Cardiovascular phenotype in extremely low birth weight infants: long-term consequences.

Low birth weight, a possible consequence of prematurity at birth or intrauterine growth restriction, has been recently recognized as an emerging risk factor to develop cardiovascular (CV) future adverse events. Our aim was to perform a short review about the studies conducted in order to evaluate the CV risk profile of a specific subpopulation of infants suffering from low birth weight, that is those having a birth weight less than 1,000 g, called extremely low birth weight.

Interaction between GSTM1/GSTT1 polymorphism and blood mercury on birth weight.

BACKGROUND: Mercury (Hg) is toxic to both the reproductive and nervous systems. In addition, glutathione S-transferases (GSTs), which conjugate glutathione to a variety of electrophilic compounds, are involved in the detoxification of Hg. OBJECTIVE: In this study we examined the association between prenatal exposure to Hg and birth weight as well as the influence of GST polymorphisms. METHODS: The total Hg concentration in maternal and cord blood was measured from 417 Korean women and newborns in the Mothers and Children's Environmental Health study from 2006 to 2008. Information on birth we…

Lung maturation: the survival miracle of very low birth weight infants.

The increased survival of very preterm infants is generally attributed to improved care strategies. This review develops the thesis that the features of abnormal pregnancies responsible for very preterm deliveries also provide an explanation of why very preterm infants often survive. A normal fetus born at 24 weeks is very unlikely to survive. However, pregnancies that result in deliveries at 24 weeks are generally highly abnormal, and may have been so for prolonged periods prior to the preterm deliveries. Inflammatory or vascular developmental abnormalities resulting in very preterm birth can

Offspring birth weight and risk of mortality from diabetes in mothers.

BACKGROUND: Mothers with lower birth weight (LBW) offspring have been found to be associated with cardiovascular disease (CVD) morbidity, which shares many risk factors with diabetes. Aims To investigate the relation between offspring birth weight and maternal risk of mortality from diabetes. METHODS: A total of 1 400 383 singletons of primigravida registered in Taiwan between 1978 and 1987 were followed to the end of 2006. The offspring birth weight was assessed with and without standardisation for the gestational age of offspring. Cox model was used to estimate the HR of maternal diabetes m…

[Morphological criteria for thymic dyschronias in extremely low birth weight fetuses].

The paper emphasizes the importance of tissue malformations (dyschronias) in the development of immunodeficiency states in extremely low birth weight neonates. Thymuses from 115 fetuses at 22-27 weeks post-conception age were examined by immunohistochemical and electron microscopic studies. Two types of thymic structural changes were identified in fetuses developing during transplacental infection. The differential diagnostic signs of thymic tissue malformations were justified in extremely low birth weight fetuses.

Reducing low birth weight through home visitation: a randomized controlled trial.

BACKGROUND: Poor birth outcomes increase the risk of infant mortality and morbidity, developmental delays, and child maltreatment. This study assessed the effectiveness of a prenatal home-visitation program in reducing adverse birth outcomes among socially disadvantaged pregnant women and adolescents. DESIGN: As part of a larger RCT, this study examined the effects of home-visitation services on low birth weight (LBW) deliveries. SETTING/PARTICIPANTS: Pregnant women and adolescents eligible for Healthy Families New York (HFNY) were recruited in three communities. Eligibility was based on soc…

Twin gestation and neurodevelopmental outcome in extremely low birth weight infants.

OBJECTIVE: The purpose of this work was to compare the risk-adjusted incidence of death or neurodevelopmental impairment at 18 to 22 months' corrected age between twin and singleton extremely low birth weight infants. We hypothesized that twin gestation is independently associated with increased risk of death or adverse neurodevelopmental outcomes at 18 to 22 months' corrected age in these infants. METHODS: We conducted a retrospective study of inborn extremely low birth weight infants admitted to Eunice Kennedy Shriver National Institute of Child Health and Human Development Neonatal Researc…

Is low birth weight a risk indicator for congenital cytomegalovirus infection?

BACKGROUND: Congenital cytomegalovirus infection is currently the leading cause of congenital infection in 0.2-2.2% of live births worldwide leading to variable serious sequalae. The aim of the study was to determine if low birth weight is an indicator of CMV congenital infection evidenced by detecting CMV-DNA in umbilical cord blood at the time of delivery. METHODOLOGY: CMV-IgG and IgM antibodies and CMV-DNAemia were assessed in umbilical cord blood of two hundreds newborns, one hundred of whom had birth weight < or = 2700 gram and/or head circumference < or = 32 cm. RESULTS: CMV-IgM was no…

[Persistent central diabetes insipidus in a very low birth weight infant].

Central diabetes insipidus (DI) is extremely rare during the neonatal period. Most cases of central DI are secondary to a known aetiology. Substitutive treatment with desmopressin is effective with nasal or oral preparation, but doses are variable and must be tailored individually. We report on a case in a very low birth weight infant with an idiopathic central DI during the first month of life. He was successfully treated with oral desmopressin. The treatment was maintained after discharge with low doses desmospressin.

Evaluation of neonatal intensive care for extremely-low-birth-weight infants.

Neonatal intensive care for extremely-low-birth-weight (ELBW, 500-999 g) infants must be evaluated to determine that it is effective, efficient, and available to those who need it. From the late 1970s until the late 1990s in the state of Victoria, Australia, neonatal intensive care has been increasingly effective, with large increases in the long-term survival rate from 25% in 1979-1980 to 73% in 1997, and in the quality-adjusted survival rate from 19% to 59% over the same time. Its efficiency has been relatively high and stable over time, comparing favourably with many other health-care progr
