A novel approach to reduce blood loss in patients with placenta accreta spectrum disorder.

OBJECTIVE: Cesarean hysterectomy for the treatment of placenta accreta spectrum (PAS) disorders has the potential to be associated with significant blood loss, massive transfusion, and operative morbidity. Two major contributors to blood loss are the hysterotomy and the bladder dissection. We introduce a new surgical technique and hypothesize that developing the hysterotomy with a linear cutter and mobilization of the bladder using a vessel sealing system (VSS) before clamping uterine arteries will lead to a total reduction in blood loss and transfusion rates. MATERIALS AND METHODS: This was

Romanian blood donors screening: is it really necessary and/or mandatory?

Blood services are required to provide the safest possible products, but no transfusion can ever be totally free of the risk of transfusion transmissible infections (TTI). Over the past decade, the risk of TTI through transfusion has been reduced (e.g. 1 in 300 000 for HBV to 1 in 2 million for HIV). With the introduction in 1999 of sensitive and expensive nucleic acid testing (NAT) technology in some countries, the disease transmission rate and the window period have been significantly reduced, but a remaining concern is the chance that a blood donor will be infected and not detected by such

The variant Creutzfeldt-Jakob Disease: Risk, uncertainty or safety in the use of blood and blood derivatives?

It has been long since French physician Jean-Baptiste Denys carried out the first successful blood transfusion to a human being. Using bird feathers as canules, sheep blood was transfused to a young man. The patient died soon after Denys' treatment and Denys was accused of murder. In the XXI century, known as the biotechnology century, we face new challenges in Medicine. New emerging and reemerging diseases, such as Creutzfeldt-Jakob disease (CJD) or "mad cow disease" and its human variant (vCJD), challenge the biosafety aspects of a widely extended and extremely useful technique, that is, the

Seroprevalence of Cytomegalovirus among Voluntary Blood Donors.

BACKGROUND: Primary cytomegalovirus (CMV) infection in immunocompetent host is self limiting infection, leading to latency of virus. However congenital CMV and CMV infections in immunocompromised patients are associated with high morbidity and mortality. Transfusion transmitted-cytomegalovirus (TT-CMV) infection in low birth weight neonate and immunocompromised transfusion recipients is being increasingly reported. Studies recommended transfusion of CMV free or CMV safe blood in prevention of TT-CMV. In this background, the study was undertaken to assess the CMV seroprevalence in blood donor.

Bacterial Contamination in Stored Blood for Transfusion and Associated Infectious Risks Among Patients with Cancer at Mbarara Regional Referral Hospital, in Uganda.

BACKGROUND: Patients with cancer often require supportive transfusion therapy. However, this approach is undermined by the risk of bacterial contamination. This study assessed the bacterial contamination rate, identified the bacterial contaminants and their antimicrobial susceptibility patterns; and the associated infectious risk to patients with cancer at Mbarara Regional Referral Hospital, in southwestern Uganda. MATERIALS AND METHODS: This prospective cohort study used a pre-transfusion aliquot of donor blood or components for blood culture. Positive cultures were identified and their anti

Large volume polymerized haemoglobin solution in a Jehovah's Witness following abruptio placentae.

Severe anaemia, with haemoglobin (Hb) levels < or =3 g dL(-1), is associated with mortality rates of 50-95%. Although accepted transfusion targets have been debated in the literature (Carson et al., 2002; Practice guidelines for blood component therapy. 1996; Consensus Conference. 1988; Hebert et al., 1999), few would argue the risks associated with Hb levels less than 5 g dL(-1) in critically ill patients. In patients who are unable to receive red blood cell transfusions, the utility of Hb solutions is an attractive solution. We describe a Jehovah's Witness patient who exemplifies the marked

Substantial international variation in the cost of blood group and save and crossmatch: A systematic review.

Blood transfusion is one of the most common interventions in hospitals. Blood tests that support this widespread practice are considered routine, such as 'group and save' (also known as 'type and screen'), yet there has been very limited evaluation of the needs for these tests and the associated costs. The aim of this systematic review is to synthesise international evidence on the costs of group and save and crossmatch tests and their component processes. Seventy-two studies published between 2012 and 2023 were identified and data were extracted. Most cost estimates were derived from local in

Pragmatic O-Positive Whole-blood RandoMizaTion in male trauma Patients (POWeR-MTP).

PURPOSE: Hemorrhage is a significant cause of trauma-related death. Low-titer O-positive whole blood (LTOWB) is an alternative to component therapy (CT) [packed red blood cells (PRBC) and fresh frozen plasma (FFP)]. We evaluated if LTOWB reduces transfusion requirement or mortality. METHODS: Adult male trauma activations requiring uncrossmatched transfusion in the emergency department underwent nonblinded 24-hour block randomization to receive uncrossmatched LTOWB or CT in the emergency department (ED). Female patients, children, and known prisoners were excluded. If LTOWB was not available,

Low titer group O whole blood in emergency situations.

