Fiscal considerations prompted comparison of cefotaxime (a third generation cephalosporin) with cefamandole (a second generation cephalosporin) for prophylaxis in the surgical treatment of the biliary tract. Three patients with idiopathic perforation of the biliary tract who had been treated by a drainage procedure, had a choledochal cyst during 2-8 years of follow up. In all patients, anomaly of pancreaticobiliary duct was revealed by cholangiography and/or retrograde cholangiopancreaticography. It was strongly suggested that biliary perforation in childhood and congenital choledochal cyst are common in pathogenesis and that an anomalous arrangement may play an important causative role.

Endoscopic papillotomy has been successfully performed in 258 of 267 patients (96.6%) presenting with common duct stones, papillary stenosis, or ampullary adenocarcinoma. A low morbidity was experienced (5%) while mortality was 0.77% (2 deaths). Surgical intervention was necessary in 2 patients. The mean age of the group is 66.4 years (range 26-95 years), while the mean hospital stay remains low at 3 days (range 1-12) days. Strangulation of the stones of the great duodenal papilla is found in almost every fourth patient with a benigh mechanical jaundice. A radical method for removing the stones from the ampulla of the papilla is the transduodenal papillosphincterotomy. The 1-1,5 cm dissection of the papilla is followed by an external drainage of the hepatocholedochus after Holstead or Carr. The nearest results of such operations with strangulated stones are favourable.

One year after total gastrectomy due to gastric cancer, the 50-year-old male patient underwent lateral segmentectomy, since metastatic foci occurred in the outer region of the liver. Three months thereafter, metastatic foci occurred in the right anterior lobe of the liver, causing obstructive jaundice. PTCD was performed, and after repeated anticancer doses of 10 mg of Cis-diamminedichloroplatinum (CDDP) plus 10 mg of adriamycin (ADM) in the tubing, partial opening of the obstructive bile duct was achieved.
