Advanced Atrioventricular Block in a Young Patient: Pacemaker Implantation Is No Longer the Only Choice.

A 40-year-old woman with symptomatic advanced atrioventricular (AV) block underwent cardioneuroablation (CNA) for the treatment of functional bradyarrhythmia, following the exclusion of reversible causes and intrinsic disease of the conduction system. Selective ablation of ganglia related to the AV node, performed exclusively in the left atrium, restored AV conduction, which has remained unchanged with 2 years of follow-up. CNA is a viable and effective option with low risk for the treatment of functional bradyarrhythmias, regardless of their severity.

Atrioventricular Nodal Re-entry Tachycardia in Identical Twins: A Case Report and Literature Review.

This report details the case of 17 year old identical twins who both presented with paroxysmal supraventricular tachycardia (PSVT). Electrophysiological studies revealed atrioventricular nodal reentry tachycardia (AVNRT) in both twins. Successful but technically challenging slow pathway ablation was performed in both twins. This is the first reported case of confirmed AVNRT in identical twins which adds strong evidence to heritability of the dual AV node physiology and AVNRT. A review of the current literature regarding PSVT in monozygotic twins is provided.

Successful catheter ablation of focal atrial tachycardia from the non-coronary aortic cusp.

We describe a patient with frequent episodes of unusual paroxysmal supraventricular tachycardia. During the electrophysiological examination, the tachycardia was easily induced and terminated by atrial pacing. The earliest activation during right atrial activation mapping was located near the atrioventricular node and the His bundle. However, detailed mapping of the aortic root demonstrated that the local activation in the non-coronary aortic cusp preceded the activation at the His bundle region. Radiofrequency catheter ablation at this site terminated the tachycardia with no complications.

His bundle block.

In patients with narrow QRS complexes, atrioventricular block is usually localized to the AV node, and it is frequently benign. The more distal intra-His bundle blocks are rare, potentially malignant, and difficult to recognize. This case report illustrates how His bundle block can be diagnosed by analyzing the effects of certain maneuvers on the heart rate. Paradoxically, exercise and atropine tend to provoke His bundle block, whereas carotid massage may release it. Implantation of a permanent pacemaker is generally indicated.

Complex dysrhythmias in infants and children.

Dysrhythmias in infants and children are, in many ways, similar to those in adults, yet several important differences exist in their presentation and management. Complex dysrhythmias most frequently encountered in pediatrics include sinus node dysfunction, chaotic atrial rhythm, atrial flutter, supraventricular tachycardia (including Wolff-Parkinson-White syndrome and junctional ectopic tachycardia), complete atrioventricular block (congenital and acquired), and ventricular dysrhythmias (premature ventricular contractions and ventricular tachycardia). Newer approaches to the diagnosis and mana

An autopsy case of congenital complete heart block in a newly born of a mother with systemic lupus erythematosus.

An autopsy case of congenital atrioventricular (AV) heart block is described. A newborn infant of a mother with systemic lupus erythematosus died 10 h after birth. Autopsy revealed hematoxylin bodies in the AV node, central fibrous body, and fibrous annulus of the heart. Also, immunoglobulin G (IgG) localization in the hematoxylin bodies was demonstrated by an immunoperoxidase technique. It is suggested that IgG or immune complexes crossed the placenta and that the immune deposition directly injured the cardiac conduction system, causing AV block.

Association of maternal systemic lupus erythematosus with congenital complete heart block.

Infants born to mothers with disseminated lupus erythematosus occasionally have transient manifestations of the maternal disease. In six infants with congenital heart block born to mothers with systemic lupus erythematosus we postulated a causative relation. In one of the infants a post-mortem study of the conduction system suggested faulty embryonic development of the atrioventricular node with an abnormally thick annulus fibrosus and the effects of early inflammatory changes. Two of the infants had a cardiomyopathy and three, associated congenital heart disease.

A Case of Lown-Ganong-Levine Syndrome: Due to an Accessory Pathway of James Fibers or Enhanced Atrioventricular Nodal Conduction (EAVNC)?

BACKGROUND Lown-Ganong-Levine syndrome, includes a short PR interval, normal QRS complex, and paroxysmal tachycardia. The pathophysiology of this syndrome includes an accessory pathway connecting the atria and the atrioventricular (AV) node (James fiber), or between the atria and the His bundle (Brechenmacher fiber). Similar features are seen in enhanced atrioventricular nodal conduction (EAVNC), with the underlying pathophysiology due to a fast pathway to the AV node, and with the diagnosis requiring specific electrophysiologic criteria. CASE REPORT A 17-year-old man presented with a history

Effect of atrial pacing on ventricular rate during atrial fibrillation. A human study.

OBJECTIVE: This research study tests the hypothesis that atrial pacing near the atrioventricular node during atrial fibrillation can affect ventricular rate. METHODS: In 13 patients, two monophasic action potential catheters were advanced into the low anterior septum (ANT) and the low posterior septum (POST) near the atrioventricular node. After induction of atrial fibrillation, measurement of the excitable gap was attempted at the ANT and POST regions. During atrial pacing, ventricular cycle length (CL) at the longest excitable gap was compared to ventricular CL at the shortest excitable gap…

The electrophysiologic effects of nicardipine hydrochloride in man.

