson's disease (PD) may result from a reduction in amplitude of lingual movements or articulatory undershoot. While this has been postulated, direct measurement of the tongue's contact with the hard palate during speech production has not been undertaken. Therefore, the present study aimed to use electropalatography (EPG) to determine the exact nature of tongue-palate contact in a group of individuals with PD and consonant imprecision (n = 9). Furthermore, the current investigation also aimed to compare the results of the parti

Velopharyngeal function from the age of three to eight years in cleft palate patients.

The purpose of this investigation was to study changes of velopharyngeal function between the ages of 3 and 8 years. The subjects were 65 (30 girls and 35 boys) Finnish-speaking non-syndromic children with isolated cleft palate (CP, n = 35) and with unilateral cleft lip and palate (UCLP, n = 30) operated primarily at the age of 1.0-2.0 years. Before the age of 8 years, 16 children required velopharyngoplasty (VPP, ad modum Hoenig). The children were followed up for speech at the age of 3, 6 and 8 years. The perceptual speech characteristics nasal air emission, hypernasality, weakness of pressu

[Anarthria, progressive apraxia and extrapyramidal syndrome: an uncommon clinical form of corticobasal degeneration? A case studied by HMPAO Tc99m single-photon emission tomography].

A 48-year-old man presented with impaired joint movement and buccofacial apraxia. The disease progressed for six years associating an akineto-hypertonic syndrome, marked anarthria, saliva incontinence, bi-opercular syndrome, bucco-facial apraxia, severe global gestual apraxia and a frontal syndrom. Oculo-motricity and gait were normal. Magnetic resonance imaging of the brain demonstrated fronto-parietal atrophy and HMPAO Tc99 tomography revealed hypoperfusion of the cortex clearly predominating in the left parietal region. These particular findings with predominantly intense joint involvement

Intervention for children with phonological impairment: Knowledge, practices and intervention intensity in the UK.

BACKGROUND: Across the world, research has shown that intervention for children with phonological impairment can be both effective and efficient. However, it has also raised concerns about the translation of this evidence to practice, highlighting questions around clinician knowledge and the understanding of approaches, and the intensity of intervention provided within real-life clinical contexts. AIMS: To investigate the clinical management of phonological impairment by speech and language therapists (SLTs) in the United Kingdom (UK). METHODS & PROCEDURES: An anonymous, UK-wide, online surv

Aphemia as a presenting symptom in acute stroke.

Aphemia is an apraxia of speech characterized by complete articulatory failure in the presence of preserved writing, comprehension and oropharyngeal function and can be the presenting manifestation of acute stroke. The responsible lesion is commonly in the left inferior frontal gyrus or the left motor cortex near the face M1 area. Three patients who developed aphemia due to acute ischemic stroke are described here. All had apraxia of speech due to acute infarct in the left motor cortex near face M1 area. Understanding the underlying speech disorder is crucial in planning the appropriate rehabi

Evaluation of a combined treatment approach for childhood apraxia of speech.

The current study investigated the impact of a dual treatment approach that included stimulability training protocol (STP) paired with a modified core vocabulary treatment (mCVT) on the speech sounds produced by children with CAS. The combined treatment was assessed for changes in consistency and expansion of the phonetic inventories of four participants. The children participated in 20 treatment sessions over the course of 10 weeks. Each session was comprised of 10 minutes of stimulability training and 45 minutes of mCVT that included words that were chosen by the child and his or her parents

Philosophy of research in motor speech disorders.

The primary objective of this position paper is to assess the theoretical and empirical support that exists for the Mayo Clinic view of motor speech disorders in general, and for oromotor, nonverbal tasks as a window to speech production processes in particular. Literature both in support of and against the Mayo clinic view and the associated use of oromotor, nonverbal tasks, is reviewed, along with literature from normal speech production and neurophysiology. This literature is organized and analysed to show that theoretical and experimental support for the Mayo view is weak at best, and that

Speech development in patients with unilateral cleft lip and palate treated with different delays in closure of the hard palate after early velar repair: a longitudinal perspective.

We wanted to find out if different timing of delayed repair of the hard palate in a two-stage procedure had an impact on the speech of 26 patients with unilateral cleft lip and palate (UCLP). The soft palate was closed at the age of 7 months and the hard palate between 38 and 89 months of age. Speech audio recordings at the age of 3 years (baseline, before any repair of the hard palate) and at the ages of 5, 7, and 10 years (the latter obtained at least one year after closure) were analysed. We used standardised speech assessments at routine follow-up and assessment by one external listener. T

The effects of redundant stimuli on visuospatial processing in developmental dyslexia.

The interhemispheric deficit theory of dyslexia postulates that reading difficulties can arise from abnormal communication/collaboration between the cerebral hemispheres. A currently popular way to gather information about interhemispheric processing and integration is with the redundant stimuli task, where participants respond to stimuli presented to the left visual field, right visual field, or both visual fields simultaneously. In neurologically normal individuals, response times to bilateral simple stimulus presentations are faster than response times to a single stimulus in either visual

Surgical treatment of submucous cleft palate: a comparative trial of two modalities for palatal closure.

