Biofeedback therapy in the colon and rectal practice.

In coloproctology, biofeedback has been used for more than 20 years to treat patients with fecal incontinence, constipation, and rectal pain. It can be performed in a number of conditions with minimal risk and discomfort. However, it does require the presence of some degree of sphincter contraction and rectal sensitivity. Biofeedback can be time-consuming and demands motivation. The purpose of this paper is to review the indications, methodology, and results of anorectal biofeedback in the treatment of these disorders. Mean success rates for biofeedback range from 72.3% for fecal incontinence

Conservative treatment of stress urinary incontinence in women: who will benefit?

The aim of the study was to find out which factors can predict the outcome of conservative treatment of urinary stress incontinence in women. One hundred and four women with stress urinary incontinence were evaluated by recall, and by clinical and urodynamic investigation and were given pelvic floor muscle exercises with or without the use of biphasic low-frequency electrostimulation and visual biofeedback. Two groups could be distinguished. The first consisted of 37 patients in whom conservative therapy proved successful; the second consisted of 67 patients in whom incontinence continued. The

Biopsychosocial approaches to the treatment of chronic pain.

BACKGROUND: Biopsychosocial treatments address the range of physical, psychological, and social components of chronic pain. OBJECTIVE: This review sought to determine how effective unimodal and multimodal biopsychosocial approaches are in the treatment of chronic pain. METHODOLOGY: The literature search identified three systematic reviews of the literature and 21 randomized controlled trials to provide the evidence for this review. RESULTS: The systematic reviews and 12 randomized controlled trials reported on chronic low back pain. Other randomized controlled trials studied fibromyalgia (t

Obesity and cortisol.

Cortisol in obesity is a much-studied problem. Previous information indicates that cortisol secretion is elevated but that circulatory concentrations are normal or low, suggesting that peripheral disappearance rate is elevated. These studies have usually not taken into account the difference between central and peripheral types of obesity. Recent studies using saliva cortisol have indicated that the problem is complex with both high and low secretion of cortisol, perhaps depending on the status of the function of the hypothalamic-pituitary-adrenal gland axis. A significant background factor se

Tension-type headache.

Tension-type headaches, the most prevalent form of headache, are differentiated as being either episodic or chronic. The episodic form is a physiologic response to stress, anxiety, depression, emotional conflicts, fatigue, or repressed hostility. Treatment focuses on the use of over-the-counter or prescribed simple analgesics for pain relief. Successful treatment of the chronic form depends on recognition of depression or persistent anxiety states. Primary care physicians can effectively manage most of these patients with nonhabituating anxiolytic or antidepressant medications; however, referr

Front and rear: the pelvic floor is an integrated functional structure.

The pelvic floor is a muscular structure, pierced by the urologic, genital and distal intestinal tract. This structure is not a frozen but a functional unit. Normal function can thus be replaced by dysfunctions of several kinds, overlapping voiding, sexual, genital and defecatory behaviour. For instance, vaginismus is akin to anismus and vesicourethral dyssynergia. Recent recognition of the medicosurgical consequences of sexual abuse has shown that many urologic, genital and digestive dysfunctions are sort of a body signature of the trauma, which must be dealt with if the symptom is to disappe

Balance retraining in a hemiparetic patient using center of gravity biofeedback: a single-case study.

A single-subject experimental design was used to evaluate the effects of center of gravity biofeedback on retraining balance control in a 72-yr.-old male hemiparetic patient. For both assessment and training the subject stood on dual force-plates enclosed on three sides by a visual surround. A 4-wk. training period consisted of subject-initiated shifts in body weight to control movement of a real-time computer display of the subject's center of gravity. A Sensory Organization Test and evaluation of weight distribution indicated that biofeedback retraining positively affected balance control du

[Electrostimulation in treatment of neurogenic and non-neurogenic voiding dysfunction].

UNLABELLED: The efficiency of transcutaneous and intravesical temporary electrostimulation were evaluated in patients with neurogenic and non-neurogenic voiding dysfunction. Non-invasive electrostimulation was performed in 42 children with voiding dysfunction, at the age of 4 months to 15 years. In 5 children rectal electrode and impulse frequency of 50 Hz was used because of incompetent urethral function. In 9 children with detrusor overactivity rectal electrode and impulse frequency of 5 Hz was used. In 12 girls with discoordinated voiding electrostimulation with rectal electrode was used in

[Urinary stress incontinence: rehabilitation treatment of the pelvic floor].