Nicardipine hydrochloride is an investigational antianginal calcium channel blocking agent with potent vasodilating properties. To study its acute electrophysiological effects in man, 12 patients (mean age 66, range 58-84), 6 with normal conduction and 6 with bifascicular block were evaluated. Each subject underwent electrophysiologic studies before and after a bolus and an intravenous infusion of nicardipine titrated to lower the mean blood pressure by at least 10%. The actual drop in mean blood pressure was 17% (p less than .01). The mean bolus was 2.2 +/- 1.2 mg (mean +/- standard deviation

Symptomatic bradyarrhythmias in the athlete-Underlying mechanisms and treatments.

Bradyarrhythmias including sinus bradycardia and atrioventricular (AV) block are frequently encountered in endurance athletes especially at night. While these are well tolerated by the young athlete, there is evidence that generally from the fifth decade of life onward, such arrhythmias can degenerate into pathological symptomatic bradycardia requiring pacemaker therapy. For many years, athletic bradycardia and AV block have been attributed to high vagal tone, but work from our group has questioned this widely held assumption and demonstrated a role for intrinsic electrophysiological remodelin

Line of block: A simple pacing maneuver to distinguish AV nodal from accessory pathway ventriculoatrial conduction.

BACKGROUND: Septal accessory pathways (APs), including concealed APs can be difficult to diagnose using standard electrophysiology (EP) maneuvers. We introduce a simple technique using the "line of block" concept that can potentially added to the list of standard maneuvers in cases where the presence of a septal AP is suspected. OBJECTIVE: This study aimed to determine the efficacy of the "line of block" concept among patients with septal APs using a reproducible and simple technique. METHODS: An exploratory catheter is used to pace the basal right ventricle, and the stimulus-to-atrial inter

Using the right drug: a treatment algorithm for regular supraventricular tachycardias.

Despite the recent advent of and the successful results from catheter ablation, pharmacological therapy is still used by most clinicians as … fect of bundle branch block on tachycardia rate. Most regular supraventricular tachycardias use the atrioventricular node either passively, as in atrial tachycardias or flutter, or actively, as paroxysmal junctional tachycardias. The Sicilian Gambit approach attempted to introduce some rationale in the choice of an antiarrhythmic agent, taking into account tachycardia …

High-degree atrioventricular block in a child with an ANK2 variant and long QT phenotype requiring leadless pacemaker implantation.

Ankyrin-B syndrome, which has been linked to variants in ANK2 and classified as long QT syndrome type 4, is characterised by heterogeneous arrhythmic manifestations, including sinus node dysfunction and conduction abnormalities. Clinically significant atrioventricular block requiring permanent pacing in paediatric patients has rarely been described. We report a 10-year-old boy who presented with recurrent syncope and seizure-like episodes. Electrocardiography demonstrated QT prolongation, and continuous monitoring revealed persistent high-degree atrioventricular block. Genetic testing identifi

Junctional ectopic tachycardia in late period after early postoperative complete atrioventricular block: Messenger of return to normal sinus rhythm? : Explanation with four case series.

Junctional ectopic tachycardia(JET) is a rare childhood arrhythmia originating from the area adjacent to the atrioventricular(AV) node. It often occurs after surgical procedures like repair of Tetralogy of Fallot, atrioventricular septal defect and ventricular septal defect, which are all performed in that area. While AV block (AVB) can occur after JET, it is very rare for late JET occurring after early postoperative AVB to be followed by normal sinus rhythm (NSR). There is no information in the literature related to the pathophysiology of this phenomenon. In this text, we present 4 patients w

Cardiac resynchronization therapy in patients with chronic atrial fibrillation.

Heart failure (HF) and atrial fibrillation (AF), 2 of the most common cardiovascular disorders, often coexist in the same patient, as 1 cond … many of those patients, often a nonpharmacologic intervention is required, such as ablation of the atrioventricular node to create heart block. The definitive role for CRT with or without atrioventricular nodal ablation in patients with AF and HF is yet to be studied in large, well-designed, randomized, controlled clinical trials.

[New aspects of the Kearns-Sayre syndrome].

The case report of the 23 years old female with Kearn-Sayre syndrome started about 15 years ago has some special cardiological aspects. By His band ECG the height of III. degree atrioventricular block was located in the atrioventricular node which is in sharp contrast with all former findings localizing the block into or distal to His band. In the pathogenesis of the unexpected block localisation--besides the muscular degeneration accepted generally in KSS--the recent myocardial infarction of the patient could have a causal role. Electronmicroscopical investigation carried out on the biopsy sp…

[Functional state of the sinoatrial node in patients with intraventricular block].

The function of sinus node (SN) was studied in 101 subjects with intraventricular blocks (IVB) and 30 controls. The patients with IVB showed a significant increase in the corrected time of SN function recovery and that of sinoatrial conduction both before and after medicamentous++ autonomic block, as compared to the controls. In general, signs of SN dysfunction were detected in 47 (46.5%) subjects with IVB. In 18.8% of the examinees, SN dysfunction was due to abnormal autonomic exposures, 27.7% had abnormalities in SN proper or perinodal tissue. ++Tachy-bradycardia was found in 53.5% patients