In past and ongoing military conflicts, the use of whole blood (WB) as a resuscitative product to treat trauma-induced shock and coagulopathy has been widely accepted as an alternative when availability of a balanced component-based transfusion strategy is restricted or lacking. In previous military conflicts, ABO group O blood from donors with low titers of anti-A/B blood group antibodies was favored. Now, several policies demand the exclusive use of ABO group-specific WB. In this short review, we argue that the overall risks, dangers, and consequences of "the ABO group-specific approach," in

Coagulation management in massive bleeding.

PURPOSE OF REVIEW: To update readers on recent literature regarding treatment of coagulopathy for patients with life-threatening bleeding, highlighting emerging therapeutic options, controversial topics, and ongoing clinical trials. RECENT FINDINGS: Massive transfusion protocols featuring immediate availability of blood products and multidisciplinary communication reduce mortality and conserve resources. There is a growing consensus that immediate administration of plasma and platelet units in a 1: 1: 1 ratio with red cell units reduces early mortality. Lyophilized and recombinant blood produ

Transfusion-Transmitted Cache Valley Virus Infection in a Kidney Transplant Recipient With Meningoencephalitis.

BACKGROUND: Cache Valley virus (CVV) is a mosquito-borne virus that is a rare cause of disease in humans. In the fall of 2020, a patient developed encephalitis 6 weeks following kidney transplantation and receipt of multiple blood transfusions. METHODS: After ruling out more common etiologies, metagenomic next-generation sequencing (mNGS) of cerebrospinal fluid (CSF) was performed. We reviewed the medical histories of the index kidney recipient, organ donor, and recipients of other organs from the same donor and conducted a blood traceback investigation to evaluate blood transfusion as a poss

Socio-epidemiological characterisation of blood donors with asymptomatic Leishmania infantum infection from three Brazilian endemic regions and analysis of the transfusional transmission risk of visceral leishmaniasis.

OBJECTIVES: This study assessed the prevalence of visceral leishmaniasis in blood donors from three endemic regions in Brazil and evaluated the risk of transmission by transfusion. BACKGROUND: Despite strong evidence of the transmission of visceral leishmaniasis through blood transfusion, the real risk, an essential condition for taking effective measures to control this serious disease, has not been determined. METHODS: A multicentre study was performed in highly endemic areas. Candidates eligible for their first blood donation underwent a socio-epidemiological interview, and blood samples

Outpatient blood donor program.

An outpatient donor program using pets owned by employees or clients is a practical and cost-efficient means of obtaining blood for transfusion. Donors can have blood typed and be screened for certain diseases, and blood can be drawn at a convenient time. Components can be prepared and stored. Although such a program requires a commitment to assemble reliable volunteers, the results is a team effort that can be rewarding and assures an adequate blood supply.

[Failures of the issue: how to improve?].

If they are not stopped before transfusion, distribution of blood components (BC) errors can have serious consequences for patients. The study of reported incidents of transfusion chain shows that the risk of failure is important, especially on the steps of patient selection in software, packing and giving the BC to transporter. The analysis of these problems most often state "human" error by misapplication of procedures. These errors are favored by systemic causes: many interfaces with numerous actors in the transfusion process, breaking between different information systems, stress of the em

Granulocyte transfusion therapy.

Granulocyte transfusion therapy has been used as an adjunct to broad spectrum antibiotics in the management of serious infections in granulocytopenic patients. This treatment is based upon the observation that the incidence of infection is inversely related to the absolute granulocyte count in granulocytopenic patients and upon the demonstrated value of replacement therapy of other blood cell components such as platelets for thrombocytopenia. In addition, the occurrence of dramatic clinical improvement frequently noted with only small increases in granulocyte count as the marrow begins to rege

UK approach to assessing assays and filters designed to reduce the risk of transfusion-transmitted vCJD.

Three cases of vCJD transmission by blood transfusion have been reported in the UK, and a fourth case discovered at post-mortem. Modelling has been conducted to predict the number of cases that may occur in the future through transfusion, based on estimates of prevalence, infectivity and susceptibility, and a number of steps have been taken to reduce the risk of transmission. These include deferral of previously transfused donors, leucocyte depletion of all components, importation of plasma for certain patient groups and for fractionation, and the collection of the majority of platelets from s

Adherence to a strict specimen-labeling policy decreases the incidence of erroneous blood grouping of blood bank specimens.

BACKGROUND: To assess the effectiveness of a system of preventing incompatible blood transfusions resulting from the misidentification of patient specimens, a prospective analysis of all blood samples submitted to a laboratory was performed. STUDY DESIGN AND METHODS: Incorrectly labeled specimens (rejected samples) were tested for ABO and Rh type, and routine antibody screens were performed. Test results were compared to historic patient data or patient data obtained from subsequently submitted (correctly) labeled specimens. For comparison, all discrepant serologic results from appropriately
