Phototransduction in a transgenic mouse model of Nougaret night blindness.

The Nougaret form of dominant stationary night blindness is linked to a G38D mutation in the rod transducin-alpha subunit (Talpha). In this study, we have examined the mechanism of Nougaret night blindness using transgenic mice expressing TalphaG38D. The biochemical, electrophysiological, and vision-dependent behavioral analyses of the mouse model revealed a unique phenotype of reduced rod sensitivity, impaired activation, and slowed recovery of the phototransduction cascade. Two key deficiencies in TalphaG38D function, its poor ability to activate PDE6 (cGMP phosphodiesterase) and decreased G…

Blindness and visual impairment from severe midface trauma in Nigerians.

Reports have shown that severe midface trauma causes blindness and visual impairment but information from sub-Saharan Africa is sparse. A retrospective analysis was carried out of patients who sustained trauma to the midface and subsequently lost vision in one or both eyes. There were 25 males and 7 females whose ages ranged from 5 to 65 years (mean +/- SD, 33 +/- 13.3 years). All patients presented between 1 h and 7 days of sustaining injury (mean +/- SD, 1.9 +/- 1.9 days). Road traffic accidents (37.5%) and gunshot injuries (34.4%) were the commonest causes. Of the 32 patients identified wit…

Blindness in the intensive care unit: possible role for vasopressors?

Blindness caused by ischemic optic neuropathy in the hospital setting occurs perioperatively and in critically ill patients, but its etiology remains ill defined. We describe four critically ill patients who developed blindness within 1 mo of one another. Three cases occurred outside of the operative arena. Potential risk factors for the development of ischemic optic neuropathy, such as use of vasopressors, venous congestion, and hypotension, are described.

Sudden blindness in a child with end-stage renal disease.

Continuous peritoneal dialysis (CPD) is the preferred modality for renal replacement therapy in children with end-stage renal disease. Anterior ischemic optic neuropathy (AION) is a rare complication in patients on CPD. AION is characterized by ischemic injury of the optic nerve caused by hypoperfusion of the posterior ciliary arteries. It presents with acute visual loss and disc swelling, without additional neurological findings. We report a 2-year-old child with end-stage renal disease on CPD who developed AION. He was dehydrated and received intravenous fluid on admission. Additional treatm

Partnership and promise: evolution of the African river-blindness campaigns.

This article describes the evolution of the partnership, between various health and developmental agencies, that has sustained the campaign against river blindness in Africa. The international community was oblivious to the devastating public-health and socio-economic consequences of onchocerciasis until towards the end of the 1960s and the beginning of the 1970s. Then a 'Mission to West Africa', supported by the United Nations Development Programme, and a visit to the sub-region by the president of the World Bank culminated, in 1974, in the inauguration of the Onchocerciasis Control Programme…

Blindness as a challenging medical and social problem in China.

China, a country with about one fifth of the world's population, has been gaining a rapid economic growth since 1980s. This country has around 5 million blind people, accounting for 18% of the blind in the world. The major causes of blindness include cataract, corneal diseases, trachoma, glaucoma, vitreoretinopathy and a number of factors contributing to blindness in children. The principal cause of blindness in China was no longer trachoma but cataracts. Vitamin A deficiency remains a public health problem in the under-developed areas. Age-related conditions in China, such as vision loss due …

Acute monocular blindness associated with spontaneous common carotid artery dissection.

The evaluation of patients with loss of vision is common in the emergency department. Central retinal artery occlusion (CRAO) is a potentially reversible cause of acute monocular blindness. When evaluating a patient with CRAO, the potential underlying causes should be considered while simultaneously initiating treatment. We present a case of spontaneous common carotid artery dissection manifesting as CRAO and propose that spontaneous carotid artery dissection be included in the differential diagnosis of CRAO.

Automatic postural response systems in individuals with congenital total blindness.

This study examined the effects of the absence of vision from birth on automatic postural responses to platform displacements during stance. … bjects. The difference in the EMG latencies and reaction times between the two groups suggests that blindness from birth may not affect the spinal stretch reflex, but may affect a volitional act mediated through the motor cortex. There were also no significant differences in the RMS values for postural sway between blind and sighted subjects with eyes open …

Immune mechanisms in Onchocerca volvulus-mediated corneal disease (river blindness).

Infection with the parasitic nematode Onchocerca volvulus can lead to severe visual impairment and blindness. In an effort to characterize the molecular basis for the inflammatory response in the cornea, we have developed a murine model for O. volvulus-mediated keratitis in which parasite antigens are injected into the corneal stroma of sensitized mice. This model reproduces the two main clinical features of human disease, corneal opacification and neovascularization. Histological analysis of corneas from these mice reveals a biphasic recruitment of neutrophils and eosinophils to the central c…

[Transient cortical blindness in EPH gestosis caused by cerebral vasospasm].

The pathogenesis of cortical blindness as a rare complication in severe preeclampsia is still unclear. The case of a women with postpartum blindness is reported in which CT and MRI initially showed cortical and subcortical edema in the parieto-occipital lobes. At this time cerebral angiography and transcranial color-coded sonography (TCCS) revealed widespread cerebral vasospasm. Mean blood flow velocities in the middle and posterior cerebral artery and carotid syphon initially reached 380 cm/s and normalized within 2 weeks. While the patient's vision improved rapidly, follow-up MRI disclosed i…

Residual vision in transition zones in patients with cerebral blindness.

Using high resolution perimetry in repeated sessions, we investigated 27 patients with homonymous visual field defects in order to detect islands of vision within the damaged area, and to determine color- and form-recognition abilities within these zones of residual vision. In most patients we found circumscribed areas within the "blind" field in which the stimulus was detected in about 50% of presentations. Only one patient had an island of vision greater than 5 degrees within the defective area. We also found an area of variable performance between the blind and the intact field. Borders of

[Progress in the control of world blindness and future perspectives].

Worldwide, there are approximately 180 million severely visually impaired people, of whom some 45 million are blind. Cataract remains the major cause of blindness, especially in the less developed countries. Substantial improvements have been achieved in the control of blinding diseases, mainly in respect of onchocerciasis and xerophthalmia. More recently, a WHO alliance for the eradicating of trachoma by the year 2020 has been set up. In Africa, the situation remains critical in spite of significant improvements in the training of eye care personnel and the implementation of new eye care faci…

[Acute blindness caused by fungal infection in chronic myeloid leukemia].

PATIENT: A 35-year-old man had suffered from leukemia since September 1990. A transplantation of bone marrow was carried out in February 1994. He developed a graft-versus-host disease in November 1995. In December 1995 a keratoplasty was necessary because of a perforated corneal ulcer. 17 days later the patient noted a complete loss of vision, first in the left and one day later in the right eye. The optic nerve head was white and the retina looked ischaemic like in central retinal artery occlusion. A hypodensic area was found in the frontal brain reaching up to the optic chiasm in computer to
