Valve-sparing aortic root replacement remains challenging for patients with eccentric severe aortic regurgitation when the cusps are totally asymmetric. We report a case of successful reimplantation operation for such a lesion. Because free margin lengths from commissure to Arantius body of the adjacent two cusps were matched while inter-commissural distances were totally different, right coronary cusp prolapse was corrected by asymmetrically aligning the position and height of three commissures without cusp plication.

A 52-year-old man with idiopathic diffuse esophageal spasm and hypertensive lower esophageal sphincter presented with dysphagia for several years. After unsuccessful therapy with forceful pneumatic dilation of the cardia, a myotomy of the cardia and distal esophagus was performed. The patient became asymptomatic, lower esophageal sphincter pressure diminished to less than 10 mm Hg, and esophageal body motor activity was normalized. This situation remains unchanged 6 years after the operation.

BACKGROUND: Intralenticular metallic foreign bodies may be well tolerated for many years.
CASE REPORT: A 24-year-old caucasian man was referred with an intralenticular metallic foreign body present in the left eye for five days. Following initial treatment with topical steroid and antibiotic, the lens remained clear and visual acuity normal. Two years later the left eye developed an anterior uveitis, with reduced vision. A left phacoemulsification lens extraction with removal of the intralenticular foreign body and insertion of a posterior chamber intraocular lens was performed.

We report a case of calcaneal osteomyelitis caused by a retained foreign body-a coral fragment. The patient presented with nonspecific heel pain 12 years after the initial injury. MRI revealed an intraosseous lesion demonstrating the 'penumbra' sign, consistent with chronic osteomyelitis and abscess formation. CT demonstrated a foreign body embedded within the abscess cavity. Surgical exploration confirmed the presence of a coral fragment. Teaching point: Retained foreign bodies in bone can remain asymptomatic for many years, eventually presenting with signs of osteomyelitis.

Trauma to the periorbital region is common in children. Foreign bodies in traumatic cases may be missed where history, examination, and investigation are inadequate. Here, we report the case of a four-year-old boy who presented with a small forehead scar and left upper lid swelling two months post-trauma. Imaging showed a retained foreign body, and a pen tip was successfully removed surgically from the lid. The child recovered well post-operatively. Complications such as orbital cellulitis may arise if the foreign body remained undetected.

UNLABELLED: Foreign bodies get into the bladder by self insertion, migration or as a component of iatrogenic and penetrating injuries. Diagnosis and treatment pose great challenges to the urologist especially if they are radiolucent. A common pattern of presentation is with lower urinary tract symptoms. High index of suspicion is required in patient with lower urinary symptoms. We hereby report 5 cases of foreign bodies in the urinary bladder, 2 (40%) self inflicted, 2(40%) iatrogenic and 1(20%) due to migration of the object; as well as their successful management.

Foreign body ingestion/aspiration episodes are potential complications in all branches of dentistry. The handling of small orthodontic components requires particular care, especially where the patient is supine or semi-recumbent. Three cases of foreign body ingestion are presented, involving patients undergoing orthodontic treatment. Once the foreign bodies had been located, all instances were treated using a combination of serial radiography and 'watchful waiting'. All patients remained asymptomatic during this period, although none of the foreign bodies were retrieved. No active intervention