Submucous cleft palate is a congenital malformation with specific clinical and anatomical features. It can be present with or without velopharyngeal insufficiency. Surgical treatment of this malformation is indicated only when velopharyngeal insufficiency has been demonstrated. This article compares two modalities of surgical treatment for submucous cleft palate. The first includes a minimal incision palatopharyngoplasty, as described in a previous report. The second combines the first technique with additional individualized velopharyngeal surgery (individualized pharyngeal flap or sphincter

Aerodynamic and acoustic characteristics of a speaker with turbulent nasal emission: a case report.

Aerodynamic and acoustic characteristics were determined from the speech of an adult female with mild mental retardation and severe velopharyngeal inadequacy. The speaker's productions of /s/ were characterized by consistent nasal grimacing and turbulent air emission. Aerodynamic assessment estimated the size of the velopharyngeal orifice to exceed 200 mm2 during plosive production. Nasal cross-sectional area was estimated to be 35 mm2 during quiet breathing. Nasometric evaluation indicated nasalance of 63% associated with the "Zoo" passage. Acoustic analysis of the separately recorded oral an

Visual feedback and lip-positioning skills of children with and without impaired hearing.

Interplay between visual feedback and lip-positioning skill was studied in 10 5- to 14-year-old children with normal hearing and 10 with severe to profound hearing impairment. With visual feedback, the subjects in both groups had similar response times and accuracy in matching six visually specified lip separation "targets." Special skill in processing visual information by the hearing-impaired subjects was suggested by higher velocities of lip movement toward the targets and shorter latencies in reaching the goal positions. In the responses of the hearing children, lip-closing movements were

Acquisition of the voicing distinction by profoundly hearing-impaired children.

The ability of profoundly deaf schoolchildren to learn the voicing distinction was studied over a period of 6 months. The children were given daily speech therapy and half of them received, in addition, visual feedback from a microprocessor-based speech training aid - the Fricative and Timing Aid. Their performance was analysed in terms of the acquisition of voiced v. voiceless stop consonants and of their position in a word. Results yielded by perceptual tests showed a substantial increase in the intelligibility of stop consonants at the end of the training programme. Retention scores, i.e. i

Radiofrequency Ablation for the Treatment of Pain Related to Bertolotti's Syndrome: A Case Report.

BACKGROUND: Bertolotti's syndrome is a congenital spinal disorder marked by back pain due to a lumbosacral transitional vertebra, often presenting with sacroiliac joint, groin, and hip pain. This condition results from the articulation of the lumbar 5 vertebra transverse processes with the sacrum, causing both radicular and pseudoradicular pain. CASE REPORT: This is a case report of a patient with bilateral axial low back pain related to bilateral Bertolotti joints who received sustained pain relief after a radiofrequency ablation (RFA) targeting the bilateral L4 medial branch, L5 dorsal ramu

Trauma complexity and child abuse: A qualitative study of attachment narratives in adult refugees with PTSD.

The present study aimed to identify trauma types over the life course among adult refugees and to explore their accounts of childhood maltreatment. A sample of 43 Arabic-speaking refugees with posttraumatic stress disorder (PTSD) attending a treatment context in Denmark were interviewed. Using a "Trauma Coding Manual" developed for this study, trauma types were identified in interview transcripts. In both men and women with Iraqi and Palestinian-Lebanese backgrounds, high levels of trauma complexity and high rates of childhood maltreatment were found (63%, n = 27). A number of concepts and cat

Pure apraxia of speech due to infarct in premotor cortex.

Apraxia of speech (AOS) is now recognized as an articulation disorder distinct from dysarthria and aphasia. Various lesions have been associated with AOS in studies that are limited in precise localization due to variability in size and type of pathology. We present a case of pure AOS in setting of an acute stroke to localize more precisely than ever before the brain area responsible for AOS, dorsal premotor cortex (dPMC). The dPMC is in unique position to plan and coordinate speech production by virtue of its connection with nearby motor cortex harboring corticobulbar tract, supplementary mot

[Maxillo-facial surgery in skeletal Class II: repercussions on the temporo-mandibular joints].

These interventions usually aimed at the correction of the skeletal discrepancy by mandibular retrognatism with an advancement of the distal portion of the mandible after mandibular osteotomy. The position of the condyle is determined during the osteosynthesis with miniplates. Condyles are set back in relation with the supine position of the patient and the weakness of his (her) curarized muscle. All studies show that surgery of mandibular advancement causes a lateral, torque and backward movement of the condyles, all harmful to the condyles. Factors that predispose to condylar resorption are

Patterns of cognitive and fine motor deficits in a case of Dandy-Walker continuum.

Cerebellar vermian hypoplasia in the context of Dandy-Walker complex is a relatively common disorder associated with a variety of cognitive and behavioral deficits in addition to impairment in motor control. Few studies, however, have examined the neuropsychological profiles of children with isolated hypoplasias of the cerebellum. Herein, we report a 6-year-old girl with Dandy-Walker continuum presenting with mild mental retardation and an inability to produce intelligible speech, despite adequate comprehension of single items and simple instructions. She was able to articulate vowels but not