The authors inspected 112 patients with diagnostic urogynecologic and urodynamic criteria. They suffered from urinary stress incontinence. Forty patients effected rehabilitative therapy of the pelvic floor with visits twice weekly with a method which included: pelvic muscle exercises, biofeedback and functional electrostimulation. The patients were divided into two groups in accordance with the kind of urinary stress incontinence: in the first group there were patients with genuine urinary stress incontinence, in the second group patients with mixed urinary stress incontinence. The results at

Biofeedback re-education of faecal continence in children.

The purpose of this prospective study was to see if pretreatment anorectal motility can predict successful correction of faecal incontinence with biofeedback. Forty-seven consecutive children, aged 5 to 18 years, were treated. They had been treated for idiopathic constipation with faecal impaction, but had remained incontinent (n = 15), had been operated for congenital anorectal malformations of high (n = 19) or low (n = 2) type, or had a number of organic congenital pelvic abnormalities (n = 11). This consecutive series represents our entire experience with biofeedback for faecal incontinence

The effects of biofeedback on carpal tunnel syndrome.

Behaviour modification based on audible electromyographic (EMG) biofeedback signals was used to discourage the awkward hand postures and the exertion of excessive force with the fingers, which are suspected of causing carpal tunnel syndrome (CTS). The null hypothesis was that participation in such a biofeedback programme has no effect on CTS symptoms. Before and during an 8-week study, data were collected from two groups of five female assembly line workers each. The biofeedback group received training and input, while the control group did not. Test results indicated that the null hypothesis

[Spastic movement disorders: non-medicamentous therapies based on neurophysiologic principles].

Lesions of the upper motor neurons cause a spastic syndrome and a central paresis. The consequence of these "plus" and "minus" symptoms on locomotor function is a spastic movement disorder. Strategies of physical therapy for the spastic movement disorders are discussed in relation to research findings in neurophysiology and muscle physiology. The main therapeutic technics are physiotherapy, functional electrical stimulation (FES) and biofeedback training (BFB). Various methodological problems which arise in carrying out and evaluating therapeutic studies are described. Valid prognostic criteri

Behavioral training for post-prostatectomy urinary incontinence.

We treated 20 men with persistent post-prostatectomy incontinence by biofeedback-assisted behavioral training procedures. Initially, scheduled 2-hour voiding resulted in a mean 33.1 per cent increase in urge incontinence, a mean 28.5 per cent decrease in stress incontinence and no change in continual leakage. Subsequently, biofeedback was used to teach selective control of the sphincter muscles and/or inhibition of detrusor contractions. Individualized home practice included a voiding schedule, sphincter exercises, active use of the sphincter to prevent urine loss and strategies to manage urge

Electromyographic biofeedback applications to the hemiplegic patient. Changes in lower extremity neuromuscular and functional status.

The efficacy of EMG biofeedback in improving neuromuscular and functional measures of involved lower extremities in an Experimental Group of chronic stroke patients (n = 7) was examined. Differences in pretreatment-posttreatment measures of the Experimental Group were compared with those of groups of chronic stroke patients receiving no treatment (n = 6), biofeedback treatment of the involved upper extremity only (n = 16), and general relaxation training (n = 8). All examinations were performed in a blind fashion. The Experimental Group showed significant improvement in active range of motion

Self-control, biofeedback, and change in 'psychosomatic' approach.

The new psychosomatic treatment modalities of biofeedback and relaxation techniques have in common an emphasis on self-control or self-regulation. The surging populatiry of the self-control techniques at present may reflect a change in the self-image of medicine, which emphasizes its role as a collaborator of the patient in his self-control of disease processes. Modern concept of psychosomatic medicine emphasizes a systems or field approach to evaluation and treatment rather than on 'psychogenic' causes of physical illness. The self-control treatment modalities form an important part of the ar

Alpha biofeedback therapy in alcoholics: an 18-month follow-up.

In an earlier study on patients with alcohol problems, an experimental group given 10 hour-long alpha biofeedback training sessions showed greater improvement on State and Trait Anxiety scores than did a control sample. In the present study an 18-month follow-up was done on those Ss. The differences between the experimentals and controls in State and Trait Anxiety after 18 months were essentially identical to the differences between them immediately after treatment, which indicates that alpha training had long-range therapeutic effects. A difference between the groups on the Alcohol Rehabilita
